Thanks so much for addressing PSA doubling time. I recently had surgery. Lymph testing found one lymph node with cancer. My first PSA test said my PSA level was undetectable. I’m going to mentor my PSA level every three months for the next two years. I’m hopefully that I can avoid hormone therapy and radiation. All of you interviews have been so helpful!

After 20 rounds of radiation treatment 60GY 2 6X, PSA dropped from before treatment 14.1 to after completion and 3 weeks later PSA was 0.04. 6 weeks later dropped to 0.19. 6 weeks later tick up to 0.59. 6 weeks later it had skyrocketed to 3.02. At that moment, made an appointment to see my urologist. About 3 weeks later when we met it was 4.25. He immediately scheduled a pelvic CT scan and a skeleton imaging scan, plus scheduled me to meet with an oncologist. Took the pelvic soft tissue scan, nothing was visible. Met with the oncologist, he immediately canceled the skeleton scan and scheduled me a PMSA SCAN. He said that would be better and more accurate if it has metastasis. 😢 Not what my options are once text are completed. Oncologist just said lets just wait first for the results. Scheduled for January 31st, that was the earliest I could get in. NOTE: I was the 2nd patient he referred for this scan. Just became available at the hospital I been going to for 6 years. He specializes in metastasis patients and a type of hemoglobin type expert. Not sure what do or look towards

What is the base starting PSA level to compute the doubling time in one who has had treatment, radiation and hormone therapy. Mine is now undetectable after 2.5 years. Gleason 9.

So if on AS for G6 and PSA has stayed basically the same in 1yrs time is that a sign of no change & less likely there is any aggressive higher Gleason elsewhere in the prostate?

My PSA is 9 went down to 8.2 two months later . My dr wanted to do a biopsy when it was 9 then when I asked for another test it went to 8.2 so he said let’s monitor it and another test in April. Hoping it goes down a lot. The reason for the Urologist was I woke up and couldn’t pee finally went to hospital where they put a catheter in, as soon as I was relieved I asked them to take catheter out and haven’t been back since, that was four months ago and peeing just fine. Still wake up at night at least twice but that’s because I drink liquids until 11 or 12 at night . I’m 69 y/o and have an enlarged prostate which may be the reason for the high score. I have cut salt and sugar out completely which may of been the reason I couldn’t pee. The flow is coming back to normal and I eat more greens and fruits cut down on red meat which seems to help

Go straight to biopsy is ridiculous. Where do you live? A MRI should be done. Then if any liesons are present a Biopsy should be done. You undoubtedly have BHP. But a mri is warranted. Find a new urologist .

My father's psa levels went from 5. 3 to 14.9 in about 2 months. Could he have cancer?

Well, that explains the Doctors excitement when I went from undetectable in Aug, to 2.7 the end of April. then 3.2 a few weeks later. Scans so far haven't shown anything... He asked if I had pain, no, and no real symptoms... Requested a PSMA scan getting an aximum scan in a couple weeks. sigh.

Depends on your initial treatment. I opted for removal and went to zero PSA. Two and a half years later PSA reappeared and was slowly increasing. Had the Aximum scan that didn't pick up anything. At the 4 year mark after an MRI and Bone scan (both negative) I opted for radiation (8 weeks) of the prostate bed. Completed that a month ago. Next PSA in three weeks. Staying ahead of the curve is the plan!

Follow up.... PSA after 8 weeks of radiation back to zero!!! I guess they hit the right spot.... life goes on and I continue to monitor my PSA on a regular basis.

@@imxploring Awesome. My second PSA test was a month after the first and it almost doubled from 2.7 to 5.0. I wanted the PSMA scan but was offered an axumin scan and it was not approved. I started ADT right away as I did not want to wait. Glad to say my last PSA was down to 1.8 so the ADT and now the generic of Zytiga is working! good luck!!!

@@steinshaw2490 That's great.... I didn't opt for ADT at the same time as the RT as my doctor had indicated the most likely area of PC reoccurrence, after all the testing was negative, was the prostate bed. I was concerned about side effects and over treating. Better to catch it early and treat it before it reaches the level of being detectable and spreads elsewhere. (Organs/Bones) When dealing with PC you play the odds to some degree... and use the treatments to manage PC if a compete cure isn't obtained as you continue your battle. ADT might be the next tool I have to pull out of the armory should that PSA reappear and treatment is once again needed to beat it back.

Is doubling times (post prostatectomy and radiotherapy) accurate at very low PSA values such as 0.02 ng/dl to 0.06 ng/dl after 3 years?

After Prostectomy PSA was .014 6 months later .191 and now three months later .225 I am 48 seeing my doctor in a few weeks my question do I need to be worried?

Those are becoming more widely available and can detect the location of a recurrence with a PSA as low as 0.2 in some cases. There is a GA-68 PSMA PET scan which is available at some academic institutions like UCLA, UCSF, Cornell, etc., and there is also a commercial version (F18 DCFPyL PSMA PET) that is more widespread and works just as well. My husband has been ill for 4 months or more, fatigued, his psa is 43.5 and keeps rising. He had a biopies recently but nothing found although mri said highly suspicious for cancer. We dont know who to turn to , his uroligist is not concerned about it. Should we be? All his other blood tests have been ok as he went in emergency 3 times recently, he feelslike hes dying he says. Help please!

Doubling Time in days = (T1 - T0) * ln|2|/ln|PSA1/PSA0| The difference in days * 0.69315 / the natural logarithm of ( new PSA divided by previous PSA)

My PSA on 07//11/19 was 2.96 and then on 06/22/20 it's 4.39...should I be worried...should I get a biopsy(which my doctor is wanting to do)...I've never had an biopsy done before. I'm 49 years old....I weight 151ish and in very good shape(compared to weight and height ratio. What should I do???

Within the last year my PSA went from 3.75 to 6.2 after 6 months. I had a a digital test which my Dr said was very normal but he recommended a biopsy due to the rapid doubling of the PSA. I did a biopsy which came out normal. I later read that at age 70 a PSA of 6.2 can be very normal. I regret taking the PSA because it's very invasive and showed tinctures of blood and tiny tissues in my urine for 3 weeks. If you are 70 and the digital test is normal don't worry so much. Do another PSA and digital before any biopsy

@@MariamAhmed-sb8kz I am fine nothing of any abnormality, just the regular 70yr old routine. I rise only once per night to urinate, but I drink natural juices, green tea or water late into the night.

My PSA has gone from 1.1 to 2.85 in a years time. For 2 years it has remained steady between.08 and 1.5. I’m a little alarmed at the doubling time as I am only 49.

I've got an elevated psa whch I've had for many years without taking any action. The doubling time has been 7 years (from psa of 10 to 20). Why is this not a relevant factor as Dr. Scholz states in this video.

@@MariamAhmed-sb8kz Yes, I did get a biopsy and an MRI as well. It turned out that I had a lesion of 2 cm with a gleason score of 7 (mostly 3+4) so I had a seed implant combined with radiation treatments. My PSA now is very close to 0.....

My Dad after 1st ADT, his psa got to the point of 0.1, then after 2.5months just yesterday its 1.4. His doctor said its still below 4 so still doesn't need another ADT shot. Should I worry about his psa increment or just agree with the doctor ?

@@ThePCRI My Gleason 9 cancer was treated with EBRT, brachytherapy, and 24 months of hormon therapy. My PSA was undetectable afterwards for about 14 months. The last three levels (two months apart) have been .06, .11, and .15. Is this indicative of aggressive cancer doubling? Thank you

Should You be concerned with a doubling time of 10% per year over the last 6 years. That’s 6 no/ml in 2017 to 10 no/ml in 2023 evenly spaced out over 6 years.
