I think it normally depends on the guide you have at the time - our guide said they don’t usually do this but he just got loads of content for us! But it wasn’t an additional fee, it was included, we just made sure to give a tip before we left 😊

I believe it’s a customs declaration form, but whether or not you’re importing goods you still have to complete the form to enter the country. This is the rule coming from the UK but im not sure about the US, but if you haven’t done it prior, they’ll just make you do it once you arrive in the airport.
