Cases with transposition of the great arteries only have a VSD in 40% of cases, not in 'almost all' as stated in this video. A neonate with TGA without a VSD needs communication (mixing) of the 2 'circles'. The level of this communication is the ductus arteriosus and the atrial septum/foramen ovale. Sometimes the foramen is too small. To enlarge this, a procedure is sometimes needed (called a Rashkind procedure)

@@danalmass im 20 years old and had the surgery at 1 week old, i never had any problems from what i know. they just gotta check how its doing bc they might need to fix something (idk what tho lol) but that surgery would be very simple

would the Foramen Ovale also allow for mixing?...in addition to the Ductus Ateriosus and the presence of a VSD

3 years later, but yeah, why not. the more holes the better. there's also a procedure done called like a balloon septostomy, where i think the surgeon basically makes the Patent foramen ovale bigger so there is more mixing of blood. Also, Prostaglandin E (PgE) is given to keep the foramen ovale open as well as the ductus arteriosus open to allow even MORE mixing. Like a mixing melting pot.

Blood flow would be restricted from the pulmonary trunk to aortic trunk via the ductus arteriosus before birth due to the pressure gradient ( higher pulmonary resistance and pressure ), so I assume the connection between the two circuits would be limited. However, after birth, the pressure gradient reverses and you can get blood flow via the shunt but wouldn't it be limited because the shunt would normally close ( or does it not close in this abnormality ? ) If I have said anything incorrect, please correct me as I'm a medical student - Cheers, love Khan Academy :)

i know you asked 3 years ago, but yeah the ductus arteriosus would normally close, when the baby is born. But i think Prostaglandin E (PgE) will be given to keep the shunt open.

Four years ago but keeping the ductus arteriosus patent is like a physiological mechanism to enable survival of the child. Prostaglandins are given to increase the chances of this, but adaptive physiology also plays a great role.

Not very reliable info esp vsd is not seen in almost all of cases plus she didnt mention PDA and ASD where mixing is still going on as in DA

I don't mean to be too picky but its best to call it Transposition of the Great Vessels because its the pulmonary Trunk that's involved, not the pulmonary arteries which are distal to the trunk. I'm sure Amy knows this (however, Amy, you make this mistake in all your videos) but it can be terribly confusing to mislabel structures in surgery especially for those who are learning or confused. From the picture, it's clear what you mean.

My daughter is 6month old she also have TGA,im scared because they told us that the operation is very difficult and 50% only the chance to success for the operation.
