Thank you for this informative video. On a personal note, I had a chin implant about 20 years ago to replace nose fragments that continuously were infected. I immediately felt the implant was too large and had it removed. Under the anesthesia I checked on the smaller implant and decided to keep the smaller rather than remove it completely. I asked about shaving the implant down at that time and was told it wasn’t done. Probably the bigger implant fit across my chin better as I can feel the ridges of the smaller one. Esthetically I am happy. I have used filler to even out the sides of the implant as I age. Thank you again for raising this issue.

Hello, Dr. Prasad. Thank you for the interesting video. After learning about filler migration and the "puffy" look effect, I was persuaded to avoid filler. However, implants are a much bigger commitment, and I was worried about the chance of not receiving the results I wanted on the first try. Your video has helped me reconsider fillers. However, one remaining concern I have is with hyaluronidase. Is it true that hyaluronidase also threatens the area's natural (original) hyaluronic acid? After using hyaluronidase multiple times, are patients left with less natural volume than they started with? p.s. On a side note, I am currently exploring your website, and I deeply appreciate surgeons like you who respect the intelligence of their patients by putting forth more educational material than hype marketing. I have also never come across a surgeon who transparently discloses their legal history on their website rather than leaving it up to the patient's digging. Choosing a cosmetic procedure and provider is a scary undertaking, but doctors with integrity make it less so. Thank you.

Another great video doctor. I am grateful that i found your video and thankful that you choose to share your unbiased information with us! Many doctors don't care about their patients and will give the patients unsuitable treatment as long as it makes profit, it makes me lose my faith in them but you regained my faith. Thank you again!

Hi@@samaha-9624, No, bone mass is still lost due to aging even with implants. As facial volume loss affects the whole face, so facial dimensions change and become smaller, eventually the implants don't fit the face. I hope this clarifies things for you.

So you talked about how your bone changes as you age but not the implant, so it wont look right with aging. Understandable, but cant you add filler around the areas where your bone may have resorbed due to aging. Also, most bone resorption within the face is at the maxilla and mandible. Because as you age and lose teeth, you lose alveolar bone. However losing bone within your cheek or chin in general is very mild if noticeable at all. Therefore implants are a much preferred option still especially for chin implants, as they would be alot less noticeable when your skin starts to thin and lose its elasticity. Also, what negative effects are there to injecting thick volume fillers once or twice a year for a long term? Doesn’t the filler migrate and most of it doesn’t actually dissolve?

Hi Michael, Yes, filler can be placed at the bone level to make up for volumetric bone loss, which uses a specialized technique to place that deep in the face. Not only will chin and cheek implants eventually not fit due to volume lost around them, but many people find chin implants uncomfortable. When I describe fillers, I'm talking about hyaluronic acid fillers. Deeper placement between the bone and muscle prevents migration, and also adds stable definition to the face. Hyaluronic acid naturally exists in the body, so the body naturally produces hyaluronidase for normal turnover, so these fillers are easily metabolized with time and do dissolve. The instances of hyaluronic acid fillers not dissolving are due to encapsulation, and injectable hyaluronidase can dissolve it. I hope this clarifies things for you.

Dr....will darkness of skin due to sun tan goes away completely if we stop getting exposed to sun.........or any treatments required to remove completely !!

Hi MG M, Your tan should fade with time if you minimize sun exposure, and you don't have lasting sun damage to your skin like solar lentigos, but these are spots that can be treated. I hope this clarifies things for you.

I don't understand why doctors will not discuss about the very very very likely scenario of bone resorption at site of facial implants. The bones will cave in underneath implants, and you will be left with boney ridges surrounding the implant if you decided to get it taken out.

Hi T-mac0192, Puffiness in the cheeks is due to how filler is placed, not the filler material itself. Also, repeat use of fillers will not stretch the skin. Hi Vanessa Palma, The muscle layer over the bone level prevents filler from migrating, and it is stronger than the fat, soft tissue, and skin layers, so it can hold a larger volume of filler without migrating, but still look defined, and not soft and doughy like when filler is placed in the superficial layers. Dr....Is rhinoplasty a very safe option?....are the results permanent?...and also is it major surgery or!!!...........Dr...Is there any surgery for small corrections in nose shape other than rhinoplasty which is permanent with not a single side effects!?

Hi MG M, Rhinoplasty is a very risky major surgery, and results cannot be reversed, only adjusted. There is non-surgical rhinoplasty that uses fillers and Botox, but results are not permanent, and less risky, but it does require maintenance of fillers and Botox periodically. Love the videos very informative. What is your opinion on microneedling? I hear it’s great and seen it turn back the clock and completely thicken skin and remove wrinkles completely, however I have heard of some people getting infected deeper in the dermis and developing a foreign body granuloma which in most cases is permanent because the inflammation is deeper it can’t come out. What are your thoughts on this?

Hi Shiloh Jetson, While I do perform microneedling, I lean more towards different laser treatments nowadays as they are more precise and controllable collagen induction therapies than microneedling. When done judiciously, laser treatment can avoid the complications you mention, but overuse, overheating, and overexposure of lasers can lead to their own set of complications. In the end of the day, whether it's microneedling or lasers, it is the practitioner who determines results and avoids complications, not the devices themselves. I hope this clarifies things for you.

I have been watching your videos on structural volumizing, and I have 3 questions: 1) I need to get dental implants in the back. Should I wait until I have completely recovered from that? Asking not just from a healing perpective, but an esthetic one, as the implants may affect my jawline? 2) How long does this procedure last? 3) How is the pandemic affecting your clinic operations? Hi Maria, Nice to hear from you again! 1) dental implants are in the inside of the jaw, while placement of filler is in the outside of the jawbone, so it should be fine if you have dental implants before having filler treatment. You should note that if you need dental implants, it is a result of bone volume loss, so the timing for both treatments sounds about right. 2) Voluma is the thickest hyaluronic acid filler, which lasts about 2 years. Since it is placed at the bone level, and in between the muscle layer, this level does not have natural hyaluronidase which metabolizes hyaluronic acid, so it should last a little longer than usual, but the filler material still breaks down from normal wear and tear. 3) I'm happy to tell you that we have been open for awhile now, but with added health precautions such as the use of face shields and dividers, and now our patients also wear masks, but we've always worn masks in our offices. New York State and New York City are doing much better with keeping infections down as compared to most of the US, especially since we were hit first. I hope all this clarifies things for you.

Hi julesjules, Filler treatment does come with its own risks, so like any procedure, risks are reduced by the experience and execution of the doctor, such as sterility to ensure that filler treatment does not cause infection. I hope this clarifies things for you.

You said that it is generally more beneficial for fillers, but as far as I can see, fillers applied to the chin leave a sagging or, how should I say, frog-like appearance on the chin after a few uses? Your colleagues say? Do you think this is true? What are your thoughts on this? And if a patient comes to you and says, I don't have a budget problem. I want to make both my jaw and chin more masculine. In this case, would you recommend implants or fillers to your patient?

Hi @hamzbana6336 , Fillers will not cause your skin to stretch after they are metabolized, or after several separate filler treatments. Implants are still an option, but keep in mind many patients find them uncomfortable after they've had them placed. Hi Joe black, You are still in the border of your face still growing, so I wouldn't recommend a chin implant done at this time. An examination is needed to determine what you want to achieve with a chin implant, and how it would it fit your face. I hope this helps you.

For those with immune overreaction issues, which fillers are the absolute safest to use? Also what happens to fillers in the face when sleeping. For instance, I don’t sleep on my back. I sleep mostly on one side and my cheek gets scrunched up firmly against my hand during the night. Surely that would push the filler to other parts of the face because there’s nothing really holding it in place? Same with chin filler. Does this mean you can never really lean your chin on your hand again?

Hi Hector, Hyaluronic acid fillers are the safest to use because hyaluronic acid is naturally present in the body, so these fillers are hypoallergenic. As explained in the video, I place filler at the deeper, foundational bone level, and the filler is held in place by the strong muscle layer, so you don't have the problem of migration, or the soft or doughy appearance that can occur when filler is placed at the more superficial dermal, fat, or soft tissue layers. Hi MA, While they will likely last, I would have to see you before they can be recommended in the first place. At your young age it's volume augmentation, which is only recommended if there is a genetic volume deficiency. I hope this clarifies things for you.

Hi Jasmine :D, Juvederm Voluma generally lasts longer as it is the most viscous/ thickest filler from Juvederm, but filler viscosity is chosen not for longevity, but he viscosity appropriate for the area. The most viscous fillers like Juvederm Voluma are used in areas with little to no facial movement, like the cheeks, jawline, and chin. I hope this clarifies things for you.

Hi, I’m 19 yrs old and have had my chin implant for 5 months. I have a procedure scheduled to get it removed in March. Will my chin look like how it was before I got the surgery? Also during the chin implant procedure do they do any manipulation to your natural chin bone? I’m nervous that my chin will be even smaller than it was before I got my implant. I am removing my implant because it is uncomfortable and also it has made my front profile too wide and vertical for my liking.

Hi cassiesr, Sorry to hear about your problem. Having an uncomfortable chin implant is a common issue. You should appear as you did before the implant, but you also may have had some facial growth since then, which is why I usually don't recommend volume augmentation at your age while the face is still growing and maturing. The bone should not have been manipulated when your implant is placed. If comfort is an issue with you, you could consider a thicker hyaluronic acid filler, which I would wait to do in your early 20s when your face has stopped growing. Hi Not So Cool Dude, It should last quite long as you are still very young, and age-related volume loss is not an issue in your 20s, and usually becomes more significant starting in your late 30s and older. I hope this clarifies things for you

Hi @lisaaustin4561, You can look for certified Y Lift doctors who are trained at placing filler at the bone level of the face. It's important to keep in mind that a doctor's aesthetic style should also be taken in to consideration, as we all have different styles, even if we place filler with the same technique. Hi Dr. Prasad, I have always used Radiesse instead of HA fillers... the one time I did use HA fillers, my face became puffy from them (not sure why), also Radiesse gives me sharper angles - can you use Radiesse with cheek implants? Thanks!

Hi Melissa Kane, I generally don't recommend Radiesse as because the material calcium hydroxapatite is not metabolized by the body, so some residual material will remain after most of the volume is gone, and it requires surgical removal if something goes wrong. Hyaluronic acid fillers are generally safe since hyaluronic acid naturally exists in the body, but you may have had a reaction due to an infection that may have been caused by placement technique. I would advise that you try HA fillers again, but perhaps under sterile conditions or a different doctor to reduce risk of swelling and infection. I hope this helps you.

What about removing the cheek implants? While they are in there you are stretching the skin when removing wont that skin be saggy? Is it worth considering them in order to deal with nasolabial folds caused by flat midface?

Hi 9999ty, Cheek implants won't stretch the skin, but the face may appear to sag if they are removed because the volume that gave structure to the cheeks is now gone. As explained in the video, cheek implants do need to be changed eventually as they won't fit the face eventually due to volume loss, consisting mostly of bone loss, due to aging. Volume enhancement can treat nasolobial folds, but it doesn't necessarily have to be implants. I know this is an older video but I’m thinking about temporal implants as I’m scared to get filler due to having to repeat injections every 2yrs in the temples and it’s a dander zone. I’m scared of blocking blood flow/headaches and other issues I’ve seen people mention! would a subtle implant be a good idea to help my temple area? As I can take it out if needed in future. Hi Anjali Mohan, Non-surgical rhinoplasty is not permanent because it uses Botox and injectable fillers, which are temporary. Semi-permanent and permanent fillers are not recommended because they can cause complications from staying in the body for too long. There few to no permanent treatments as facial volume changes with age, so our bodies and features are not permanent. I hope this clarifies things for you.

Doc I would like to ask about finasteride please ..can I use quarter of 5 mg finasteride pill for my hair growth because 1 mg is not available in my country?? Is it possible to increase side effects?? another question about shedding I have used minoxidil for 1 year and still using it and my hair shed for about 2 months but now no more shedding ..if I use finasteride, will my hair shed again??

Hi mohamed elsaye, Finasteride is a prescription medication, so you should only be using it with a prescription. The chances of side effects from finasteride are pretty rare at about 2% of patients having them. I've had patients who were on finasteride for 20 years who didn't suffer from side effects. Of the patients I've had with side effects, they were temporary, and restored normal function after stopping the drug. Finasteride does not treat hair shedding as it slows hair thinning. Hi Mocha B, We have treated patients with rheumatoid arthritis, but since their immunologic status is unique them, there may be a need for skin testing with filler, and coordination with their rheumatologist for their recommendation for filler treatment, or a surgical implant. Hi @nickgerr1991, As explained in the video, facial dimensions change with aging because of volumetric loss of mostly bone, but also muscle, fat, and soft tissue. While it is possible for you to have filler for the chin first, having an implant at an older age doesn't mean it will be permanent. Also as featured in the video, many people who have chin implant surgery done don't find the implant comfortable, and filler feels more natural for them. I hope this clarifies things for you.

Hi Carol, If the implant started to show with progressive facial aging, then the implant likely doesn't fit the smaller facial proportions due to volumetric loss due to aging. The implant would likely need to be changed to fit a smaller face, as repositioning doesn't affect the size of the implant. I hope this clarifies things for you.

Hi Rachel Hughes, While grafted fat that survives the surgical procedure can last long-term, 30-70% of grafted fat doesn't survive in the short-term as it needs to take to a new blood supply. This need for a blood supply makes results of fat grafting unpredictable, and it usually requires at least two surgical fat graft grafting procedures. Also, fat cannot be placed at the bone level because there is no blood supply at that level. I hope this clarifies things for you.
