This is absolutely fantastic. These are the kind of guidelines needed to create a good campaign. I would definitely have liked my DMs to ask me these questions before we played. Would have avoided some undesirable interactions.

I think that a survey like this would actually be highly useful information going into a campaign to help tailor it to your player's interests. I also think it'll be useful to do something like this after the end of the campaign too.

I just started using a survey in my last mini campaign and it was very insightful as to the type of game my players wanted but your questions actually fill a gap in my questionnaire. I send mine through a google survey and have the option of responding anonymously just in case.
