The Blue Sky Bio Tissue Level implant and UCLA Abutment is an easy solution for cementing restorations when the implant platform is placed too deep.
In an ideal situation, the implant is placed even with the tissue allowing the use of a stock abutment. Sometimes, the implant is placed too deep under the tissue. When this happens the use of a solid abutment and cemented crown could result in cement sepsis. In this situation the UCLA abutment is used to bring the margin of the crown up to the tissue level allowing the crown to be cemented and excess cement to be removed. The Blue Sky Bio UCLA abutment has a long body that can be shaped to the desired size along with a platform that engages with the implant. The abutment has a locking 8 degree taper to help reduce screw loosening. The abutment sits on the outside bevel of the implant. Remove the cover screw from the implant. Insert the impression transfer into the implant. Use driver to tighten screw with light finger pressure. Apply wax to the top of the impression transfer to seal screw access. Inject impression material around the impression transfer. Fill the tray with impression material. Insert the tray with impression material and then seat the impression to capture the dental arch. Remove the impression. Use a sharp instrument to remove the wax from the impression transfer. Remove impression transfer from implant. Insert cover screw back into implant. Insert the impression transfer into the analog and tighten with finger pressure. Insert transfer/analog assembly into the impression. Apply soft tissue replica material. Pour dental stone into the impression. Remove the stone model from the impression and remove the impression transfer. Insert the driver assembly into the Blue Sky Bio UCLA screw within the abutment. Only use Blue Sky Bio screws.
Insert the abutment into the implant level analog, making sure to align the male octagon of the abutment with the female octagon of the implant level analog. Use a bur to shape the abutment to the desired shape. Apply and contour the wax to the desired shape. Cast the under casting. Block out screw access hole with Teflon tape. Wax up the crown coping in the usual manner. Apply the ceramic in the usual manner and complete the restoration.
Remove the Teflon tape and remove the abutment from the stone model. Insert the abutment in the mouth. Use the torque ratchet to tighten to 30 Newton Centimeters. Apply cement to the crown. Block out screw access hole with Teflon tape.
Place the crown into the mouth and remove any excess cement.
