In his weekly clinical update, Dr. Griffin discusses the fourth US case of H5N1 human infection and reminds the audience of the definition of “pasteurization” before reviewing the recent statistics on SARS-CoV-2 infection, where to find PEMGARDA, the effectiveness of molnupiravir and dexamethasone, convalescent plasma, what to do when healthcare workers succumb to SARS-CoV-2 infection, how an EEG can be used to look for signatures of cognitive decline after SARS-CoV-2 infection and what we observed 2 years following SARS-CoV-2 infection for tissue-resident T cell and viral RNA persistence.

Back in the hand milking day, had an uncle(as a boy) that would point the teat at his face and squirt the milk directly in his mouth while milking the cow. He got very sick one day, had run into a cow with a brucellosis infection. Survived, later(1970s) ran his own high end dairy operation on the same family farm. Milk was sucked out of the cow, and sent via glass plumbing to a chilled stainless steel storage tank. Plumbing was washed and sanitized after every milking(twice a day). Cows udders are washed before attaching the milking machine. All farm kids I knew, myself included grew-up on fresh raw milk and butter as well. Operations were regularly checked by health inspectors, milk is closely monitored for bacterial contamination. Milk went from cow to chilled storage tank in minutes. Need milk(on the farm)? with clean container drain from storage tank, take to kitchen refrigerator. Can be done safely if you are close enough a healthy cow. There is always a low level of bacteria/fungi present, after pasteurization the levels are much lower, which makes it safer and extends the shelf life.

Regarding the raising of awareness on the pasteurization issue: Sounds like that would be a great public education project for the schools of Public Health. Just my two-cents.

Where on the cow are the teats? Where on the cow does she defecate? What is the consistency of the manure? When the manure accelerates and hits the ground behind the cow, where does it splatter? How effective is the preparation of the teats at milking time? Commercially clean or surgically sterile? No thanks. I’ll take my milk pasteurized…
@silentrunner3067
@silentrunner3067 Ай бұрын
Google is your friend. The udder is normally washed with soapy water that may contain a disinfectant before attaching the milking machine. A little bleach goes a long way. And no, nothing is surgically sterile. The equipment is sterilized with disinfectant after use. If you have ever streaked agar plates, you would realize that the dust in the air and even your breath has bacteria, contamination is everywhere. Even surgical instruments become contaminated as soon as removed from an autoclave. It is a question of amount. Cow manure has the consistency of wet cement. And yes it does tend to splatter when it drops from a height of4-5 feet out of the "south end of a northbound" cow. Knee high rubber boots are a wonderful invention.

I haven’t watched in a while. Daniel looks slimmer in the face. Not that he looked particularly rotund in any way….also my sister and I were recently looking up how to pasteurize eggs for making homemade mayonnaise.

We have a farm locally that supplies raw milk, reason enzymes in milk not denatured by heating, lots of testing, chilled storage and delivery, not shipped across country to milk plant and then homogenised, slimmed, etc. but also occasional recall notices, because it is a risk. I usually just put water on my cereal or eat yogurt.

There is more than one kind of pasturized milk- The kind with a long shelf life that doesn't need to be refrigerated before opening (for example, parmelat brand milk) and the kind that must be refrigerated before opening. The hosts go on a rant without addressing the subtleties. There is also canned milk, which is similar to the former.

I had COVID this past week after traveling throughout the UK. I’m hearing reports that there is a spike of cases in Stockholm and a few other areas of the world. My symptoms, fever up to 102°F, extreme fatigue, weakness and dizziness. Since I’m 65 I requested Paxlovid, but it increased my nausea, and after the 3rd dose I started vomiting. I ended going to the ER for fluids. I stopped the Paxlovid and felt so much better. I never had any cough or respiratory congestion this time around. Oh. I had the Novavax vaccine in October.

That more than 6 months which is ample time for your antibodies to lower. The vaccines don’t cause lasting immunity, just creates about 4 months of antibodies and then the antibodies start to go down. Plus with so many doing nothing to stop transmission, everyone is constantly creating new variants. Wear an N95 if you want to avoid another infection and further damage that this virus causes.

Are the bird flu cases in cows happening in both factory farm raised and non-factory farm (grassfed, organic, pasture raised, etc.) cows? Those poor factory farm cows are living in very unnatural / petri dish-like conditions. Does the heavy use of antibiotics in raising cattle play a factor?
@marklemont3735
@marklemont3735 Ай бұрын
The dairy cows get it from the shared milk pumping equipment. I have read also, it may be possible for infected birds to invite themselves to share the water tanks outdoors, for the cows. Same can be said for food troughs.

Birds, such as sparrows, live in the rafters in barns and dairies as well as out of doors. There is no getting away from them. The use of antibiotics has nothing to do with this situation.

The problem with these retrospective antiviral studies is that the criteria determine who gets the antiviral and who doesn't, which is antithetical to finding a 'matched control'. Also a little disappointed that the EPIC paxlovid trial is the only randomized early treatment trial that exists, so everything is based on that one trial. One problem with that trial is, the percentage of people progressing to severe symptoms was about the same (20%), yet hospitalizations were 10x less in the paxlovid arm.

What about UHT milk? Pasturized milk is centuries old tèchnology. It requires refrigeration, thereby ìncreasin̈g its price & decreasing its shelf life. Mention it as all of Europe has abandoned unsàfe pasteurized milk. UHT milk tastes better too!
@jdp9994
@jdp9994 Ай бұрын
When you can't trust the milk source, pasteurization makes sense. But to assume it is the better option is not scientific. Yes, there might be pathogens that might cause a problem (but what are the statistics), but there also are non-pathogens that can be beneficial. Not all bacteria are detrimental. Microbiome etc. It at least deserves a little thought and not mocking dismissal.

Science education is so important, but obviously we have to go back and address what we erroneously assumed most people HAD LEARNED. Confused people need tiny, quick bites of helpful information, e.g. a bright sticker placed across top of milk container defining pasteurization simply in every day terms. “Heat treated for your safety.” With a graphic of an anthropomorphic flame chasing away an anthropomorphic germ. O.K. maybe professionals can come up with something better. 🤷🏻‍♀️
