Shortly after takeoff, the pilots of Flight 3378 decided to leave the landing gear fully extended since a malfunction had prevented its proper retraction. @@dx1450 : Yes. Although it's OK to fly to an alternative airport that is within half the distance to the original destination. (Lowered landing gear approximately doubles fuel consumption).

It’s frustrating that every single crash investigation report seems to conclude ‘extreme workload and stress’ almost like a routine whiplash claim. The captain made a number of erratic errors and calculated decisions and this is nothing more than gross negligence.

Mistakes and poor judgement happen when you are sleep deprived and overworked. It's scientifically proven that your brain operates similar to being drunk after not sleeping for a night. I could imagine that jetlag and long flights cause similar problems..

Being overworked creates a stress that reduces judgment and decision making and causes people to take more risks and bigger risks because their judgement is impacted. It's like being sleep deprived.

Why would the A-310 crew decide to make the entire flight with the gear down? That's pretty stupid when all they had to do was turn around, land and see what the defect is. Very poor judgement from the Captain.

That was my thought. But then I saw the rule saying that you can continue as long as you respect the speed limit for when the landing gear is down. The fuel miscalculation was the biggest problem there.

They could safely fly with it. The decision itself isn't stupid. What got them in trouble is that they didn't actually calculate their fuel levels, and the captain ignored the warning signs he wouldn't make it to Vienna

Would the faulty indicators allow them to calculate the rate of fuel consumption, even if it gave the wrong indication of the amount they had? Or would it not even allow that calculation to be made? The Captain wasted too much time getting the FO to try restarting the engines, instead of preparing for the ditching. I hadn’t thought how important unlocking the cockpit door would be! He may well have made a PA and liaised with the flight attendants, but it wasn’t mentioned.

you cant land with fuel tanks full.... too heavy. they would have to dump fuel first and cant do it over populated area.

Something quite similar to the first clip happened recently in my country (Russia). A plane headed for Omsk was experiencing hydraulic issues, and instead of landing at Omsk they decided to shoot for Novosibirsk (which is about 600 kilometers away) because it has a longer runway. But since they were flying with the gear down and were unable to rectract it - lo and behold they ran out of fuel short of Novosibirsk. They were able to successfully land the plane on a field, apparently nobody was seriously hurt.

Kind of amazing how much more fuel is used when the gear is down. I get that it makes the plane less aerodynamic but that is a huge drag effect. Obviously that pilot underestimated it too!

It's not just the drag from the landing gear itself, but also that the plane's airspeed is severely limited with the landing gear down (to give one example, for a Boeing 737 the limit is 270 knots indicated), preventing it from operating in an efficient flight regime.

They could have looked at the fuel flow gauges and see how many miles they were doing in a certain amount of time. That would have showed them roughly what their range would be. Without looking at things like wind etc. Real shame though. There were many big airports where they could have landed. Athens, Dubrovnik, Tirana, Brindisi etc

In the second video...I was shocked to learn what happened to the pilots. In March of 2009, both the pilot and co-pilot were sentenced to ten-year jail sentences Charged with Manslaughter over the death of 16 passengers. I feel sorry for the pilots and those who lost their loved ones who didn't survive. ✝ Edit: *My comment was referring to the 2nd story only.* .

I see what you write but I totally disagree. None of this would have happened if, when realising that the landing gear would not retract, he immediately did what any pilot would do and turn back to the take off airport. It was totally his mistake (allowing for the circumstances of the wrongly fitted parts) and mistakes in life get punished.

I work in airport operations at a large hub airport about 3 weeks ago we had a frontier a320 that departed but the landing gear would not retreat pilot IMMEDIATELY made a go around and arrived back at the airport and a alert 1 was issued. It just amazes me how the pilots completely disregarded the landing gear malfunction and continued to fly in the aircraft in such a handicapped state

The captain was charged for both case: For Flight 3378, the captain was charged for safety issue and result in suspended along with 6 months penalty For Flight 1153, both pilot was charged for praying during the crucial moment, broke the cockpit rules and was fired from the job.

Yeegads...I learned flying jets in the USN, never, NEVER cycle the gear to get an 'up' indication. You run the risk of getting a stub or one down and all up..which is always bad. Cycle to get a 'down', but never cycle to get an Up...

Well, you should also remember that every single mistake has to be made once for you to know about its existence, and every single aviator out there has to appreciate the fact, that most of them have been made by others in the past, so we can profit from their suffering. This accident occurred 20 years ago. There's no way any pilot of any kind of plane would actually make the insane decision to continue a commercial flight while the landing gear is stuck in its down position, nowadays. None of today's knowledge, innovation or technical awareness existed back then, they were literally flying ticking time-bombs, considering the lack of technical availability or structure in these old birds.

Btw don't get me wrong, ain't disagreeing with a single word you commented, neither am I tryna excuse the poor decision making of this captain. He failed in any possible way. The only thing we might argue about is the decision to continue (which must've already been considered mental by the investigators, back in the day), but it's at least understandable considering time pressure

⁠@@PatmofarI thought it was because he could have made the glide to land, but didn’t think he could. But it was a very close call, only a tailwind would have allowed them to make it. Not something to be jailed for! The fuel issue was from the faulty gauge that was installed the night before. Not their fault.

I was shocked to learn that in the ditching there were 16 passenger deaths. It's one of the few places where the simulation may have had noticable flaws; the hull stayed intact in the simulation but in real ditchings, it almost always breaks apart and the pieces tumble. The deaths are by blunt force trauma as much as drowning. It is *incredibly* difficult to ditch a plane and keep it intact; as far as I know it's only happened once.

@@jokers7890 Fresh water is just as hard when you hit it at speed. There have been freshwater ditchings that ended exactly the same way: A broken hull and a lot of dead people. No matter what angle you land at, sooner or later, the nose is going to catch the surface, and at that point the hull breaks up and people are thrown into the water, or banged about enough to kill them. Even the famous Sullenburger landing worked only because the plane's *engines* took the brunt of the impact. One was ripped right off, but they acted as brakes, so by the time the hull hit the water it was almost at a dead stop and only submarined instead of breaking apart.

Yes, they would had made it without issue if they feathereed when the double flame out occurred. Instead they were still trying to restart engines even though they were going into a ditch procedure. Ditching doesn't have a step to include trying to restart engines.

Remember its easy in say what we know now in hindsight. However being in that situation its another thing altogether. In a full motion atr simulator you could get the aircraft to glide the distance. However the simulator pilots knew they needed to glide. The FQI was still indicating fuel in the real aircraft this caused the pilot to believe that fuel was not the issue so they continued to try and restart the engine because the FQI was lying to them. In the simulator if you mess up you have a magic reset button which returns everything to normal. This is very different in real life.

They should have terminated the flight immediately and returned to Chania. The failure of the landing gear could have been caused by another problem on the aircraft.

Just wanted to say Chania, Crete looks to be a wonderful place. I've only seen it from the passenger seat of the Google Map Car, and can say even though I don't fly anymore, it'd be tempting to go on a trip there. So sorry people who are just going for a good time suffer such tragedy.

Your films are excellent btw and addictive. I actually watch them before I fly - strangely calms me! This incident has a familiar scenario with the captain, who later got a suspended sentence, in denial and refusing to admit the full scale of the situation. Why? Because he should have turned back at the start of the flight. I'm starting to notice patterns of behaviour having watched your videos where crew double down on their mistakes.

The common them of survival on all of these emergency landings is to dump fuel. Leave the plane with little to no fuel and the chances of survival increase exponentially due to no major explosion.

Using the FMS..geez, they know how much they have, via fuel flow gauges, how much they are burning per minute, what's needed at their destination..pretty simple arithmetic.

In the second incident, I think the pilots were criticized for not feathering the props. Apparently the plane should have been able to make Palermo, but the props not being feathered produced a heap of drag.

I was an AC fueler in the USA for 5 years... any fuel quanity issue required a calculated fuel load followed by a mechanic physically verifying the load with the mag sticks intalled on the main tanks and monitoring of the fuel added... This was stupid.... 1000lbs of Jet A is 150 US gals...

With that first one, well, at least they reached the airport. But that captain needs to lose his job and his pilot's license... how hard would it have been to turn around and land at the airport & have maintenance check to see why the landing gear won't retract? And then maybe put the passengers on a different plane?

I think the water decelerates the horizontal speed of the aircraft incredibly fast, causing it to breakup and/or backflip, which contributed to their deaths.

You're not tied into your seat like an F1 driver, neither do you wear a helmet...you can't just expect to survive a sudden deceleration from at least 250 kph to a standstill (I don't know the exact figures for an ATR72). You're gonna be extremely lucky if your head doesn't get slammed against the seat in front of you without actually crushing your neck. Sorry to point it out like that, but it ll help you understand how lucky the 23 survivors actually were.

Thanks for the video. In the first incident, why is there a rule that you can continue flying with a landing gear down, as long as you keep to the recommended speed limits? The drag it creates clearly contributes to mass fuel consumption, as shown in the incident. And they managed to make it to Vienna by the skin of their teeth, fortunately for them they were approaching terrain and not more sea. The second incident with the ATR 72 was tragic. And I do feel it when I learn maintenance did not do or check or were incompetent, leading to a crash. A part that wasn't compatible for the ATR 72, are they not labelled?! Maintenance should be briefed regularly, to comprehend how important, if not fundamental, it is to get the job done right, because there are lives at stake. How the ATR 72 just had no chance at all from the outset. It must have been horrifying for those onboard. The captain and FO were jailed for the crash. Why, did they do the maintenance?! The same captain noticed the fault and registered it with maintenance the day before, it should have been maintenance up in court over the crash, not the two pilots.

I remember the beginning of Hapag Lloyd Flug in 1975, just operating 4 Boeing 727 from their homebase Hanover-airport HAJ. I completed a professional internship as an aircraft mechanic at their homebase back then. It was a very good airline, even offered a scheduled flight HAJ-FRA-JFK using the Airbus A310. Their onboard service was very good. Later their operations and aircrafts were taken over by TUI-flight.

According to the internet the Captain and first officer on flight 1153 survived, however the engineer(who was just a passenger trying to help, not crew) did not make it.

For that ATR42, in the episode of Aircrash Investigation of that crash they showed, that if they had known that they were out of fuel, and had immediately started the glide towards Palermo, with feathered props, instead of trying to restart the engines, they would have made it safely to Palermo! But of course they saw on the gauge that there was plenty of fuel on board, however actually the tanks were completely empty.

In the Hapag-Llyod case, were both pilots so fatigued they couldn't function? The captain declined to take action until it was too late. It was the copilot's obligation to recognize that the pilot was, effectively, incapacitated due to ego, fatigue or whatever other reason. It was then the copilot's obligation to remove the captain from command of the aircraft and decisively take control of the jet, declaring an emergency and looking for an airport closer than Vienna. I would have thought that FMS's could allow you to plug in not uncommon anomalies, like stuck landing gear, into its models. If not, perhaps the airline should create a suitable calculator for each model of aircraft. Is that done today, in 2023?

Also, its not like ATC was telling them...ok, we will contact these ships and make sure they are aware you are ditching......and then its like....Come on Guys, its only 20 miles.....surely you can make that....what is wrong with you? .....I mean, I suppose it would help if the Pilots would tell them both engines are down...then maybe that would make the ATC a little more aware of the dire situation as oppose to....what? You can't make it......surely you can....whats wrong with you?" kind of attitude......the other interesting thing about the second tragedy is ....did they even inform the passengers as to what was going down? and how to handle a ditching.....as it seemed like they were only communicating between themselves and the ATC......but I hope there may have been some flight attendants that were aware and prepared the passengers.....anyway, great vids as always and I seem to recall you having done the first one before......as the casualness of the Captain on the low fuel....its almost like....sure, its low, but we will have not problem getting there and if we do run out...no big deal.....almost like he is driving in a car and playing daredevil with the fuel tank gauges.....incredible, although the 1st officer knew what was up.........

Agree. It almost seemed like there was language barrier or some other communication issue with ATC. How many times did they ask them to notify the ships and ATC never responded to that request? Even if you can't do it, at least tell them you can't do it. I thought the same thing about notifying the passengers...didn't hear them make an announcement to prepare to ditch. Maybe the cabin crew took care of that....

Something similar happened in Russia few weeks ago, I believe September 12. Airbus A320. In Siberia, between Omsk and Novosibirsk. Gear down, so pilots apparently miscalculated remaining fuel. They landed in a corn field, I believe 170 total people on board, no fatalities, no injuries! The airplane looks intact on the pictures. Some say the pilots are heroes for MASTERFUL landing in a corn field, others are saying (myself included) they need to be criminally charged for not calculating fuel properly. Obviously they made a mistake, the real question : was this mistake criminal negligence. Of course, the investigation will take time.

With the first plane, everyone lucked out. And I mean luck. Bad decisions by the captain. Even ignored his first officer. Almost didn't make it. But the second plane, the pilots and crew did nothing wrong and people died. Maintenance mistake, putting in the wrong part. Very sad. RIP 16 souls.

The landing gear , producing more drag would have increased fuel usage. I think the captain and co pilot did well to glide that plane to the runway in Vienna.

Ain't that the truth. I didn't realize how many viewers are pilots, and faced the same emergencies, but came thru safely. Also, as to the second situation, I really doubt that the pilots were jailed for praying. One of the comments stated that the pilots are supposed to calculate the fuel on board and not rely on the instruments. If true, then they were negligent and should have been jailed. However, I am one of the few viewers who is not a pilot, so I don't know if that comment was correct.

This is one of the many reasons I won't fly. I have not been on an airplane since 1976. Too many things go wrong. Run out of fuel, mechanical issues, plane has metal fatigue, pilots are inept. I'll keep both feet on the ground.

I dont understand,if theirs anything not working properly,especially right after takeoff,why not cancel the flight and get everyone back on the ground safely? Ive seen so many of these videos,and heard other stories on the news,of the flight crew not taking stuff as seriously as they should,when youre thousands of feet in the air,the room for error is extremely thin,if at all,just abort the damn flight regardless of the issue and take the L on the money lost and fix the aircraft,why risk everyones lives? Plus the financial repercussions are gonna be way more if theirs an accident,I just dont get it

I don't understand why the pilots in the second video were blamed when the wrong parts were the cause of the fuel deficiency. Were they held responsible for not noticing the inconsistency of the readings at the beginning of the flight? Could they have done anything better if they hadn't resorted to praying when the situation looked hopeless as they were landing on the sea?

Best tip to survive ditching that most pilots didn’t know: once the aircraft is above 100 feet from water, u put your hands to your chest and try to be strong enough to stay remain on the seats when the aircraft have touched the water

As a pilot, the decision by the captain to continue with flight 3378 with the gear down over water is astoundingly reckless and irresponsible. The right decision has to be an immediate return to the departure airport, and the follow-up erroneous fuel burn calculations are just further proof of negligent incompetence by a supposed experienced captain who should know the basic fuel burn penalty with the gear down. And where was dispatch? Did they agree with this horrific decision? The crew took the blame, and rightfully so, but there were other participants who should have gotten that plane turned around, regardless of the maintenance ramification or the inconvenience to the passengers. Safety ALWAYS comes first. They were lucky they didn't auger into a condo!

@@Vonononie IIRC several passengers died because they inflated their life jackets before leaving the airplane. They needed to dive down to reach the door but couldn't due to the the inflated vests.

Sorry, but I can’t understand one thing. He makes the calculation using the FMS and the captain concludes that it isn’t possible to reach Hannover… but after you say that the FMS doesn’t take into account the extra drag… so, why he couldn’t reach Hannover? If FMS doesn’t take into account the extra drag, why it was saying that the plane could only reach Munich?

@@jokers7890I think the difference between this accident and ‎US Airways Flight 1549 wasn't so much salt versus fresh water, as the lack of waves in the Hudson River. Also Sullenberger may have been advantaged by the fact he was flying a low-wing aircraft with underslung engines, meaning that the engines took the brunt of the impact rather than the fuselage.

Referring to this video, the pilot in the second video did manage to safely ditch the plane into the water. The plane did not crash. So how come the fact that as many as 16 people did not survive the landing?

Because in reality (unlike in this video) the fuselage broke in two on impact -- successful ditching of an airliner is _extremely_ difficult.
