I am on cellcept for hereditary Irits. It weakens my immune system to fighting off infections. My rheumatologist run blood work for TB and it came back positive. I now have to get a chest x-ray today. Am concerned!

Greetings Doc, Have a question regarding the diagnosis of Tuberculosis. If one individual is PPD or TST positive, rendering a 10mm indentation, but chest xray shows normal result, can that individual be considered to have a Latent Tuberculosis Infection? my other Question is, do Granulomas occur ALWAYS on the chest Xray of a person with Latent Tuberculosis infection?,or an individual might have Latent Tuberculosis Infection without the manifestation of granulomas in the chest X ray, or having a normal CXR result.? +jp yra Hello JP - sorry about the delayed response! In the US, a 10mm induration response to the TST is considered indicative of TB infection (in a patient not previously vaccinated with BCG). If there is a history of contact with a TB infected individual, then the threshold for a positive result is just 5mm. Once a positive test has occurred, it is recommended that a follow up physical exam and chest xray be performed to rule out active TB. If there are no signs of active TB, then the patient is considered to have latent TB infection, and should have a course of treatment. Patients with latent TB have normal chest xrays. I hope this helps...

Nice video I was diagnosed with TB this February came in hospital in really bad shape, spent in hospital about 3 months literally fighting for my life worst experience ever, now after 9 months of recovering and all that drama I had CT few days ago doctor said lungs recovered beyond expectation finally It is over

Hi - pneumonia can damage the lungs, but typically this involves damage to alveolar units and formation of scar tissue in airways without the neo-vascularization and extensive cavitation that occur in TB. I'm almost 60. I had active pulmonary TB in 1971, and took meds for a year. I do have several calcifications in my lungs. I am curious if the TB can re-emerge from this? Should all of be wary or just let it go and forget it?

Great question Jo. Conventional wisdom has been that calcified granulomas do not contain TB bacilli capable of reactivating the disease. However, recent research has found evidence of the presence COMPONENTS of TB bacilli in calcified granulomas, thus raising the possibility that TB may reactivate from such lesions. Its important to emphasize that finding components of TB bacilli is not the same as finding whole, viable bacilli. Further research needs to be done.
