In echocardiography? well if there is a suspicious of pulmonary hypertension, the right thing to do would be a right heart catheterisation.

Measurement of the IVC diameter to be done in Left lateral position as much as possible

Some guidelines recommend the left lateral position, some other the supine position... Guidelines are different in every country, just follow your guidelines
