I think if you are given an unsafe nurse to patient ratio to work with Do not accept that assignment if you can’t give proper care. At the end of the day that facility will throw you under the bus if something goes wrong. Watch out for yourself these days!

Love all of this! Great tips! I take about 4 pens with me to work 2 are my “throw away pens” those are the cheap ones I don’t mind not getting back. I keep my “nice” pens. I called them bubble packs when pharmacy send 3 bubble packs of the same med I would write in sharpie “1 of 3” “2 of 3” that way the next shift knows we have more in the med room. If I had time during restocking the cart for the next shift I would use a rubber band put the next bubble pack of medication so it’s ready. Sometimes it sucks you go out of your way setting up stuff for the oncoming shift but sometimes they won’t do it for you.

Great Video thank you for this and I can see how you have grown in your professional career you have a very good attitude I appreciate how you have embraced your career as a Professional Nurse you are providing really good consistent care I can tell. I've been watching your video's and I've learned a lot but this is one of your all-time best video's for sure great stuff here thanks for these videos. it's good to hear this in case I decide to work in an S.N.F. ( I haven't made up my mind yet as to where I will apply, but I am leaning towards a hospital setting in the EMergency department hopefully being on a Trauma team.

I've been a nurse for almost 10 years now, worked home health for 8, and a detox/psych center for a year. I just started at a nursing home. Finished orientation today and am already overwhelmed knowing what I'm about to get myself into. This video gave me so much relief, and knowledge. Thank YOU ❤

@@yyysseee5370are you talking about pushing away nursing school applicants? Search the literature and you will that the issue stems from a lack of nursing faculty. If there is a no nurses to teach prospective nurses to be nurses, then they have to turn away nursing school applicants. One professor and clinical instructor can only teach a handful of students. We need to focus more on fixing that issue and the other issue will clear downstream. Take care

Dr. Schol’s or Muks’ compression socks are wonderful! Working doubles in MC & then working 12 hours in IPR is no joke! In TX, the ratio for SNF (average) is 10:1…….some have told me average was 12:1 & if facility was short staffed, then it’s 15-20:1, which is totally UNSAFE!

Awesome advice! Love the tips on keeping it short and sweet. I think working in a SNF would build me boundaries of steel because I have tried to bond with patients in my past jobs working in the hospital and infusion and even with those smaller ratios I burned myself out. I'm considering taking a job in SNF and really appreciate your insight. I also heard in my interview that there are sometimes residents with CPAP/Bipap needs. Do you have any advice for/experience with caring for those patients?

Thanks for the info I live in Wisconsin we're short staffed as well , I carry my own pens and I don't hand them out , I keep guarded bc I do work with all ages. Drama big one for us I swear some folks won't do team work !

It’s depends on acuity, but we have 22-30 in TN & Idaho. But in MO it’s 30-35 but I have Med Techs.

Take those breaks! Stay later if you need to, but never sacrifice your unpaid meal!

@@rachela23 girl, LTC's in NJ are hot mess as well. Every time we go for clinical there are always short staff, nurses complaining and pts not being taken care of.

Owners of facilities want to make more money. That's the only and only reason. Because not only short staff but short supplies, internet is very slow, one computer for 2 nurses in a nurse station.

Hello! I have a job which starts in June at a hospital, it’s a residency program. But I also got an offer from a long term care facility recently, should I try it out with them since I have 8 weeks of just being at home? Or just rest and focus on getting into my job in June

I have no problem asking for my pen back lol. I’m the beginning I was so quiet and shy, but you can imagine this job changes you. I refuse to let another pen leave me lol

I looked at the protest of assignment did the RN give you the facility code? and what does code of nursing unit mean? I’m thinking about asking about this on interview at a LTF or is it a bad idea?

Im a new passer of local board exam here in Philippines and no experience ,planning to grab opportunity there in newyork surely be assigned to SNF .despite me graduating decate ago im desperate to work as soon as posible tho im anxios of what im hearing about the patient nurse ratios 1.is the patient ratio be shown in contract between you and employer 2 what is the ideal patient to nurse ratio?is there law where to loook at as basis 3 .can one choose night shift duty the whole months 4 are documentations there are computerized? so it is must that i shall improve my typing skills to maximize time.what is the standard size of keyboards there that i will be familiiarized with .

Hey, I'm a new grad rn/bsn in NYC and see a lot of long-term care facilities hiring for nurse supervisor and floor positions. Do you know what it's like for nurse supervisors? I feel like it would be super overwhelming as a new grad.

I’ll be honest I think it would be overwhelming at first. But with anything after time you’ll be able to handle it. I recommend doing a floor position first (maybe ask wherever you work if you can train the floor first before going into supervisor role). The supervisors at my place either have easy days (no new admissions) so they just do computer work all day (or hardly work lol). Then there’s days where there’s so many new admissions, or if the floor is short they will shift you to do the floor. That’s all in addition to any IVs that need to be started, TPN’s to hang, etc. I know it may sound like a lot but no matter where you go, you will get the hang of it Also if your body can handle it. Night shifts are the best!

Working the floor as an rn is exactly the same as an LPN but you get paid more. So it’s a win for you.

Hello I'm an international nurse preparing for NCLEX, i wanted to know tht RN single handedly should watch for 40/50 patients alone? all the work? won't there be anyone else in the team?

In the long term care facility it depends your role. If you take supervisor you will be in charge for any emergencies or new admission but you will have nurses below you so you won’t be alone. If you are not supervising and take the floor nurse position. Then you alone will be with up to 40 patients, but will be able to call a supervisor if you need something. I’m not sure how it works in the hospital.

At the time of the video I had 38-40 pts some days 18-20. Now I mostly never have more the 28, which still is a lot, but I manage. They only trained me for 3 days 😭😭. Thank goodness I’m resilient.

Many of the nurses I know do doubles everyday at my facility. There are also others that work at the facility next door and they just walk over for the next shift. So it’s definitely possible, but say goodbye to your personal life & expect burnout

From my experience it’s been one to one interviews. Hospital interviews are more likely to be panel.

What's the difference between lpn and rn job in long term facilities? They do the same job but rn make more? It don't make any sense

The RN has the option to be a supervisor. If you go that route you will be doing the initial assessment of all new admissions, creating the care plan, communicating with the doctors and serving as support to the staff wherever needed. You also has the option to work the unit passing meds etc, which in this case your job will be identical to the LPN, but your pay is higher

@@africanyvette2908 well as for new admissions at where I work, the lpn takes a set of initial vitals and can assist the RN in assessment but are not required to. So if your busy after the vitals you can go back to doing what you need to do. I can see how facilities would let the LPN do the entire new admission because they like to take advantage, but that first initial assessment is usually done by the RN. & it’s not because the LPN isn’t capable, but it’s just how the hierarchy works

Yes I’m positive. And most cases it’s 1-28 for me. Other floors are 1-40. It is long term care, so the patients are much more stable then in the hospital
