Staging is an important component of good prostate cancer care. The PCRI Staging System is a user-friendly way to categorize prostate cancer information. In contrast to other staging systems that are designed for doctors, this staging system helps patients find relevant practical information about which treatments apply to their case. To do this effectively, it is important to learn all the information that pertains to one’s case, and not waste time and energy trying to sort through the overwhelming amount of information that is available. The goal is to help patients get more from their interactions with their doctors by being informed and being able to forge the basics, getting to deeper discussions in less time.

The stage is determined by information input into a simple 6 question quiz. The information can be readily found in a patient’s medical records. The key components, are PSA, Gleason Score, Clinical Stage, and Imaging Scans.

The Five Stages of Prostate cancer are Sky, Teal, Azure, Indigo, and Royal. Each refers to a specific disease type that behaves radically different from each other. So distinct in fact, that the treatments aren’t even the same. The first 3 stages are newly diagnosed stages. Azure being the stage that behaves most similarly to other cancers. Sky, on the other hand, is scientifically shown not to spread, and thus treatment is too toxic to justify. Teal is in between where treatment may is most likely necessary, although the treatment regimen is typically less intense.
The last two stages, Indigo and Royal refer to PSA relapse after treatment, and advanced cancer respectively. Each has a wide variety of effective treatments

I continue to appreciate the information from this source. I have 3-4 intermediate. After educating myself through the PCRI resources, I opted for radiation and hormone treatment. Ninety days after radiation and 3rd lupron shot, my PSA is .5 from 24.5. Feeling blessed.

Had a 21 PSA. Fortunately had prostate removed just before it was getting ready to metastasize. Been six months and doing well. Working on getting sexual function back.

I am 68 y/o, PSA 6.6, volume of prostate 32 cc. Under active surveillance. Diagnosed with T1c, Gleason 3+3 cancer in Sept. last year. My urologist said it was a T1c stage cancer based on DRE. However, I am worried that it might be T2b or T2c because the lesion shown in the MRI is 2.5 cm diameter. Volume of lesion 8 cc at the apex. NCCN Guidelines for patients states “T1 tumors can’t be felt during DRE and aren’t found on imaging tests ….....….” Therefore, it might not be T1c based on DRE that is subjective, only part of the prostate is in contact with the rectal, not the whole prostate can be felt. I consider MRI imaging to be more reliable because ' seeing is better than guessing'. I think my case is in the gray area. My urologist says he does not interpret MRI. It is a matter of life and death to me. I might miss the opportunity to cure the cancer and suffer from great pain if it becomes advance stage cancer. After watching your video, I think there might be an opportunity for me to make sure of my Tumor Stage. I think it is no harm asking for help to confirm it so that I can at ease continue active surveillance. July 2023, MRI findings: Lesion 1, ROI 2, 2.5 cm diameter within the left anterior peripheral zone at the apex. Bulges the capsule. T2: circumscribed homogeneous moderate hypointense focus/mass; ??1.5 cm greatest dimension; PI-RADS 5; Lesion 2 - RA lateral peripheral zone PI-RADS 4. Aug 2023, random biopsy findings: 1/14 cores (1 mm in 11 mm core); Stage 1 Gleason 6 Prostate Cancer; non palpable T1c Then, I got an MRI guided biopsy after I queries about the two large lesions shown in MRI that there is a possibility that these areas were not sampled during the random biopsy. Sep 2023, MRI guided biopsy findings: Gleason 6 involving 2 of 3 cores (6mm of 13mm core; 3mm of 10mm core) and small focus of atypical glands on another core, suspicious for carcinoma. Gleason 6 involving 2 of 3 cores (6mm of 17mm core; 4mm of 15mm core; 1 mm of 15mm core) Your help in offering second opinion on my Tumor Stage would be very significant to me to decide the way forward. I would ask for radiation treatment if it is not T1c. Your prompt response is greatly appreciated.

An increase in PSA-value need not be cancer . . . In 2001 when I was 55 my doctor recommended that I started having my PSA measured once every year ( normal small blood-sample from the arm ) because in the first year a Prostate ( Pro )-cancer grows it is INCONSPICUOUS i.e. you feel or see nothing ( no blood in urine ). My first PSA value in year 2001 was 5.5 i.e. 0.5 too high ( normal range 3 - 5 ) Yet the doctor said it was OK as long as the value did not rise. Each year until 2007 , my PSA was 5.5 but October 2007 it was 8.5 . January ( winter ) 2008 I started getting trouble by urination. No pains just difficult passage especially in cold weather . . . In winter one can catch a cold and the nose swells and closes. All men over 60 years have several small chronic inflammations scattered in Pro ( I saw my own small nodules with inflammation after my Pro had been removed and sliced in laboratory. These nodules are NOT cancer ). Probably all these nodules swell in cold weather. Spring 2008 a needle-biopsy was made by me ( several hollow needles are stuck one by one from rectum into the prostate. It makes a click for each needle, but no pains ) . Bad luck for me: in two of the eight needles the laboratory found cancer-cells. Good luck was that the cancer-nodules were small and six of eight needles free of cancer . . . I was operated ( the "big" Pro-operation ) by two surgeons in the Planegg Hospital south of Munich some weeks later . . . Many men get IMPOTENT by this operation because they arrive too late, the cancer is in an advanced stage. There are two NERVES creating erection: one leading to the left side of penis and the other to the right side. These nerves can close-off muscles so that blood can flow into penis but not out. These nerves lie on the top surface of the Pro like two ELECTRIC CORDS on a table. If the surgeon can move these "cords" ( and everything else looks normal ) then these nerves can stay while the Pro and all connected lymph-nodes are removed . . . If the surgeon CANNOT move these nerves it is due to ingrowth of cancer tissue and everything has to be removed. Thus those clinics promising that patients will not get impotent when operated in their clinic are lying, this is first ascertained during the operation. There are two phases of Pro-cancer: first and second. In the second phase cancer cells have grown THROUGH the wall of the Pro and are spreading everywhere in the abdomen. The patient will NOT be cured by the operation. Radiation can only be used once otherwise the colon ( which is right behind the prostate ) will develop chronical inflammation. Thus it is an exciting moment when the laboratory-report arrives a couple of days after the operation.

I had a radical prostatectomy 4 years ago at the age of 47 --my pathology report showed 2 tumors both lobes involved --extension outside the gland on both sides ,extensive perineural invasion and seminal vesicle invasion but only Gleason 6 and no Psa rise since.

I had a similar experience except I had bladder neck invasion too. However I had no lymph node invasion. After one year and five PSA tests, my PSA dropped to “0”. Time will tell.

@@ShubhamGupta-md5ps I had a Radical Prostatectomy and was placed on a new Trial program which offered me 6 weeks of radiotherapy and 6 months of hormone therapy but I pulled out of the trial after 1 month so never had Radiotherapy. My Psa has remained undetectable but I have since suffered a minor heart attack and stroke in 2019 but still here and surviving just about

@@sailorman8590 Looking back I feel sometimes the treatment is worse than cancer it takes so much away from you as a man but some men get through it with no problems so it's just the luck of the draw but if I could turn back the clock I would have chosen to delay treatment as I feel my last 9 years have been a nightmare as I can barely walk 20 feet now and have incontinence of the bladder as well as other medical issues.

Gleason 6 prostate cancer has a very low risk of spreading abd causing death. For this reason most men with Gleason 6 prostate cancer are monitored for changes in the cancer and then treated only if the cancer gets worse.

My biopsy results showed cancer on only the right side of the prostate gland. Of those six cores five were a Gleason 6 and one a Gleason 7 (3+4). The six question placed me in the intermediate risk category which I believe is incorrect. I am on AS.

There is a publication by Dr. Lawrence Klotz looking at the outcomes of active surveillance in men with 3+4. In summary, over 10 years, about 15% of the men in the study had their disease spread but the other 85% did just fine. This means it's important you monitor the cancer careful when you choose active surveillance in men with 3+4.

My dad aged 70 diagonised by prostate cancer and glenson score was 3+3 and PSA was around 6.Doctor removed his testical and denied doing prostactomy considering his age.Please let me know,what will be next treatment or how to treat him and cure his cancer.thank u.
