Thank you. The slab looks fail possibly in four direction. Why we consider as if it fails in two direction?

the slab fail in 4 triangular planes. however, the deflection is considered in two adjacent sides. The remaining two sides have the same deflection with the parallel side. Thanks.

It would have helped if you said the yield line pattern is as a result of the type of spanning of the slab whether one way or two way spanning slab

Yield lines originate from intersection of axis of rotations.
