Tendons are thick, fibrous cords of connective tissue which connect muscles to bone. They facilitate movement of a bone or structure. In this case, the Achilles tendon is responsible for helping the knee joint to flex and plantarflexion of the ankle joint. Under too much strain and stress, even the strongest tendon can become injured. Overuse can cause the tendon to become inflamed, this is known as Achilles Tendonitis or Tendinitis. As the injury progresses, collagen degeneration and scar formation can occur within the tendon. This deterioration of the Achilles tendon is known as Achilles Tendinopathy. Not only does this reduce the flexibility of the tendon, thickening or lumps can be felt within the region of scar formation. If further overuse injury occurs, the tendon could rupture or tear.
Symptoms of Achilles Tendonitis
1. Ache or sharp pain anywhere along the back of the tendon and posterior heel
2. Limited ankle flexibility
3. Redness or heat over the painful area
4. Severe pain the day after exercising
5. Pain and stiffness along the tendon in the morning
6. Swelling at the injury site
Causes of Achilles Tendinitis
Achilles tendonitis is typically not related to a specific traumatic incident, and results from repetitive stress of the tendon. This can be seen when individuals push their bodies too much and/or too soon, but other factors can also contribute to the injury.
These factors are:
1. Leg length discrepancy
2. Overuse
3. Increasing the intensity of activity too quickly
4.Changing training surfaces too abruptly (soft to hard)
5. Inappropriate footwear
6. Too tight or fatigued muscles
7. Not stretching enough before activity
8. Deviated or unstable rearfoot axis
Other factors such as obesity, age, and diabetes can also predispose an individual to sustain an Achilles tendon injury. From a physiological perspective, the lowered blood circulation around the Achilles tendon predisposes it to delayed healing compared to other tendons in the body. The Achilles tendon can receive a stress load of up to 4 times our body weight while walking… now imagine the stress load when running and jumping! That is why Achilles tendon injuries are most common in high-load, high-impact sports such as Basketball, Running, and Gymnastics. Due to the frequent use of this tendon, Achilles tendon pain should be treated as soon as possible to prevent it from deteriorating.
Achilles Tendonitis Treatment
1. R.I.C.E (Rest, Ice, Compression, Elevation) for a new injury -
Rest the injury by cutting back on daily activities.
Ice the injury using an ice pack for 15 mins several times a day. Ice calms down the damaged tissue and slows down blood flow to an injury, reducing swelling, inflammation and controlling the pain.
Compression can be performed using an elastic wrap or bandage.
Elevate the affected limb to reduce the swelling.
2. Eccentric stretching of the calf muscles daily, after the acute pain and inflammation has abated.
3. Strengthening of the leg muscles and ankle stabilizers.
4. Ultrasound and/or Shockwave Therapy can accelerate the body’s healing process, produce pain relief, promote reduction in muscle tension, and stimulate neovascularization.
5. Customised foot orthotics will improve foot alignment and control joint motions during propulsion/ toe-off in the gait cycle.
6. Immobilisation is needed if the injury is severe.
Prevention of this injury includes wearing correct footwear for specific activities and lifestyle. Individuals participating in sports or exercise should warm up properly before, and stretch and cool down after, as well as alternate training regimes where possible. Stretching is very important to injury prevention by maintaining optimal range of motion and increasing flexibility. Changing exercise routines slightly can make a big difference in avoiding an overuse injury such as Achilles tendonitis.
If your condition is recurrent and deteriorating, then you need to be mindful of preventing an eventual tendon tear or rupture. Podiatrists are lower limb specialists who will assess and diagnose your issue, then devise a treatment plan which will accelerate your recovery, optimise your sports performance, and most importantly, treat the pain in your Achilles tendon.

Im a ballet dancer and i do have achilles tendonitis in my right ankle,kinda affect my dancing because i cant focus on dancing because of pain awareness ksksksks

Hi John, if you have Achilles tendonitis, you should look for a good podiatrist in your area to seek treatment. As a ballet dancer, anything that affects your ability to dance should be addressed immediately.

Hi OTRHQ, you're very welcome. Achilles tendonitis and plantar fasciitis do share some commonalities so it’s not unknown for cases such as yours to happen. For people who suffer from both, there is also a likelihood that they are flat-footed or have high arches, both of which can lead to the development of these conditions. To anyone watching this, GET THE TREATMENT AS SOON AS YOU FEEL IT. I got my injury from soccer/futbol and I decided to take two seasons off to see if that helped. Little did I know that I had an extreme case of Achilles tendinitis and I struggle anymore to run, let alone walk on the bad days. Had I gotten treatment sooner, I probably wouldn’t be in this predicament... I’m slowly trying to include a regular exercise regimen back into my daily life, but it comes with so many limitations that it literally is driving me insane. Don’t do what I did.

Man I can’t stop playing football and I have this pain u explained but voltren cream and tablet is my favorite after every game I play but when I woke up sometimes I wish I never play football but thanks now I learned something usefull

I think you mean plantarflex, not dorsiflex. The Achilles is located posteriorly. And the tendon attaches to the plantarflexors, it doesn’t actually act to flex anything.

Ty! Mine hurts like hell in the morning, going downstairs is hard. I'm a heavyweight boxer and I move furniture. I do not want to quit. Ankle cracks when I roll ankles - discomfort seems to temporarily alleviate pain. Idk if it's stemming from a leg imbalance or wat

Hey MisterBinx, you don't necessarily have to stop working out for months. Consider seeking treatment instead. There may be other causes for your pain and a podiatrist will be able to help you reduce recovery time as well :)

Do you recommend wearing an medical walking boot for Achilles tendinitis? I started off with plantar fasciitis which has turned into Achilles tendinitis.. I wondering how I should treat this injury.

Hey B Barr, a walking boot can help as it should provide increased support for the structures of your lower leg. However, we would recommend that you speak to a medical professional specialising in lower limb conditions, such as a podiatrist, before you purchase anything. Simply buying a walking boot off the rack may not be helpful and even worsen your condition.

I’m very active and play basketball almost every other day. I previously had this condition on my right foot but now have it on my left. After running and playing basketball my Achilles hurts and It’s difficult to even walk afterwards. Any suggestions or thoughts as to how I treat it specifically for my case?

I started feeling this pain a few weeks ago. It goes away when I stretch and massage, but will come back again. When I sit or lay down then get up again, the back of my ankle is so stiff! Especially after I wake up or stand up after driving 😵

I've started feeling a weird sensation on my achilles a week ago after i heard a small crack from it that made me less confident in putting my weight on it, now it cracks atleast twice a day and everytime it does i feel a mild discomfort. Just earlier today there's like a cold sensation in my achilles after it cracked. Update: now my achilles feels stiff after the cold sensation

Hi Baka Mitai, you need to speak to a podiatrist or doctor in your area right. A popping or cracking sensation usually indicates an injury to the area and if you continue walking on that injury it may lead to something a lot more serious.

Hello Dr., my dad is a podiatrist and he got me a pneumatic walking boot for my Achilles tendonitis. Do you agree it could help? I don’t want to get a second opinion at a podiatry office, as I don’t want to doubt my father’s expertise. But I’m just so worried. The tendonitis has been a problem for going on 3 months now. I hope the walking boot fixes it, he said I have to wear it for at least 3 weeks. And he also said that if I continue to walk my dog (even with my boot on) it would impede healing. This sucks ‘cause I love going on long walks. I really hate tendonitis!

I’ve got EDS and the Achilles tendonitis keeps flaring due to over probation. Or really pits pressure on the insertional part at the heel . I’m on my way to lose more weight before looking at surgery. We tried orthotics but I just flatten them lot.

Can you get this while not being a runner? The part of my achilles that attaches to my heel feels bruised and sore. It's been this way for months and I just reaggravated it by jumping down off something yesterday. It hurts on foot forward movements

Hi NooDLES411911, absolutely. While running and other high impact activities can cause Achilles tendonitis, it can also develop if you have a flat foot or tight calf muscles, neither of which are necessarily caused by physical activity.

I wonder if this is what I have sort of, if it is - it wasn't caused by "overuse". Ages ago I used to walk downstairs missing a step at a time, on the last step my foot landed just over half way off the step, and with all my weight coming down it slipped off, and ended up at a angle / like that, with all my weight still going forward. The pain is nothing like I had ever had in my life. Even a broken bone probably wouldn't be as bad. Either way it didn't swell, it just hurt for 5 minutes then it was like it never even happened, so I never got it checked. So fast forward to today, my ankle sometimes gets pain like being stabbed with a needle in a nerve, sometimes to the rear of the ankle (shown in this video), and sometimes it's at the front. It can last for a few minutes, to maybe just being a one time stabbing pain. I have a sneaky feeling my ankle is just permanently damaged though. I have seen a doctor maybe a year ago (probably 4 years after I injured it) out of curiosity. I was just told to rest, despite having ample time to have done so. Useless GP I suppose.

my heel has been hurting me on and off over the past couple of years. I play soccer basketball and track, and it only seems to start hurting after running full speed. I try stretching it and everything but nothing seems to work. I’m still in high school and refuses to drop any of the sports i play.

Hey Evan, based on your description, there are a few possible options for your pain aside from Achilles Tendonitis. You have quite an active lifestyle and that is likely to be partly responsible for your pain. We would recommend you see someone for it to diagnose your issue and see what suitable solutions there are without compromising your lifestyle.

You're going to have to stop for a while and heal up. If you keep going every time you feel a little better it's going to get worse. Take time off now and strengthen up your heels and calves

Hi Jharna, the plantar fascia and Achilles tendon are closely associated. Strengthening the plantar fascia could help with Achilles tendonitis as a strong plantar fascia will help maintain gait and posture and reduce stress on other parts of the body, including the Achilles tendon.

I'm suffering right know but using stretching, massaging, and icing in just 2 days. Heck yeah, I can walk now with slightly pain, also with the help of pain killers so I can't go insane while stretching. Hohoho. Hope I can recover soon. I want to workout, and walk normally again!!💜

Hey Ximenaa, beware of painkillers! It is very easy to worsen your injury while on painkillers as they mask the true extent of the pain, causing you to underestimate the severity of their condition. You may end up worsening your condition with inappropriate stretching as well. Please see a podiatrist immediately!

Hey Carol, falling down is one way to injure your Achilles tendon and the location of the injury suggests this is the case. You should look for a good podiatrist in your area for a proper diagnosis, especially if the pain persists. Take care!

We'd highly recommend speaking to your doctor about alternatives for when you aren't allowed to use the boot as it seems that your job requires you to be frequently on your feet. You might want to consider looking for a good podiatrist in your area for a consultation if that doesn't help. Take care!

Hey tom_824, rehabilitation is typically recommended for recovery from severe injuries. In less serious cases, rest is often sufficient to allow the body to recover.

Achilles Tendinitis has cost me a few good paying jobs. Whenever I have a flare up I take a 800mg Ibuprofen which gets rid of the inflammation every time

Hey Leony, a physical therapist is the same as a physiotherapist, and both terms are used interchangeably. A podiatrist, on the other hand, is different as they also perform diagnosis and treatment of the problem, rather than a physiotherapist who focuses on rehabilitation of a problem. By seeing a doctor or podiatrist first, you will be seeing someone who refers you to the treatment most suitable for you, which could be seeing a physiotherapist.

I have no arch and I'm completely flat-footed. I'm having that pain a lot on the back of my left ankle. I'm trying to ice it whenever I can and keep it propped up

Hi New Jack Swing, while ice and rest can help alleviate the pain, it does very little to address the underlying issues causing the discomfort. We would strongly advise you to consult your local podiatrist to have your flat foot condition properly addressed.

thank you for sharing this video. My mum have been suffered from Achilles for three weeks she do RICE method but the pain flares up once walking. She can’t go for physical examination at least for a week so if you could give me any advice will really be helpful.

Is there anything I can do at home? I’ve been resting my foot for months now (from March, which is when is happened, to September). I want to get back to working out but it starts to get inflamed whenever I work out or am on my feet for too long.

Hey luzyinthesky, for home based treatments, you can consider getting yourself a heel lift or calf stretching board to help you stretch your Achilles tendon and other associated muscles.

i went to see a foot doctor and left with a boot (plastic cast really) for three months!! but idc, tbh. at this point I only care about returning back to working out and being pain free.

@@luzycuty Hey luzyinthesky, based on your description, it sounds like you've developed a chronic case of the condition. For our patients, we use a combination of shockwave and other advanced modalities but I'm afraid it's not always available at other practices.

What happens if I was riding my bike ,and out of no where my Achilles tendon and tendons in front of my foot start hurting as well as the arch part of my foot? I’ve done some research and I think it’s tendonitis but my ankle got swollen and I was fine yesterday plz help! 🥺🙏🏽

Hey Ibi, have you recently changed the way you ride or the position of the seat? That can the areas mentioned to feel tight or overstretched. If you are a frequent cyclist, you may also have overexerted the tendon, causing that pain. However, swelling of the ankle is an acute issue that you should see a medical professional for just to ensure that there was no fracture or hidden injury.

I have Achilles bursitis and tendon tears for several years. I was told to take 2400mg motrin a day. Ha. I got custom orthotics, can't wear them because of the bursa swelling. I tried night splints for a month. Not much help. The bursa is like a marble on my heel and I can't wear shoes only hiking boots. I also have super high arches. I've seen four podiatrists and we don't have a plan to improve my situation. Any ideas? I don't have flat feet nor plantar fasciitis which seems to be the only diagnosis they give here.

@@mohergenrader2113 Hey Mo, 2400mg of Motrin is about the maximum one would take under usual conditions so you must be experiencing a fair bit of pain. While we are unable to give you more advice without physically examining you, your conditions sound like it has persisted for a long time and you could speak to one of your healthcare providers about the option of surgery.

@@mohergenrader2113 Hey Mo, yes we agree. As podiatrists surgery is definitely not a procedure we will recommend for most situations. The good news is the modern podiatrist will have tools and modalities to treat a variety of foot conditions non-surgically. If you are not keen to proceed with surgery, it may be best for you to seek out a good podiatrist in your area for consultation :)

Thank you for this video. I have been dragging my Achille's tendonitis for over 2 years now. I play basketball and used to play a lot when it wasn't this much painful. I've had 2 soles made by my local podiatrist, that I do not use religiously anymore because after a couple of months I there was difference, it felt like the pain had worsened. The season started and from the first running session, I already couldn't push. exercises given by my GP also was causing more pain. As of today, after 10 minutes in a standstill position (work, when driving, seating on the couch, every morning), I have to move slowly and limping until my right achille is warm enough to allow me to move properly. When I play I can barely jump. I can't play tennis anymore. I am interested in ESWT and will ask if we have it locally in the south of France. At this point I'm hoping there is a surgery that can help? Any advice is greatly appreciated.

You have to free up space from above and below meaning the foot and calf to release some pull on the achillies

5 months that I can’t run after tearing my Achilles insertion I still can’t run without pain 😭

What if my pain is at the sides of the heel but still hurts when I move my foot up and down? I also do javelin and trained daily.

Hey Jessie, if you are experiencing pain at the side of your foot, it could be peroneal tendonitis or tarsal tunnel syndrome, both of which would hurt when you move your foot in a vertical plane. Please see a podiatrist as soon as you can!

I had surgery on both feet 👣 but different years. I had to get my Achilles reattached on both 👣 My right foot surgery was a success back in 2009 I wore a cast and had crutches and no Physical Therapy I was back at work 4 months later. I was alright with having my left foot surgery because the same doctor was going to do the surgery. Omg!!This was a totally different experience I had to get a boot and a knee scooter and later on crutches I had surgery in September of 2020 I was out of work for 6 months I was told after surgery I developed tendinitis on my left foot. I was released back to work February of 2021 the worst mistake ever🤦🏾‍♀️ I had to be released from work after 5 months of returning because I was in more pain before I left to have surgery in September. Coming back to work doing the same work took a troll on me. My job requires standing, stretching, bending and a lot of walking constantly up and down stairs plus, I worked 4/10 on concrete floors and lots of overtime daily. I decided to moved to another department which did no justice because I still had to be on my feet continually on concrete floors & lots of bending as well🤦🏾‍♀️ the only thing was I didn’t have to do any stair climbing. This department closed 2nd shift. I went back to doing the job I was trying to avoid the stairs and a whole lot walking and overtime 🤦🏾‍♀️Friday July 16th of 2021 I worked overtime. I worked 11 hours that day when I got home I felt a bump on the back of my heel I must’ve overworked myself. I called my podiatrist on that following Monday morning she couldn’t see me until two weeks later. On that doctor’s visit she started me back going to PT in August of 2021 my job gave me the time off because I couldn’t work in pain plus,I had to attend Physical Therapy 2 days out of a week. At PT I was evaluated by a therapist my ankle was swollen plus, I was told I had bone fragments flowing around. I attended Physical Therapy faithfully thinking it was going to help me and release this pain. After, a month going to PT when it was time to visit my podiatrist she didn’t like the way my foot was healing. I was getting frustrated!! So, I went to get a second opinion this doctor did an X-ray and couldn’t see anything because my foot was inflamed he said he only saw bone fragments flowing around and he recommended me to have a MRI done. This was the best decision I made.The MRI was done a week later after my insurance approved it. I was completely shocked what the MRI showed I had a sprained ankle I was told know telling for how long 🤦🏾‍♀️ I got a mild tear on my left foot also,Plantar Fasciitis on both feet lastly, it showed I am flatfoot and a lot more issues. After, these results, I was put in a cast also crutches and back on the knee scooter again for 8 weeks of November of 2021. December of 2021 the cast was removed and I continued physical therapy until my next doctor’s visit which was on January 31st of 2022. After, she examined my foot my podiatrist recommended me to let go of my job because my foot wasn’t improving and there wasn’t anything else she could do. After 20 years working there l couldn’t maintain the job requirements due to Chronic Tendinitis. Because of the use of the knee scooter I have issues with my knees. It’s been 2 years I still have stiffness in the mornings up on rising the bottom of my feet hurts daily even tho I wear heel lifts in both shoes my feet still gives me issues when I sit too long or when I’m too active it’s irritates my feet 🤦🏾‍♀️all I can say is being in pain every day isn’t a good feeling. I still do my stretches and very light walks to keep my health up. My podiatrist limited the amount of walking and standing I can do. My lifestyle has changed 💯to all going through this don’t give up.

Hi Nahum, 2 years is a very long time to be suffering from this condition! Have you sought treatment? Typically the longer a patient goes without treatment, the higher the risk for severe injury. Please see a podiatrist immediately!

Whenever I stretch my Achilles’ tendon, it hurts really bad same thing with walking, I can’t even walk

Hi Ashayan2015, if you are experiencing that much discomfort in your daily routine, you should definitely see a podiatrist to see if there are any underlying causes for your pain.

Achilles tendon injuries, curse of dancers. I have injuries to both left and right.

The function is to dorsiflexe your foot? Maybe tell them it's the excentic function that controles the dorsiflexion while walking. Otherwhise it sounds really weird.

Hi, I am a track and field athlete and hit my upper achilles while riding a bicycle downhill, my inner calf happened to get swole and I can barely move my feet

Hey Pablo, if it's been several days without a reduction in swelling or pain, we highly recommend that you speak to a podiatrist or doctor in your area. A warm and/or cold compress could be useful in alleviating your symptoms temporarily but you still need to have the injury looked at by a medical professional as soon as possible.
