In this video I discuss about choosing an inhaler which is adapted to each patient. There are many inhalers for asthma and COPD available, however some people find some devices hard to use and therefore have poor inhaler technique. This results in suboptimal benefits despite using the newest inhalers available.
Having the right inhaler for each patient is more important than having the best inhaler.

I've finally gotten an inhaler as an adult after I stopped using it as a kid and it really does help when I'm having a flair up. I used to suffer for the evening or the entire day with severe tightness but now it can be not so bad

I have been prescribed multiple inhalers both rescue and maintenance. To start with the abuterol inhalers don't work for me at all. Neither did the normal rescue inhalers. Eventually I tried the trelegy inhalers and they worked but not the way you're supposed to use them. If I used them daily they really didn't help much but if I used them as a rescue inhaler they worked pretty well. My doctor decided a nebulizer would be more appropriate. Still doesn't work with albuterol but the trelegy ampules in the nebulizer will stop an asthma attack in minutes.

Hello Doctor and thank-you 🙌 for the informative content! What are differentiating efficacy outcome factors when beclomethasone dipropionate is administered via nasal vs oral inhalation? Is it advantageous to shake the medicine containing canister and wipe orifice nozzle before each spray is administered?

Thank you Doctor. It really boils down to what the insurance company will cover. Mine no longer covers Advair but covers Dulera which is basicaly the same two drugs. I do much better with hfa inhalers rather than dry powder types. The one that works best as a bronchodilator for me is Combivent.

My insurance just stopped covering advair and I switched to symbacort and I've been noticing my asthma symptoms again so I'm wondering if I need to try another one or wait longer to see if it will work better with more time

@bonitagirlyy Advair hfa worked. Symbicort didn't work for me. Duelera seems to have the same, if not better effect as Advair .

Thanks for your video, I have in last few months changed to fostair been good for asthma, can’t find anywhere for sure but can these have side effect of nose bleeds at all, maybe unlikely but timing does seem about the same time as I changed to that one.

Been using Ventolin brand blue inhaler for 43 years. Have tried other suggested brands and types over the decades but find sll others tend exacerbate my difficulties. I find Ventolin works right for me. Accept no substitute.

Ventolin is a rescue inhaler. I use Trelegy every day to try to keep from having asthma attacks, and Ventolin for breakthrough attacks.

Hello Dr. Stanel, Thank you so much for these videos. It is clear how much you care for people. My question is somewhat simple I hope… I am currently on Breo, but I find that when I take it, I feel a little anxious and my heart speeds up for almost the whole day, and even part of the day after. I tried it again yesterday and didn’t particularly enjoy it, so I decided not to take it today. However, I felt pretty crappy today. Is there another medication out there without the jittery and anxious side effects? I take it for allergenic asthma. Thank you again. 🙏

I am using Bufomix easyhaler. I give it a shake. Do one click. Inhale and exhale my breath. Insert mouthpiece and inhale deeply and hold my breath for 10 seconds and exhale slowly but once I am done using I don't find any taste in my mouth or even feel I breath in any powder. Is it normal for Bufomix to not feel anything after using it?

Hi doc. Just want to ask if seretide is okay? My brother told me to stop salbutamol for a moment because sometimes my salbutamol ihaler won't work thinking I might be immune to it and it will not work for me anymore. Is it a good decision to use seretide (evohaler) salmeterol (as xinafoate) 25/125? My asthma is not that serious if it is summer but when nov to feb it is sometimes bad. Thank you so much

I saw somewhere they've created, or are at least working on a vaccine! So a permanent solution to it! Could be years away before it gets popular though. Might not fully do it either.

I've always used the Ventolin blue inhaler (via a spacer with a mask) as a reliver & a Fluitiform inhaler as a preventer. I was recently switched to the fostair inhaler, initially to replace the fluitiform & then to use as both a reliver & preventer (instead of the blue one). I have been struggling with it a lot. I'm meant to take 1 puff in the Am & PM (instead of x2) & use it inbetween as needed. I've now been switched to the fostair Nextinhaler (which can't be used with a spacer). I'm struggling even more and it doesn't seem as good as the ventolin when using as a reliver when im wheezy. I'm struggling a lot to get used to having to put my lips around it and with how hard a breath you have to take to get it in. I feel like im ending up with powder in my mouth. Im bed bound & and my head cant be raised which complicates things as well I think, and i can't have my mouth swished round with water afterwards (due to the choking risk due to my dispashia). Advice would be greatly appreciated. Any tips on using the Foastair nextair? I feel like i cant do a deep / quick enough breath in with it especially after its clicked. I struggle to hold it in afterwards too.

Do you mean you have the Pink Fostair 100/6 micrograms? I use this one with a spacer. AirChamber. I use two puffs which is much easier. One puff using the chamber then you inhale slowly 3 times. Then you press again and inhale slowly 3 times. When you breath out you do that through your nose. I hope you can understand me.

I was originally on the aerosol version of it too with my areochamber spacer, same as you, but they switched me to the powder version of it called the Fostair NEXThaler, which is the one I'm struggling greatly with. They said it's 'better for the environment' than the aerosol version. You have to breath it in very quick / deeply, instead of like with a areochamber where you can take slower breaths in as it can't be used with a spacer. I end up with powder in my mouth and many more side effects than I had with the other version of it. My doctor has finally agreed after 2 months of struggling to switch me back to the aerosol one though, thank god. This is the one: www.asthma.org.uk/advice/inhaler-videos/nexthaler/

I'm using Trelegy Ellipta. Ihe powder you feel in your mouth, I get that too. My Dr. told me to make sure I rinse inside my mouth and my lips after I use it. Since you can't do that I would talk to the Dr about it making sure you're alright not rinsing. He didn't tell me why you rinse. Just said it was important that I do.

@@thatgirlthatsme It's because corticosteroids, including ones that come in powder or aerosol form i.e. for asthma, if you don't rinse and end up with powder or the aerosol in your mouth or the back of your throat it can cause a hoarse voice (+ if that's happening it means the correct dose is not getting all in your lungs) and mainly as it is a steroid it "reduces" or better turns down the response of a part of your immune system response, locally, promoting relatively unharmful microorganisms from thriving and since the immune system can't keep a control of it due to the nature of the steroid oral thrush commonly caused by Candida albicans, can occur. Reference: Biomedical Science graduate & fellow asthmatic. Hope this clarifies. :) I'm on Flutiform 125/5 and Ventolin although haven't been called for a asthma review in quite some time. Have to see them about that actually. Love, Mila.

My grandma is suffering from COPD-emphysema but she also has dimentia and she doesn't know how to use these inhalers. I keep on teaching her how but it doesn't work. 🥺 I hope there are some more alternatives. 🥺

Hi Dr, I suffer a lot of phlegm, it gets on my chest and as I run 6 times a week and race 5K every week, my doc gave me Anoro Ellipta to take before my run, I run for two miles before the race to clear my chest and then race, Is it the C.O.P.D that produces so much phlegm ? My brother suffers the same phlegm problem, he is a non runner, so is it possible a virus could have started it for both of us ? We both got the problem at the same time, Thanks, great video ! 👍

Did you ever catch covid? It very well could just be exposure to contaminants in your environment. High quality air filters & maybe supplemention of NAC would help. I had the same problem. Those helped alot, over time.

Somewhat not true....I had a bad reaction on a generic Albuterol Sulfate (generic??) than I did with ProAir HFA After five days I could hardly move, and I felt like I was breathing in/out through a folded towel. It took my insurance co about 3 weeks to approve the one O had which was perfect....It was like not having breathing problems at all.

Taro ent is really bad, I have never had reactions to puffers but atrovent has affected my eyesight, has given me muscle tension in my neck and shoulders, I’m really really tight since taking it….i have been feeling foggy and confused….and generally feeling very bad all round. I started feeling a little better and I noticed my chest was a bit tight so I took the puffer and within minutes ALL OF THESE SYMPTOMS BLEW UP, so I will not take it again…it also affects my heart. I take one puff a day sometimes twice.

It depends. I use both. You can't carry nublizers on the go so always need to have your portable inhaler handy with you. Also if your doctor allows try a simple steam inhlaer when you feel congested. It also helps besides nublizers. Wish you a speedy recovery.

Yes, true, but controller and reliever inhalers are different types. When it comes to comparing controller inhalers between themselves, the differences are not that dramatic

This is really simple and calming, but you can’t forget there are 4 main classes of drugs for COPD and asthma. Everyone with Asthma should have a rescue inhaler which is a bronco dialator or beta antoginst like albuterol as the key drug. But for each of these drug families (saba, Laba, lama and inhaled steroids) there are lots of different drugs all with side effects. You will need to work with your doctor to pick the best for you

Nice comment, i always try to convey the same message that although there are many options picking a treatment requires an honest and open conversation with your doctor to make sure you are getting the treatment that is best suited for your case (this may not always be the latest inhaler)

I Don't have Healthcare insurance in the USA i have asthma and high blood pressure. I usually use Primatene Mist. How dangerous is this. I have to use it 3 to 4 times a Day

My asthma got worse on that, please tell Dtr that you don’t have insurance. They can set you up with either samples, use generics, or use a Canadian pharmacy, Symbicort in the US ~$400 each, from Canada its $78 each. Have your Dtr write you a paper Rx.

Can someone maybe explain to me how this little inhaler of atrovent hfc costs $570.00?? Does it contain some kind of rare element? Cuz that price is asinine.... seriously? I think the pharms really take advantage of medicare and medicaid. Do they really think a person can just fork over that much money for a tiny little canister of this crap? Listen brother I've suffered from asthma my whole life. Have weeks of wheezing. Smoke ganja daily. No inhaler works. I bought a primatene Inhaler at walgreens. Changed my life. Some states it's banned. Look it up. Primatene spray

​@@TheUnabomberFN old primetime mist CFC worked best the new primatene mist uses a chemical to dispell the epinephrine,. It helps some but not like the old one I still feel super tired never like the old primatene. I switched to asthmanephrine but sadly there has been a shortage / backorder for months now and I am finally running out, I used to smoke ganja everyday too but since I get really sick and ran out of asthmanefrin I stopped.

I would first recommend discussing the matter with your doctor for advice which applies to you. But would think about 1) is the inhaler appropriate for your condition (i.e. Do you understand your diagnosis and why the inhaler was given in the first place?) 2) are you using it correctly? 3) would you need a second inhaler? - for asthma for example there is often need for a controller inhaler which is taken regularly (but not the blue one/ventolin).

See your doctor asap for reevaluation. Your condition may have worsened. Or perhaps you're not using it correct therefore not getting the most benefit from the drug in the inhaler.

I have listened to three videos now. My first guess is that your mother tongue is Romanian. You sound like a pastor from there who leads a food distribution program for the poor in Canada. You do not speak a Slavic language but you are from Eastern or S.E Europe. So, that leaves only one language. It is not so much your slight accent as your manner of speaking.
