What is narcolepsy? It’s a neurological disorder that results when your brain is unable to regulate your sleep wake cycle. In this video I will discuss how to recognize narcolepsy and how it is treated.
Two of the classic symptoms are excessive daytime sleepiness and cataplexy.
Cataplexy is the loss of muscle tone that is triggered by strong emotions such as laughing or anger. The muscle tone loss can be very dramatic such as collapsing to the floor. Or it can be very subtle such as a slight facial droop.
With narcolepsy, REM (rapid eye movement) sleep intrudes into your wakefulness. So while you are awake and talking, you suddenly become overwhelmingly sleepy. When you fall asleep, you immediately start dreaming.
People with narcolepsy are not necessarily sleep deprived. They usually get an adequate amount of sleep, but they can’t control the timing of their sleep.
Signs of narcolepsy
Hypnogogic hallucinations - This is a weird experience that happens at sleep onset. You can see bizarre and vivid images as you are transitioning from being awake to being asleep.
Sleep paralysis usually occurs with the hallucinations. Because your body is normally paralyzed when you dream, and this is called REM atonia, people with narcolepsy can get sleep paralysis at odd times of the day when they fall asleep. The paralysis usually lasts a few seconds to a few minutes. Because the paralysis and hallucinations happen at the same time, it makes the whole experience of falling asleep very creepy.
People without narcolepsy can experience hypnogogic or hypnopompic hallucinations as well. Hypnopompic hallucinations occur during the transition from sleep to waking up. It can be accompanied by significant sleep inertia and even confusion. You may have seen someone close to you wake up confused and talking without making sense for a few minutes until they fully wake up. If you ask them about the nonsensical stuff they were talking about, they probably won’t remember what they said.
Other symptoms of narcolepsy include choppy sleep, nightmares and leg jerking.
Diagnosing narcolepsy
Your doctor may suspect narcolepsy from your symptoms, but the diagnosis requires a sleep study. The sleep study would include a polysomnogram and multiple sleep latency test (MSLT). The polysomnogram measures your muscle activity, breathing and brain activity. The Sleep latency test measures your time to fall asleep. A normal sleep latency is 15-20 minutes. Narcolepsy is diagnosed with a sleep latency of less than 8 minutes and the presence of 2 sleep onset REM periods. With narcolepsy you must show on testing two instances when you go straight from awake to REM.
Narcolepsy treatment
Medication
The gold standard treatment is a medication called Xyrem or sodium oxybate. This medication treats the daytime sleepiness and the cataplexy.
Sodium oxybate is related to the drug GHB - gamma hydroxyl butyrate also known on the street as liquid ecstasy. GHB is illegal, sodium oxybate is not.
If you continue to have daytime sleepiness, your doctor may add a stimulant such as Provigil or Nuvigil to your Xyrem. Sometimes antidepressants are used to treat the cataplexy, hypnagogic hallucinations and sleep paralysis.
Behavioral treatment
We usually recommend people take at least three scheduled 10-15minute naps during the day to help with the sleepiness. It may help prevent the sleepy episode from occurring by napping BEFORE you get sleepy. With this approach, you are sleeping on your terms and not having it creep up on you while you are at a traffic light.
Narcolepsy usually develops in late adolescence or early adulthood. It can go undetected in many people for years, but if you are age 50, and only recently having trouble staying awake during meetings, it’s probably not narcolepsy.

My Narcoleptic development was TERRIFYING. Around the age of 18 I was just a bit more tired, I could still function. As the years went on it got progressively MUCH worse. I wrecked my car twice in 3 days, I had to quit my job,. Due to the job loss I in turn lost my home and car. I finally found a Dr who was willing to test me, there was no question with the MSLT what was going on. She put me on Adderall, OH MY GOODNESS! It was as though I got a second lease on life (at 23 years old) I went back to work, I could drive, go out to eat. Be some what normal again. I miss those days, I got pregnant last year and she took me off my medication. Had the baby, got back on it...My little girl died at 29 days old. I was back on my medication, just trying to learn how to get from one moment to the next. She passed in June 2018. I was taking my Rx from June till October 31st 2018. That was the day I found out that I am yet AGAIN pregnant. We conceived back in August, so I was only not pregnant for a whole 3 months. Off my Rx till the new baby gets here 5/11/2019.

Omg! I am SO VERY SORRY about the loss of your baby! My daughter list twins from twin to twin syndrome right on her daughter’s 3rd birthday, but hasn’t been able to conceive or carry another baby since. 🙏

My story is long tbm..initiality in adolescence ... diagnosis at 13 years old ... two children, both lived the ups and downs of the disease ... but thank God we are here living one day at a time .. . today I am 41 years old(narcolepsy with cataplexy)

Thank you so much. I'm a narcoleptic and it's been traumatic to deal with; it's changed my life and disabled me, making me rely on others much more than I wanted at my age. It means a lot to have it discussed in a scientific manner without being laughed at.

@@samanthadelahunt3698 some do, but you never feel “rested” and then you can’t sleep well because of the lingering stimulant effects. So I feel exhausted all the time, but maybe 40% less on meds.

I myself suffer from narcolepsy with cataplexy since 2015 and only a week ago got my Nuvigil amd its not perfect, but at least i'm actually able to feel that i have more power and much less anexity - wich all derived from me being tierd all the time. My heart goes to you. I'm myself going this journy feeling i'm all by myself, because people dont really understand the effects of it on every aspect of the life and you feel like you should be able to function like everyone else but its allot of trial and error, changing eating habits, medications, and staying with a positive attitude twords life. Good luck on everyones journy. Dont give up on your dreams - everything will come at the right time for you. ♡♡♡

Me in college: Sleeping for 12-20 hours straight, falling asleep in lectures after 5 cups of coffee, losing my work-study job because I couldn’t wake up at 7 am, no matter how many alarms I set, and handing in countless assignments late. Oh, and also, having the incredible talent to sleep for 9 hours straight after eating 25 mg of Adderall. Doctors: Oh, you must just be depressed. Here, eat some Prozac. How about some sleeping pills to help “regulate” your sleep! Of course, expressing any frustration because you have weird symptoms that compromise your performance and ruin your life = DEPRESSION. We don’t really have any answers for you. We guess that it just sucks to be you! We completed our medical school and residency, and have a job with a sweet paycheck, so we give zero fucks about you - our lives are set, we’re not the suckers napping like a baby sloth, loser! Oh, and by the way, if you start asking what is the damn point of living when all you do is sleep, fail, and get screamed at - well, careful there, if you say the wrong words, we can send you for a psych evaluation and even get you involuntarily committed faster than you can blink. Remember, it’s your life that sucks - not ours!

Thank you for this. My daughter is N. She would swear that she is without cataplexy. Although, I've seen her go down when emotionally overwhelmed. This presentation is a generalization of N. There are common symptoms, but not all people present exactly the same. Some are higher functioning than others. My daughter is a power house of energy, until she's not. Then she's down for months. It hurts to see her that way. As her step-dad says, if it weren't for narcolepsy, she'd be boss lady of any organization she chose.

I also have a lot of energy, until i get a sleep attack. Most people get very surprised when I tell them I have N. But what has helped me a lot is changing my diet. Low carb has given me parts of my life back. Over 10 years, my symptoms have improved mainly thanks to the diet. Everyone with narcolepsy should at least get this information before giving up hope of having any control of their quality of life! 🙏

Brilliant video. Accurate too. I was diagnosed over twenty years ago and I can personally attest that all of these symptoms are spot on. I am a perfectly functional person though, in spite of this. Lifestyle changes are the key. Never lose hope people.

I’ve took Provigil, Nuvigil & Ritalin at various dosages & none of it worked. As these medications wore off my body found & forced itself to stay awake as an instinctual reaction to all those medications I mentioned that were wearing off.

Since i was little i could be doing anything at anytime of day and just fall into a sleep paralysis episode multiple times a day it still happens to this day but on a lighter scale

My late uncle had narcolepsy. I remember he would start reminescing deeply about the past, and his facial muscles would droop, then it was nap time for him. The second last time I saw him was at my brother's for my birthday. We'd all been laughing and joking quite a bit and we noticed it was triggering a lot more narcoleptic episodes. It was peculiar to witness as his facial muscles would sag, and his arms and hands but five minutes later he was back to normal - like an on-off switch being flipped.

I always dream, and dream immediately after falling asleep for 10 or 20 minutes. I have hallucinations, and hear people call my name, sometimes hearing loud explosions. I'm always tired during the day. I will huge urges to fall asleep. I'll wake up from a dream, bite my tongue, and fall right back a sleep. I noticed driving today that I was in another world like I was asleep, but awake. I also sleep walk. I have had a sleep study done, but that only determined I fall asleep quick, and only for short periods of time. 😩😩😩 Sometimes I wake up sitting on the toilet or eating food. It's kinda scary. It's like I'm a zombie or something.

This is the best description of my symptoms of narcolepsy that I’ve heard so far. I’m doing some research because I think my narcolepsy is coming back. I have a strange story about mine…and I wish I could be part of a research study because throughout my 66 years, I’ve had everything, including things that were for kids., I was 61 when I was diagnosed, and very few people believed me, and thought I was just tired, but it’s way more than than that! I did go directly into REM dream cycles out of nowhere, and it came on while I was driving at first, so, I made my son finish driving, I didn’t have any of the scary dreams. When I had a sleep study done, I knew I had sleep apnea and figured I had narcolepsy, but they woke me up about 4 hours after I went to sleep and said I could go home. It turned out to be a severe case and they almost took my license away, but I promised to take the highest dose of Nuvigil every day. Here’s the catch: I had saline breast implants and had been extremely ill for years, and then moved and got mold toxicity, but I didn’t know about any of those things and why I had such a severe plethora of autoimmune diseases and infections! I also have had restless leg syndrome since I was young, found out by word of mouth (an Angel in my mind!) and had 3 operations within 5 months and waited another 5 before I was well enough to have my implants removed. All my symptoms Of Sjogren’s, fibromyalgia, RA, Raynaud’s, and other issues like thrush, infections, are almost gone now, and disappeared right after the implants were removed. The capsules looked like huge slabs of bacon and salami, but every doctor either didn’t know about the dangers or lied to my face, saying they were “perfectly safe” and there was a big “to do” about them causing illnesses. One surgeon was the head of plastic surgery at U of Michigan just two months before I had them removed! I should have sent him a picture! Thank you for putting this post up and being truthful! 🙏💕💫

I was diagnosed 30 years ago and have never heard of Xyrem!! I currently take Ritalin and provigil and still have trouble staying awake. Will definitely be asking my doctor about xyrem.

I've been looking for more information on the sudden onset of REM sleep, which has recently become a bit scary (particularly while driving and being given no warning); nothing like drowsy driving, where you DO know you're getting tired or fatigued... and found this video. Since I was originally self-diagnosed through my own research and finally convinced my doctors to send me for the MSLT (to obtain a formal diagnosis), for the most part, I've only seen information about narcolepsy in written form.... the doctors never used the terms we all know that are related to narcolepsy, like hypnogogic hallucinations, which was the key to my nailing down what was wrong with me. Consequently, I've been mispronouncing "hypnogogic" for at least 10 years, saying it with the second "g" soft, like the first one. Thanks for that! If a person HAS a disorder, the least they can do is know the terminology AND correct pronunciation of related symptoms, huh? LOL Thanks again!

Have a sleep study done. I had the same thing happen when I had breast implants and found out I had such SEVERE narcolepsy that they almost took away my license. I was put on Nuvigil, thank God, and I’m ok. After the implants were removed, I got WAY better with not only this issue, but sleep apnea, many autoimmune issues, infections, etc. 🙏

Thank you! I developed severe narcolepsy as a part of the many autoimmune issues that breast implants caused in my body for 12+ years. It subsided when I had them removed, but I get bouts of it still. Has anyone ever expressed this to you? My breast implant naturopath had other patients with the same issues. I think maybe this should be brought to everyone’s attention. Thanks for letting me share! 💫💕

Same thing here! BII and tons of autoimmune including NT2. It has robbed me of my life, even after removal of implants. I have seen other friends with implants suffer from very similar symptoms but unfortunately are too vain to have them removed. Hopefully my comment and yours help others. I know we aren’t the only ones.

I searched for this to find out what was wrong with the man i shared a room 10 years ago... The man would be fully awake and talking and suddenly falls asleep... I was shocked at how suddenly his sleep pattern was.

I'm really not happy that this is showing the classic known narcolepsy. I didn't hear any mention of type 1 or type 2 .... as usual. And this is why so many people are not diagnosed

So if none of those medications work, what is the point of being put on prescription medication if none of that shit works? What I’ve found works perfectly well for me is setting an alarm to go to bed at a decent time between 9pm-11pm & waking up well rested at 7am-8am. After waking up, I’ll have either a small breakfast, with either coffee, energy drink or a fruit smoothie with energy in it. That’s usually more than enough to keep me awake throughout the day…..

I did not know what naçrolepsy really was. I been having these symptoms for years since I was a teenager... Im waiting on the results of my sleep study test....

Dr I have always had most of these symptoms to some degree but was wondering if someone who drinks heavily as I once did prior to being medicated for adhd, could somehow regulate their sleep cycle, perhaps unknowingly by heavy alcohol consumption? When I was a heavy drinker I would remember that it served my schedule and sleep cycle to the extent that this was a major cause of disruption that I was concerned about overcoming upon no longer drinking habitually. The times I would get uncontrollable bouts of sleep, straight to rem, would usually be while sober and coffee seemed to worsen it, however I admit that warm humid temperature would always be a fairly consistent trigger as well. Recently the bouts were begging again, but it is difficult for me to distinguish wether they were simply because of my adhd medication being absent in my body for a week. Indeed there was warm humidity, however I underline that in the same breath as how strong the sleep with rem was. I was at a Whole Foods, then it’s been happening at the university as well.

Meh, it's marginally effective -- at least for me. Adderall and Dexedrine are better.

I don't know if I have narcolepsy, because my symptoms are not obvious. I often get scared when I think about going to bed but when I do, I usually sleep very deeply, usually 6-7 hours if no one wakes me. And it only took me 6-7 minutes to fall asleep, but when I woke up I was always tired, and my body ached, had frequent shortness of breath and impaired memory all day. Sudden sleepiness is less common if I'm not doing anything but just lying in bed, but when I'm working or active, I often have short bursts of sleep from 9am to 10am and 3 to 5pm.

i do have a sleep paralysis, it was scary at first but nowadays its a common thing for me and i normally get back to sleep. idk if my situation is a narcolepsy but i even can see things when im half asleep (just waking up). i see views, animals or people in colours while closing my eyes but i know im not sleeping, cuz i can hear people around me. sometimes its tiring since im always curious what those images were. it last for few minutes before it end. i have anxiety, so i always think that i might have some mental problem or something haha. so, is that narcolepsy¿?

Diet has helped me tremendously! Been eating a low carb diet for 11 years and my symptoms have improved a lot over time. I found it more helpful than modafinil so i stopped taking it after a year on the diet. There is hope to improve with lifestyle tools! 🙏

A very young locum for my GP decided that I had narcolepsy and suspended my driver's licence. It took a sleep study and a report from a psychiatrist to convince the Ministry to give me my licence back. Some new doctors are over zealous and don't ask nearly enough questions. I was sleep deprived and I have none of the symptoms you discuss. :(

I don't know if I have narcolepsy actually I have hypnogogic hallucinations and REM also in sleep paralysis it happens sometimes when I wake up and while I am sleeping. Sometimes I missed a lot of my school work it's because of I sleep quickly

Please can someone help me out. I have many symptoms of narcolepsy. I know hallucinations are one of them. so during the day or during the night whenever I feel an overwhelming tiredness right before I fall asleep it feel like something shakes my body awake. Right before I fall asleep. It happens all the time. Sometimes I am so tired that I just ignore it and basically pass out from being so tired. But most of the time it scares me so bad I fight everyday not to fall asleep. Please anyone give me an honest opinion. Is this a symptom of narcolepsy or do I have a poltergeist?

Wow I am so sorry. I am about to have a strong talk with a doctor today who left me hanging with not meds. I have to take off work. Personally I am trying herbal remedies also. This is ridiculous.

@@Lex4233 I would say please see a homeopathic doctor or even an herbalist. I have been told to take in tea form burdock root, I was taking sarsaparilla, ginseng and I just started trace minerals. My doctor told me to get my adrenal gland checked also. It is a lot of options out there and I don't know what your specific diagnosis is. On occasion I take Ethan's clean energy shot tea. I was told mushrooms are good for energy and that is a main ingredient in the tea. Again consult an herbalist These are just some things I have tried through the years. Horny goat weed was recommended but did not work great for me.

The cure that I found . The cure . Mediate and confess on this words " I am healed in the name of Jesus ,I am healed of every sickness and deseases say it has many times you can Say " I am sleeping well at night and I and i am functioning well during the day" nothing is wrong with me" Say it and act like it . Read this bible verse and pray with key area that concerns in Jesus name : psalm 6 - 1to 10. Confess this also as many times Mathew 8: 17 before bed. Go to be bed early as 9:30. When every wake during the confess and affirm again as many times with stong faith do this continuesly and you will be completely healed. Also sleep with your lights on if you are having ilusinations it will disappear. That was how I got cured. When your hear voices rebuke it in the name of Jesus

If you eat a big meal right before a work meeting and find that you have fallen asleep during the conference, you may be suffering from a serious case of Food-Comma. Recommendation: eat less.

If you do have narcolepsy disgnosed already, carbs make symptoms worse. Its not juat food coma. But yeah if you dont have it diagnosed and your only falling asleep after food, it could be as simple as food coma 😅
