My theory is that omen died in the First Light or the Point Light where all radiants got there powers. Omen said he was split apart either by being too close or directly under the First Light or the Point Light. It's intertwined with Viper as well because it is believed viper tried to save omen but couldnt. Viper also has a dialogue about sage, i forgot it but it was something about viper being mad at sage for, "letting them down" before.

(this is all seen in the trailer where omen uses outlaw forgot exact name)
