I think when she says "mamas comatose she cant leave the bed something smells rotten and its starting to spread" is both literal and metaphorical. Like maybe her mother is just severely depressed or maybe she is literally rotting. But theres also the incestuous abuse happening throughout the family. The "rot" is spreading.

By the lyrics it seems like the brother is more of the protector and someone else in the family is abusing her. Could be the father or maybe an uncle. I wouldn’t think a neighbor because then it wouldn’t be incest.

this songs always confused me because at first i thought her brother was the one SA’ing her and manipulating her because it would make sense if she was looking through holes in his door and maybe her being so defensive over him is because he’s the only love she’s ever known, maybe her and her brother are products of incest and the line about something smells rotten and it’s starting to spread is talking about the incest continuing, but then in the second half it sounds like another older man is abusing her and if her brother found out about it he’d kill him, there’s so many ways to interpret it

“I’m not scared of God, I’m scared he was gone all along” Is insane to me it means like I’m not scared to die or If I do I’m not scared of going to hell because I’ve already been through hell (I’m scared he was gone all along) so she’s more scared of experiencing the abuse she faced before again that to face god. Her rapist has more power over her than god himself…
