Truck gets lifted as a container is picked up at a container terminal. As can be seen, the lock securing the container to the trailer on the back-right is not released before the crane starts lifting the container. This is the result.

Several years ago i worked at container depo. One of my team just was done finishing stacking 40 feets, decided to take a rest waiting for next batch. He left his sideloader with spare helmet and high-vis jacket on it next to the stack and just start walking when a strong gust topple the top kontainer, falling into the side loader, and just a metre next to him. I heard the loud bang from front gate and along with field-ops and securities run into his side loader, saw smashed helmet and jacket with red liquid all over (turns out friggin hidraulics oil). Frankly i went pale thinking he got smashed - until i found him sitting pale in the kontainer being repaired several meter behind. That day become another reminder that the helmet only there to protect us from falling nut and bolts - not friggin 20/40feets falling.

It's possible that when the head hit the container it shook a twist lock and got it to reattach. That's why you use a spotter and two-way radios. You don't just lift a skel frame 12 feet in the air.

No Shannon , it is completely the truck drivers responsibility to check the chassis for any obstructions regarding the lift , driver needs to stay at all times in the designated safety zones where they can make eye contact with the operator , it was the truck driver who had to double check if his goods are liftable , if not he should signal immediatly , if he can’t he should back off as far as possible and not doing a Clint Eastwood style walkaround around the chassis , these actions will get yourself killed, and no there’s not always a ground operator who checks it , if they don’t care with the material they work with , they should.ve stayed home in the first place . You need to check your chassis more often , entered the port ? Turn them loose or not , driving to crane ? Stop a few meters before and open your locks , standing still too long and you’re relocated to another loading/ unloading spot ? Check em again ....

When we get to the terminal, off the city/county road... First thing we do when we reach the lineup is to get out and unlock. Longshore can see when we pull up to the top strad, side strad, or RTG crane, if the container is unlocked. Once or twice per day I see a chassis rise into the air like this. This will bend the frame of the truck and chassis, resulting in both never being alligned properly. The twist locks may snap or suffer metal stress, rendering them weak and susceptible to failure in the future. This is the fault of the truck driver, not the longshore operator. It is a lesson to us truck drivers to unlock in line outside of terminal operations.

Yup. Junk equipment most of the time. The dogs are opened at entry to the port, but often the distance to the crane is enough to vibrate them closed by the time you get under the crane. That's why you see tape, paper towels, all sorts of debri stuffed into the dogs to make them stay put. The crane operator is probably an angry drunk who thinks his shit don't stink. True if this was Dennison or an SDC trailer ( where you just turn the pins 90%.. but it's a Noteboom trailer the mechanism is a different design you have to unscrew them and when they are unscrewed properly they drop down clear of the corner columns so... so don't rush when you do this with a Noteboom trailer and if in doubt double check. This is a good example why there should be a slingman or a trucker hired to give a signal to the rtg operator. Safety should be the #1 priority of the companies. The crane driver should always float the load after picking up the container until he sees the semi driver pull forward a little. After he sees the semi has pulled forward then he can hoist the load. Lets keep saftey a priority because and injury may cost a life or a lifetime of pain.

I'm doing this job 17 years now The trailer has 4 keys for each corner of container You can't go in too the port if the keys are not opened but some times as the truck is rolling to the lift machine the keys its too sensitive and they turn to the lock position with out driver knowing that The lift operator is too high and he dont have good visibility and this is the result

The driver screwed up because he didn't unlock the twist locks prior to lifting. Additionally, there is supposed to be a ground man with each crane to immediately communicate a problem.
