Both the Jedi & Sith have totally misread the prophecy - remember what Yoda said in ROTS - he claimed that the 'Chosen One' Prophecy could have been misread. The Jedi/Qui-Gon believed the Prophecy said the Chosen One will destroy the Sith - but it doesn't say it directly - It just said 'the Chosen One will bring balance' but didn't say for whom - it could have been the light side or the dark. there has to be balance of good & evil in order to sustain the galaxy as a whole. Anakin did bring balance in the end, but he had to sacrifice himself to do it. Much like Tony Stark in Avengers Endgame.

This Darth Vader has fail safes; he's weaker, in some ways, his armor keeps him dependent on it, and is super-vulnerable to the Emperor's force lightning. He wasn't training Vader to replace him; he had won, and never intended to pass everything on to his apprentice. In part, though, that only worked because of what happened to Vader; what he didn't see, or plan on. If things had gone more smoothly, and Vader had retained his strength, and robust vitality, what wad Sidious' plan with THAT Vader? Maybe the silly dyad thing would have worked, where it couldn't with our Vader, but Vader would have still lost his family (probably), and been set to just keep growing stronger, without any of the limitations Palpatine could exploit. He was still a mighty Sith Lord, and used to existing among myriad threats to his life, and wellbeing, but Vader could have outgrown him, and then what? Hiw did the Emperor plan to deal with him, when, even broken, he didn't just kill and discard him? Do we know, currently?

Man Anakin is a reckless hothead but he was manipulated his whole life. His relationship with Padme is the only real decision he’s ever made beside turning to the dark side. That still was layered in manipulation. Crazy how much the Jedi feared his turn but also overlooked and gatekept him. If they focused on flow and change instead of the light winning the war they could’ve evolved. Unfortunately they become tools to their own downfall.

force visions are not misleading, they are the Force allowing whoever has the vision to see the current path of the force they are on. for Yoda the Force showed him what was ahead on his path. because the Jedi were deluded they always misinterpreted the Force, as they stop seeking to understand and deepen their connection with it. this is why, in my opinion, Yoda did not understand his force vision. he stop looking at the Force as a living being. Anakin never even learned to view the force as such. as he thought his vision of his wife was a possibility as well, when in fact, he was on the path to the dark side and the force showed him the outcome of him turning to the dark side. his wifes death. if they viewed the force as a more omnipotent existence. i do believe they would of understood that the Force visions only allowed them to see in the direction of where the individual was headed. not a possibility. but instead the outcome of remaining on the path that was against the Force. the ultimate goal of Force Sensitives is to become one with the Force. not most powerful in the force.

If this is cannon then it makes zero sense - Yoda didn't see that the Sith would want to turn Anikan first? Even when he saw so clearly how vulnerable Anikan was to the Dark side? Complete contradiction. And frankly this happens far too often in Star Wars to advance a plot and it ends up making one side or the other look like complete idiots. Yoda couldn't do anything right . Yoda brought the clones into the Republic . Yoda trained Darth Tyrannus. Yoda ran away and hid when Darth Sidious took control . When Anakin came to Yoda for advice , Yoda basically told him let your loved ones die and be happy about it. The corruption and arrogance among the jedi happened under HIS watch as grandmaster.
