Never the less Paya is acting foolishly and worrying for no reason. The doctor knows Tharn for so long but he still couldn't gain his heart. He's apparently eliminating everyone who's very dear to Tharn (parents, lovers) so Tharn will be in need of a good psycologist and friend to overcome the trauma. Tharn is going to be in danger after being in a relationship with Phaya and trying to save him, while finding out "doctor's" evil doings. Probably Tharn was engaged or spoken to the son of the governor and that's why the "doctor" feels wronged and haunts him.
