Update on Diabetic Foot Infections -- Richard Oehler, MD

Dr. Oehler reviews infections of the diabetic foot. He begins by introducing the concept of a multidisciplinary team as essential to the management of these patients. He then describes the current epidemiology of diabetes and diabetic foot ulcers. He also discusses Charcot arthropathy and how it occurs . Annual diabetic foot exams are also crucial to these patients, and Dr. Oehler also reviews proper techniques and what to ask/examine. He also covers diabetic peripheral vascular disease, diagnostic imaging modalities, wound management, and the proper empiric antibiotic regimens and durations of therapy. Lastly, he discusses prevention techniques necessary to avoid recurrent ulceration.

Thank you for the video. Some references consider that Wagner's degree 1 is a superficial ulcer without clinical infection, which is called NDFU. And the second degree is a deep ulcer that reaches the joint or the bone, and there may or may not be cellular tissue inflammation, but there is no abscess at all, and no bone and pure lesion, that is, we may face two inflamed cases (dfi and ndfu). As for the third degree, it is an ulcer Deep idfu but with abscess or with bone and myeloma or inflammation or abscess in the joint capsule Fourth degree is gangrene The questions are as follows: 1) If we have a patient of degree 4, is the ulcer considered infected or not? That is, will grade 4 evolve de jure from grade 3, or not necessarily? . 2) In the event that we have a patient with Wagner’s degree 1 and he later has an infection, will he be classified as 2, and if the infection turns into an abscess, then the Wagner’s value will be 3? Great video. Charcot Foot is the worst thing I've had from 30 years of T1D. I have a huge hole on my foot and I just spent 46 days in the hospital, in Portugal. Through Christmas and New Year's and my wife's birthday. Now I have hyperbaric oxygen therapy and the VAC negative pressure therapy. Unfortunately my infection keeps coming back and I can't get the cast put on my foot. I'm so emotionally exhausted over the stress it's causing and I have a horrible feeling that I'll lose my foot. There are no other options for me in Portugal and I can't afford to go back to the States to get proper treatment. So I better enjoy the rest of my life, I'm 44, because I feel my life is in danger. And tbh I will be happy to finally rest without fighting this disease anymore.

Michael I've had Charcot for 15 years now, every year I suffered from wounds on my feet. In 2015 I had a BKA of my left leg. Best of luck to you, take good care of yourself....

Swollen foot cdc said the swelling was not due to infection. Gave him a teds sock he could not walk. No infection and sent him on his way no real answer wow

Many thanks, been searching for "diabetic nerve damage reversible" for a while now, and I think this has helped.
