In the society of japan people would look at Josuke and think he is a kid looking to do bad things to others. All delinquents take similar hairstyles and looks and are usually bullies or in gangs. The fact that a delinquent (someone society japanese society looks down on) had a kind heart and helped Josuke and his mother while a respectable looking business man Kira, is a serial killer is the intention. That guy was never meant to be Josuke, the blood doesn't need explanation if you know enough about delinquents or or watched enough school anime. They get in fights all the time because they rebel against the norms in japan. Simple as that, no scrapped time travel or anything.

That could kinda make sense, but, usually in this type of scenes in anime/manga, were someone is telling something that happened, the flashback always is represented accurately as posible, so the viewer/reader can understand what actually happened And, even if that were the case, why Koichi imagined the person that saved josuke all damaged, with all the blood and wounds over his face? I think that's the biggest problem.

I don't buy " koichi imagines the man to look exactly like josuke does now" In similiar scenes i've seen in that past that wouldn't really make any sense.

Kira, in his final showdown, sent Josuke a decade onto the past, and Josuke would have to fix the time line by saving himself to defeat Kira.

When I was first watching part 4, I thought Josuke would be able to rewind time because his repairing ability is kind of rewinding objects to restore what they were before (either their raw materials or a repaired version).
