When you hold doac do you still put them on heparin for high chads vasc or recent stroke patients?

Usually we don’t bridge DOACs due to rapid onset and short duration of interruption (unlike warfarin which has to be held longer and takes longer to restart), but if they are very high risk of VTE and will be off the DOAC for a long period then it could be considered!

Last year i had MVR cabg and i am on warf. But from the last 3 months i have inguinal hernia and surgeon advice me to stop warf 5 days before the surgery date. He doesnt told me about BRIDGING THING? Next month is my surgery and my age is 67 yr old.

I would definitely send him a message and ask if you should bridge with heparin during the 5 days you aren't on warfarin, due to your history of mitral valve replacement! Mechanical valves especially
