Endometriosis

I’ve been diagnosed with endo and some people will say “just get pregnant it’ll make it better” is that false? Also feels insensitive for folks to make comments like that when a lot of patients with endo have fertility trouble. Thanks for the great information!

Thank you for this. I told my gynecologist about how painful my periods are and she just said “some people have pain”. She made me feel like I’m crazy. So thank you for making me feel heard. I really appreciate it.

Thank you for this great info!! I was diagnosed with endo at age 27 after over 2 years of infertility. It is incredibly frustrating that being diagnosed earlier (and going on meds to treat it) might have prevented what I'm suffering through now. I wish everyone knew the basics about endometriosis so it could be identified more easily!

Hi Dr. Natalie, I've been struggling deciding if I should go on hormonal birth control since I had surgery to remove an endometrioma. This video and another one gave me reassurance that it's the best thing to do for me and my fiancé's family planning future. Thank you for doing these videos! You helped explain endo to me better than any doctor I've ever seen in person.

Wow. Wish I would have seen this years ago. My husband and I have been TTC for 5 years! Painful periods and all the symptoms you described that doctors ALL said would go away once we got pregnant, but still haven’t been able to. Doctors wouldn’t even refer us to get the endo surgery done… I have a schedule appointment with another doc next month 🤞🏼 This is the one that’ll help us! I’ll be going in asking for a laparoscopy.

Hi Natalie, love your videos and I’m thankful for them. You mentioned stage 1&2 is a time when it’s good to make some lifestyle changes. Can you please do a video on this? So many conflicting things online, some saying limit red meat others saying that’s an old study and the latest says it doesn’t make a difference. Some saying limit dairy and others saying there isn’t strong evidence supporting this. So difficult to know what is the most important few to limit seen as many lists are very long on things to limit and it’s hard to know which are the most important. Many thanks!!

I went to the dr and told her i am having terrible pain payed all the exams necesary and she send its probly endo and gave me a pill told me to leave, i needed a confimacion not a probly, so now im walking around scared it could be and thinking ill never have kids because she didnt explain properly, so thanks for the info you are a great dr.

I got diagnosed with endometriosis at 16, and I'm now 28. It's been back and forth as far as pain, some months/days are worse than others. I hope one day I can get it a bit more managed than I am right now.

Thank you so much for sharing your content. Can you pleased a video where you talk about risks and benefits of hsg and laparoscopy procedures for endo? Specifically, will lap surgeries have side effects, leave scarring, prevent or help fertility, etc? What is the difference between regular surgery and robotic surgery to remove endo?

I was diagnosed with Endo in June of this year, but have suffered from painful period since 8th grade (I’m 21 now). I’ve been on birth control since 8th grade and when we noticed it not working as well (cysts began to grow and I had breakthrough bleeding), along with extremely painful symptoms I would be in pain daily because of my Endo whether I was menstruating or not, my doctor finally decided it was time for surgery. So they found Endo, and did ablation to remove it. He said I was at a stage 1 but that doesn’t mean my pain was any less. So since my surgery in June, I have tried different birth control pill, a birth control patch, the depo shot, and none of them worked. I just got done bleeding for 43 days straight and I was prescribed medication to stop the bleeding and I have just started a new birth control pill in hopes it will work. It is a 3 month long pack so I shouldn’t get my period until the end of every 3 months (my doctor is trying to get my period down to 4 times a year to keep my Endo from growing sooner) and is a progesterone and estrogen mix pill. I have only been on it for a few days so I’m not sure if it’s working yet, but if it doesn’t work my only option left is an IUD and I really don’t want to get one. I’m just looking for some overall advice about my treatment so far and if you think I should get fertility testing to see where I’m at. My mom had this. She was very ill for years. When I was one year old, she had a complete ovariohysterectomy. She was 29.

Only thing I would add to this video would be urinary symptoms. That is a huge part of my progression of disease. At first I thought I was just becoming incontinent at the age of 21 or having a constant UTI. I always had painful periods but thought that was just normal. I was always told it was. I went to an obgyn and explained my symptoms and how I suspected I may have endo because my aunt did and had similar symptoms. Unfortunately was made to feel crazy. It was years of progression and worsening symptoms before I got the courage to see a different obgyn. He said he would look and found the reason for my incontinence. Through the laparoscopy I finally had treatment and answers. Unfortunately my body does not tolerate hormonal birth control and I have had to resort to Lupron for treatment. But after 7 years with my husband ( 3 years after I finished Lupron the first time) we finally were able to conceive. I don't believe without diagnosis and treatment that we would have ever been able to. Also my quality of life was 💯 improved after treatment. Unfortunately when my son turned two and quit breastfeeding it seemed my hormones went crazy and started having all the symptoms again. So now I'm a month into my second bout of lupron. The side effects are annoying but nothing compared to my endo symptoms. Hope that I have lasting relief again. 🤞

While my great grandmother and grandmother were able to pop out babies like nothing, my mother had issues. She was diagnosed with endo, but was able to have my brother and I (after suffering from a miscarriage). Now I'm married and 30 years old, my husband an I have been trying for a couple of years. We were successful last year, but I had a miscarriage. I'm now doing everything , from tracking to my cycles, to ovulations tests, and taking lots of vitamins for both my husband and I. However, we've had no success. I had blood work done at my OBGYN and the only abnormal thing that came up was that I might have a lower ovarian reserve, but they said I would need to have it confirmed by a fertility doctor, but told me to keep trying naturally until the end of this year before calling. I informed them my mother had endo and they stated that could be a possibility for me but didn't seem to concerned about it. While my cycles are fairly regular, I do have the painful periods with constipation during my cycle. I also suffer from migraines and muscle pain, and when I was younger I broke out into hives regularly. I almost always have the worst migraine right before my period. Because of these symptoms I had a lot of blood work done during my life to try to determine where my pain and hives were coming from. My ANA level came up high, they told me I most likely have an autoimmune disease, but could not determine which one (went to multiple doctors 😔). But with the high ANA levels, the possible diminished ovarian reserve, the miscarriage, my family history, painful periods, constipation during, it all makes since now! I never connected it all until I watched your video! Not trying to sound like a hypochondriac, just looking at all possibilities while TTC with my husband. I live in San Antonio, thinking about calling your office in Austin to schedule an appointment. You're probably super booked, but I'm willing to wait if you can help me have our miracle baby for sure! ❤️

Dear Dr. Natalie, thank you for doing this! I am following from Italy and your videos are so informative. Better than most of doctors I met so far...it took me 10 years to get diagnosed with my severe endometriosis, I had already 2 laparoscopy (without success) and I am 32yo. I have just seen also your IVF video and wondering how the two interact... how worse can get the disease if anyone goes for a stimulation (or even more than one)? Thanks a lot.

I'm pretty sure my sister has endo. She is 24 now, but ever since she got her period at age 10 it has been DEBILITAING for her. She would always be doubled over in pain, cant get out of bed, always crying, and just miserable. She would have to call out of school, and later work when she got older. I went on birth control and learned that sometimes it can help with period cramps and reduce the cycle. So I told my sister about it, she has been on Birth control for 3 years now and she is soooo much happier. She can actually DO things now on her periods compared to before. Every doctor she went to told her "oh thats normal, if your period" but we both know it isnt. I was just diagnosed with endo, but mines is really mild, and Ive been on birth control for around 7 years now. I told my sister this might be something she has as well. She told me that she is not thinking about having any kids any time soon, so she will stay on birth control for as long as possible. She literally said, "IF i get off birth control I will literally die. I dont want to go thru that pain anymore. I'm doing so much better now with the pills, no pain, no bloat, im not crying and feeling miserable" She will be seeing the obgyn that I see now, but its crazy how different stages of endo can affect you. I believe she has it pretty badly. Especially just remembering how bad it was for her.

So appreciate your videos! I was diagnosed with endometriosis stage 4 last Fall. I had little to no symptoms but found out that I had endometriomas when I was in sonography school. My surgeon wanted to try to save both ovaries but ended up having to remove the right one 😞 I just turned 30 and still have cycles every month. Can you explain how losing an ovary impacts fertility? My surgeon said losing my ovary in itself shouldn’t impact my fertility but having endo does

Thanks for the video! I heard about you from a MDJ Q&A video; this is my first time watching you and I have just subscribed to your channel. I found your information informative and helpful! I have done my fair share of research / information gathering and was happy to learn something new. I do have one question though... Do you have a way to help me find or refer a good specialist in my area or tips to best pick one for myself for treating endo? I have never been diagnosed surgically, but at the same time... My mom, her older sister, and my maternal grandmother all had endo and had to have hysterectomies between the ages of 29-35. My older sister had extensive surgery at 29 with 75% of her ovaries removed; the surgeon did save her uterus and she was able to have two sons via a donor and IVF! 😍. Anything you can tell me would be helpful! I really appreciate you taking the time and effort to give this information out freely and making videos!; please keep it up!!!

i would look at surgeons trained in minimally invasive gynecology (MIGS) they are your experts, many Reproductive Endocrinologists (like me) do surgery too - or at least know who the best endo experts are in our area. Hugs!

I have had endometriosis probably since I was 14/15 but wasn’t diagnosed until 23. I have had 6 laparoscopic procedures. My last one was in august, ob/gyn clean up my uterus and surrounding organs and flush my tubes. Now waiting to get pregnant naturally?? I had no idea that endo could affect my ovarian reserve. Because the infertility doc said that”you have low ovarian reserve you can’t get pregnant with embryo adoption, Ivf won’t help you, etc...” I have had gallbladder bad appendix removal because of the endo as well.

Hi doctor! Thanks for all the information. It has been two years since my diagnosis of a left ovarian endometrioma that was the size of 5 cm. But then it shrunk on its own to 3 cm and has not changed its size for 2 years. Recently, USG report is telling it might be hemorrhagic/endometriotic cyst. I am confused as to why it is now being called hemorrhagic.

Can you please explain the difference between endometriosis and adenomyosis and how they affect your fertility differently? Can you have both? I've been diagnosed with adeno and my IUD isn't holding back symptoms anymore and I was told my best chance at getting pregnant would be to jump right into fertility treatments right after IUD removal as to not let symptoms worsen as you mentioned in your video. Thanks for explaining that! (assuming adeno is similar) Love your channel!!!

Not Endo related but maybe . . . I would like a video on a tilted uterus/cervix that is off to the left. I was told this is what I have but my doctor never fully explained what this means to me. I'm told that's what the pain could be during intercourse. Periods are painful but not to the point I can't push through it. It would be great to hear your thoughts on this.

I haven't been diagnosed cause I always thought endometriosis was rare. But I've always been like 70% sure I have it. My mother and sister both have hypothyroidism and if there's a link between thyroid issues and endo and a hereditary component to endo and it's 1 out of 10 women I'm up to like 90% sure now. Is there a non-surgical test for endometriosis? I try to limit surgeries I have. Even if it's not a definitive diagnosis is there a test? Maybe you could explain the diagnosis surgery more?.

I was just diagnosed with stage 4 endo due to my ovarian ectopic being found on an endometrioma. If my HSG came back at open/slightly restricted, semen analysis good, and my AMH is 3.5, would you advise me to pursue IVF sooner than later?

Thank you so much for this video. I've never had surgical confirmation but my family has a strong history of reproductive issues and endometriosis and the syptoms have always fitted for me. The fact about teenagers and painful periods blew me away. I went to see my normal doctor and they just gave me stronger pain killers saying there was nothing wrong.. I wasn't diagnosed until I was 21 and a doctor took me seriously and referred me to gyneacologists. My question is: how difficult is it to get pregnant if you have endo? I see a lot of conflicting evidence so not sure!

Since it’s a surgical diagnosis- hard to know for sure (some women May get pregnant without issue and some with disease have major issues ) but about 30 % if patients with unexplained infertility have endo

I really wish I found this channel a long time ago because three years ago whenever I was 12 years old I was diagnosed with multiple ovarian cyst and a cyst on my Fallopian tube that was the size of a tennis ball so now I also have damaged Fallopian tube and lots of lesions so I don’t know what to do except take birth control and in iud Oh my gosh I also have celiac disease are you serious this is also because of my endo?

If someone has endo, but is ready to have a baby, and therefore needs to go off the birth control pills, what are some effective & safe pain management options when those really painful periods come back?

This is absolutely wild and I feel like I have found a few missing puzzle pieces. I appreciate all this info and never considered endo getting worse during each cycle (without birth control). If there has been an early miscarriage, would this potentially cause a larger impact of "stress/growth" as the body was "more prepared" for a pregnancy than a "typical" cycle/hormones? Very curious if it is "similar" to pyometra that female cats/dogs get. In the sense that the reproductive organs may "fight" against itself if there has not yet been a pregnancy. *Using terms loosely as I know each woman is unique!

Wondering if you would recommend egg freezing for confirmed endometriosis (after surgical removal of an endometrioma) and low AMH, especially if you’re growing so many follicles at once would you predispose them to getting more endometriomas in the future? What about egg quality and number if at least 1/2 of normal ovarian tissue is removed from previous surgeries?

YES YES YES to egg freezing. Since we are aspirating the follicles, the procedure does not usually predispose to more endometriomas - but you are at risk in the future and women with h/o endometriomas often run out of eggs early. Preserve options if you can with egg freezing.

I have been trying to conceive for 1 year and I went to a fertility doctor and the doctor believes I have endometriosis.... Never would I have thought that because my periods are painful but not that painful 😣

I don't have painful periods and they r regular but my mother had endometriosis and I'm wondering if I might have it been trying for a baby for a yr and had one miscarriage and not been trying for 7 months!! I've taken a few ovulation tests and have gotten positive so it seems I'm ovulating just not sure what my next step should be!

Do you recommend the use of lupron for potential endo patients to reduce the inflammation right before FET? Is it necessary to do laparoscopy or can’t you just go straight to endo-specific protocols in order to get pregnant?

Does Endo have any connection to decidual cast? I was diagnosed with Endo 10 years ago. I've had 2 surgical operations since, due for the 3rd coz the cyst is 12cm in diameter. I used to have decidual cast once in a while before but for 3 years now it's every month.

I've been ttc for 4 years, I'm 27. My Dr had me do letrozole and gluten free after running all those tests you are talking about, for 4 cycles. We just had our first IUI. I had asked her if maybe doing surgery before the iui would be better and she said she didn't think so. However after another consultation she told me that after 3 IUI cycles, then we could do a laparoscopy before IVF. Is that normal? Wouldn't it make sense to do a laparoscopy before when I had originally asked?

I have been on birth control, to stop having a period entirely, pretty much since I started getting a period. I recently went to the ER because I had an endometrioma (that I didn't know about) rupture. What sort of preventative care can I even consider at this point? (edit to add that I started BC to stop periods around 20 and am now 36, No plans to have children but do not want to remove reproductive organs and go on HRT unneccessarily)

If any thought maybe would want kids - freeze your eggs, if not then consider extensive surgery with a MIGS/endo specialist and suppression with birth control or other options (sometimes lupron and progestin, there are other options for pain too like orlissa). In the end, see a specialist in endo to truly discuss options - it is such a hard disease, sending love.

I had a softball sized cyst entangled in my ovary which led to my left ovary being removed - as well as them diagnosing me with severe endometriosis. I’m now 26 and ready to have kids but I feel my doctor doesn’t take me seriously about going straight to treatments to try and get pregnant - she’d rather me be off the pill for 6 months and then try the treatments. Especially with being prone to cysts that some fertility medications can cause more cysts. But she’s the “best” in my area. Are we allowed to travel out of state for infertility treatments? Are we allowed to request our medical notes from previous surgeries? (I feel like no one believes me and I need proof! 😭) I feel like I have no idea where to go from here!

Dr. Crawford says this is autoimmune, however, there’s much debate wether Endometriosis is autoimmune.

I have been sort of diagnosed with endo- I haven’t had the surgery but my doctor (actually Mama Dr Jones) told me she’s about 99% sure I have endometriosis. However no one has ever told me about all of this! I’ve been lucky to have been on the pill from very early (since I was 15) due to all the pain and ovarian cysts I was getting. Birth control has helped a lot but I still have a lot of pain and occasionally cysts. Now I am almost 24 and married and will be thinking about kids in the future, but I’m afraid to stop the pill now for fear of all the symptoms getting worse 😩 do you recommend I get the surgery for a final diagnosis as I’m getting closer to wanting kids? I also have a LOT of digestive issues which make since now 🙃 ahh so many questions!!

I recommend seeing a fertility specialist before you do anything. Time off the pill allows endo to get worse - so let's check tube, AMH, sperm and then decide on surgery vs no.

Hi! Doctor! I had a miscarriage 3 years ago, I had no endometriosis back then but when we want to try again, our fertility doctor found out I had unilateral cyst on both ovaries. I am afraid it may affect our chance of conceiving a child. What can we do? 🥺

Cut down on all processed foods, especially GMOs and junk foods. Check into an Endometriosis Diet. Take extra care and eat super clean, drink plenty of water. Exercise and get good quality sleep. Maca root is good for fertility. Best wishes!

I'm on a waiting list to see an OBGYN due to extremely painful periods and general pelvic/uterine pain throughout the month. However, my GP doesn't think endo is likely as my periods are extremely regular and very textbook (28 days, 5 days of bleeding). Is it possible to still have regular periods and endo?

YES!!! Endo does not change the frequency or length of your periods - just associated with pain. Hoping an obgyn can help you. HUGS

Yes! I had my periods like clock work. I was in so much pain I’d be throwing up. Did the laparoscopic surgery at 21 and they found a treasure trove of endometriosis. 9 years later I’m in remission

Birth control does not stop endometriosis disease from progressing. It may help to improve symptoms in some people, but that’s all. Please look up the latest research on that.

Actually combined ocps can prevent disease progression where as progestin only pills/treatment does not, per the latest research 👋🏻

Another question sorry- if I was tested for celiacs and showed no antibodies, does that mean I don’t have endometriosis? Or would they be different antibodies for that?

I'm not a doctor (biology PhD student) - antibodies are specific to a certain "target", in case of celiac this target is gluten. For Endo, the target would be the endometrial cells (different target, meaning different antibodies). Anyway, inflamation can occur even with no involvement of specific antibodies.

There are no antibodies for endo. Many are gluten sensitive but not truly intolerant. Some endo patients notice that by taking gluten out of their diet they feel better.

in other videos she described it as a level of pain that makes you miss daily activities (like if you routinely miss work/can't leave the house during your period).

On the whole, an abnormal level of pain means you miss school, work, or you can't engage in other activities you normally can. I missed SO much school as a teenager beause of my periods.
