Have you tried the Zen Bivy system? I did one backpacking trip with a regular quilt and hated the cold drafts when I flipped over. When I did my second backpacking trip, I had the Zen Bivy and have never looked back. It’s the best of the quilt and sleeping bag worlds because it has the baffles on the sides but gives you plenty of room. Like you, I sleep extremely cold. I’m also a side sleeper and a tosser and turner. I look forward to bed every night on trail because I know that I will be comfy and warm in my ZB. (Unfortunately ZenBivy does not pay me to say this, lol).

I have a 30 inch wide pad. It's heavier but I have arthritis and anything smaller is impossible. I don't know how anyone could sleep on a 20 inch wide pad. 25" is better but barely.

I totally agree. I'm a rotisserie sleeper and was constantly worried about falling off the pad. not a good recipe for sleep!
