How is the first one not adjustment disorder? I understand why it's MDD but don't understand how it's not adjustment
@hyguruprep
@hyguruprep 2 жыл бұрын
Great question, a quick review of adjustment disorder: Distress that is disproportionate to trigger which occurs within 3 months and lasts ≤ 6 months following the termination of the stressor. In this case, the best answer is depression given the major depressive episode i.e. 5/9 SIGECAPS. As the stem states what is the likely diagnosis -- more data points are related to SIGECAPS and thus this helps solidify MDD as the diagnosis. Here's a Step 1 UWorld QID related to Adjustment: 15348. Hopefully this question will help solidify this concept!! Thank.

54:00 a patient that "says" they don't feel depressed & is showing signs of psychosis/paranoia is probably lying the vignette should say "the physician determines the patient is not depressed", otherwise it should be depression with psychotic features since (hx and) we can assume the patient is lying about their depression merely to end the interview
