I have a question, please. I’m someone who had made the choice to allow my Registry to lapse. It was NOT revoked. I was out of radiology for a number of years. I’m interested in retesting. I am a very good student and am wanting to get a high score again. Then I aim to take a community college program in MRI and be certified in that. It looks as though I must have a current Registry, AND I would like to refresh my capablities anyway. Do you have any advice for me on preparing for my ARRT exam in the near summer months? I’d like to be ready before the Fall semester for MRI program.

I just recently got a X-ray for my left knee, the radiologist didn’t use any protection shield for my a upper parts. I felt very uncomfortable right after the the X-ray, thinking that perhaps she should have protected my upper body. Your video clarified my concern. Now, can someone order or demand a protection shield to a radiologist and what is the best way to do it?

Hi there! As a patient though, you are your best advocate and we encourage you to ask for a shield whenever you have an x-ray exam. "I would like to be shielded for my x-ray exam today" would be an appropriate request. The National Council on Radiation Protection and Measurements (NCRP) recommends that shielding is not provided for abdomen or pelvic studies. The American Society of Radiologic Technologists (ASRT) supports this and has created educational materials for patient education related to shielding and radiation protection best practices. Even though a knee exam would fall under the recommendation of shielding, there are additional considerations for radiation safety practices during an exam in which you were not shielded that include collimation of the radiation field size, low dose radiation exposure in imaging, and reduced repeat images. If these processes were followed, your dose to radiation would have been extremely low. One problem is guessing the correct angle for mobile focused grid use. To determine this, use a cube (or square once you understand the principle). Stand it so one edge is along the central line of the grid. If the tube focus is in alignment, there will be no shadow cast over the line or onto the surface of the cube above the line. I call it 'the sundial effect'. Once your tube focus is proven to be in exact alignment with the grid, you can then angle the tube laterally on the grid without cut off. The Townes projection of the skull was done incorrectly 40 million times due to an error made in the 1920s. The result was the face and neck were included in the primary beam for no diagnostic reason and not shown on the radiograph because that radiation missed the cassette. Shoulders are the hardest to xray because posture and injury can cause the shoulder to be raised, lowered, shrugged forwards or pulled back. All of these will change the position of the scapula. In my 40 years of practice, I never did an AP scapula. I always did it PA with a 10 degree down tilt. That was mainly because in my early days the focal spot size was too big and the exposures too long. I also never used any centring points, only knowledge of anatomy and of the way the radiograph was supposed to look. Can you pass the xray beam geometry and grid test? There is never a reason to centre through the ankle joint. That idea was developed in 1920, long before the light beam diaphragm. Centre several cm above the ankle so you can exclude any radiation dose to the foot. Changing this centring point means nothing as any change in geometry is not recognised given the variation in patient size. No collimation was demonstrated, so does that mean every axial shoulder include a pointless exposure to the elbow, forearm neck and part of the head? From my 40 years experience in radiography, I couldn't see what you did as working. It would require some tube angulation to direct the shoulder onto the receptor.

I would ensure to collimate less then. 24x30cm and rotate the collimate so I can exclude a lot of the chest from the primary beam
