I have been on APAP for years. I switched to one steady pressure last night. I slept really good. I don’t think I moved all night. (My pillows around me were all still there. Which rarely happens.) Thank you! I have to trial pressures but on the right track. 👍🏻

Just got my Resmed 11 to replace my old 10 yesterday. Been walking the 10 down from 14 for about a month now. My alleged doctor only said that I had extreme apnea without any details whatsoever. When I went back for a followup, there was a student there with him. I had to walk him through the exam in order to tell the student what he was doing. Never went back. I would wake up in the middle of the night with an extremely dry mouth. Then I researched it on the net and started lowering the pressure. Got down to 9.6 till I got my new machine. Told them to set it on 9 since it didn't do decimals. Works great. I woke up this morning having gotten a great night' sleep. It is a journey.

Dang! I’m going to do it again. This is so spot on I almost can’t believe what I’m watching. I have performed multiple titration studies in the years since COVID-the years when APAP became 80% of the home CPAP set-ups-who report that their pressures were raised maximally, their problems continued, and the in-lab titration found they needed, say, a setting of 10. I know what was happening. I have become jaded-jaded-jaded by the sleep industry, especially on the one-size-fits-all healthcare side of it.

I started off with Auto for about 6 months or so then switched to fixed. Even thou i had the same good results with both auto and fixed i feel like i'm getting a much deeper restful sleep. The changing pressure during the night on auto seems to knock you about in your sleep plus auto reactive where as fixed prevents events even beginning when on the right pressure.

Ok, I guess this is a love comment too lol. I have been a Respiratory Therapist for 27 years and a CPAP user myself for 4years. I would recommend a titration study because a good sleep tech like Jason makes a big difference. I wish more sleep labs were able to do this type of interpretation more often for pts. . Since I do homecare I have to field a lot of troubleshooting calls and a lot of people get ordered a machine with a wide-open range of pressures with no titration study. This can cause a lot of issues for some people. I was fortunate to be able to self titrate my machine and find my best range of settings, but not everyone has that option.

My doctor has never looked at the detailed info from my machine. In fact he does not even have the software to do so. I have a Phillips Dreamstation and the only machine that they can look at the detailed info with is Resmed. I actually have to print off the reports from OSCAR so that he has any idea what my numbers look like. Unfortunately, he is my only option in my rural area. That said, my AHI is consistently below 2 and my therapy is working. He does listen to my input, but I get the feeling that he is not happy that I am better at diagnosing my issues than he is.

@@prayingforhealing4039 Thank you, but no need. I have become well educated in the use of CPAP and can read the reports and adjust as needed. With my current adjustments my AHI is below 1 consistently. 😀

@@markbans I watched a crap ton of videos from LankyLefty and Uncle NIcko over on the CPAP reviews channel. It won't replace the need for a sleep study to determine the cause of your Apnea, but it will help you to be more informed when talking to a doctor and/or sleep tech. You have to be your own advocate.

I was lucky enough to have the person that did my sleep study make sure to explain my situation and show me the data from my whole night before I left because I had 136 apneas in an hour. I've since figured out my own mask situation and how to get my backup travel cpap off of 4 to 20 and to where I'm prescribed and have done super well... as I wait for my full time cpap to get fixed or replaced. If the info is available I will learn so thanks for the information.

Wow! A throw-back to the good old days. My first lab was set up in a physician practice and as part of the lung health clinic I was running. The physician oversaw the clinical education, and he was often present during our post-test education sessions. Pretty cool. Sigh. The good old days.

Just dropped my pressure, went back to APAP 9-14, and selected A-Flex 3. No more leaks and actually woke up refreshed for the first time in a while. 90% was about 12. AHI 2. 9 hours solid sleep! I’ve had to manage my own due to cost and availability. It is also a bit of a racket here in Australia: the initial sleep study was at home with tired and incredibly uncomfortable equipment which practically guaranteed a high score for the total 4 hours sleep I achieved. I don’t have much faith in the people selling this equipment here.

As a brand new member of the sleep apnea club, AND seeing that my prescription is for an APAP machine, I can't wait to deal with this BS.. I'm supposed to get mine at the beginning of January. It almost seems like I'll be sleeping even worse than I already do.

APAP machines can be set to fixed pressure. You can change the settings on the machine yourself if you want. Lanky has some good videos on here about oscar and such plus you could buy his service to get you on the right pressure.

@@elian222 Yeah i recommend people start with auto to get use to cpap over time. i narrowed down the low and high pressure with a range of 7-12. took about 6 months to get really use to cpap till eventually changinto fixed at 11. some people are fine on auto and some are sensitive to the small pressure changes whilst asleep. In my experince i rarely seen the machine ramp up to max as demonstrated. Maybe they have been improved. It's taken me nearly a year to get into the swing of it and now find cpap easy.

Yeah I use an APAP started out at 4-16 but after gaining a better understanding I'm now 10-10.5. Think I should just go 10 lol. Sleeping like a baby with really low events, I solved the minor mouth leaks with the ole boil and bite mouth guard and some tape and if it wasn't for you I never would have sorted it out, Thanks Jason.

Jason, Thank you for your informative videos. Here is a tip on a video if you’d like to run with it. REM related OSA, REM dominant OSA, or another name, REM specific OSA. This subset of people seem to fall between the cracks. If they have an overall decent AHI (let’s say 0.5-3 AHI)in all stages of sleep other then REM but have OSA only in REM sleep (let’s say 6-15 AHI), what should these people do? Especially if they need “higher” pressures in REM sleep (7-12) but that pressure disrupts their sleep. Some research articles say it should not be treated and some articles say it should be treated. What’s one to do?

To say the doctor "doesn't have time" to look at the data is a cop out. Thats their job considering they are a specialist and being paid by patients to help them with what is a serious medical condition. What makes it worse is they should know cpap machines are not always interpreting the data correctly as you kindly point out. If they dont have the time, maybe they should be hiring or outsourcing to people like yourself? The problem with today's medical system is its all about herding people in and out of the office to maximizing billing instead of emphasizing quality care.

I have used a cpap since 2018, my first at home study i had 97 disruptions/hr and my o2 got down to 67%. needless to say I have used cpap ever since. My AHI is 0.7 but i wanted to do another sleep study just to be sure i had the pressure right, I almost feel like i could increase my pressure up above 14, good to know I can probably do that manually without any harm.

I recently started using cpap. I am finding that nasal masks contact my nose in such a way that they squeeze and block the nostrils. The machine doesn't overcome this squeezing. I have tried all the available sizes. I have found that the nasal pillow masks work best but do have their own set of issues. I'd like to know your thoughts

My ahi is below 1. Been on my cpap for one month. My pressure is 4 to 10. But i can honestly say i don't feel much of a difference. What gives. Should I raise my pressure. I was once on 4 to 20. Pressure was way to high. Could barely breathe. I do better breathing on a low setting. I just dont feel amazing. Like most people describe.

I recently started therapy and I believe they have my mask pressure too high. Hi have high mask leaks with a resmed f30i and they recommended switching to a nasal mask and just letting my mouth vent. It seems like the mask is “cranked on” to keep the mask from leaking. I watched a few of your videos and plan to use the OSCAR software to identify why I am getting mask leaks. Thanks for the vids.

Ps using RESMED 10 APAP and they have the max pressure at 20, I have noticed that over 17 seems to wake me up. I may try adjusting to ~16 max and see if that fixes leaks and pressure waking me up.

Hi Jason I’m new to the whole cpap journey - My sleep study revealed an AHI of 24 though I have severe rem obstructed apnea where my oxygen levels dropped to 60% for 55 minutes on two occasions - I have a rental Resmed 10 not sure which machine to purchase I’m thinking the mini. I’m struggling to keep my mask on for any more than 5-6 hours last night was 3 hours - my sleep lab have been useless in supporting me and I’m seeing my sleep doctor next week. I paid to see a respiratory sleep Physio a couple of weeks ago and she was very helpful in changing my pressure settings from 4-6 to 5-8 I seem to be sitting at 6.8 for the night - I don’t feel I’m well rested, how do I measure the quality of my sleep and specifically my oxygen levels throughout the night? I tried close to 8 different masks and have landed on the P10 nasal pillow, switching the ramp off and having a warm air tube has also been helpful. Will I ever get a full 7-8 hours uninterrupted sleep again?

i just bought the dreamstation go ..on night 2 ..i have had sllep apnea for a while but the machine i had cpap machine the pressure was too high and i kept riooing it off now after years of using alternate treatment to keep my air way open..im back to giving it a shot with the drreamstation go..i would really like a consultation if i can get one..thanks...im having issues with the whole process and i need some help.. thanks..

I have a Dreamstation and after a few weeks I can't see how long my nightly sleep time as it appears on the screen. I do a reset and I'm good for another week or so and it starts again. I can't get a readout on my computer from the SD card, can't use Oscar.

I have a Phillips Dreamstation I recently turned up the pressure because I couldn’t get restful sleep, . It has helped a lot and I’m sleeping better, although I get dry mouth even with the humidifier all the way up on the unit, but I’m wondering if I you have any suggestions?

Perhaps, like me, your jaw drops open during sleep. Try a chin strap, turn tube temp control to 2, moist to 3.

I tape my kisser with 2 inch Micropore tape. Two 4 inche long pieces overlapped from below my nose to my chin plus one 2x2 inch piece sideways on each corner of my mouth. Wash your skin before going to bed and keep it shaved once every two days at least. This is the only way I was able to keep my mouth from drying out like a mummy.

So my issue is an I’m on a bipap machine where the doctor prescribes the pressure and the supply company sets it, I can’t change anything. I’m currently on 25 over 20 per the crappy sleep test I had. I think I’m on the wrong pressure because It’s so hard to breath out. I maximize the ramp up to 40 minutes and as soon as I hit that max 25 over 20, I wake up immediately because I can’t breath out. Dry mouth and swallowing air. Does anyone have any suggestions? I’m close to calling and trying to get my pressure adjusted lower cuz I just can’t get any sleep. Thanks for any help you can provide.

Thanks for this. Your videos are very helpful. I need to contact you for a consultation. I'm thinking I may not need a machine as my AHI has been 4 at the highest and 1.3 at lowest. Going to try a nite at lowest pressure (4) for high and low and see what my result is.

So, are you saying get a Titration study as opposed to using APAP? My wife's pulmonologist recommended she have a Titration study, however, she did not want to.

I just got the reamed 11 machine yesterday. My DME was able to get one prior to aug release date. You know if Oscar will work yet on this machine yet. Can I just insert my blank SD card into the machine to start recording data?

Was on variable pressure of 5.0-15.0 and recently was changed to static pressure of 11.0. Sleep is worse, so maybe it's too high?

My doctor did the same thing, i was 5-15 but than my pressure was reduced to 5-10 and i feel worse.

whats the difference between a Apap and a ASV (which i what I'm on)? most nights I do fine. been on it since last year.

APAP treats obstructive sleep apnea, while ASV treats central sleep apnea, or a mix of both. That said, ASV has the option of using a similar “auto-titrating” algorithm as APAP, called AutoASV, where if it thinks you had an obstructive event, it will automatically increase your EPAP as described in this video. I would stick with the APAP for several reasons.....none of which I will share here because it sounds like excellent content for another video!

Not a bad choice you can set your APAP as a CPAP. Also, most APAPs will provide much better diagnostic data on sleep reports compared to lower-tier machines.

If your machine has an SD card all the data is on the SD card, you just need the free software OSCAR to read the data, which will cover every breath you take with the machine on for 18 months (I think), which is more data than you will know what to do with, so hire Jason to interpret the data for you.
