So, I recently was in the Er / then hospitalized. I’ve never before had to go into the hospital in my 45 years (minus my two births)…. The second time I had to be hospitalized because my oxygen was at 70 - I couldn’t breathe when laying down and felt like it was murder going up a flight of stairs…. Anyway, the ER gave me 80 lasix in an IV shot - and didn’t prep me with one of these prior to giving it to me. No one explained to me what would happen - and just told me call me when you need to go to the bathroom. Not 5 min later, I’m calling because I need to pee. They bring this to me and start setting me up - now, realize I had been completely calm and cheerful throughout this but, idk if it was because they hadn’t prepped me that this is how I would be peeing or what but my blood pressure went through the roof and my oxygen way down - and I start hurting like hell in my lower back…. Anyway, I had to be hospitalized so they could drain all the excess fluid from my body (lost 20 pounds within 4 days) but I came to really appreciate this. However, k don’t recall them securing it in this manner AT ALL. So many steps I feel were missed when doing this. Idk how many times I had to buzz them because when changing they would forget a step (I wouldn’t hear the suction - and knew something was wrong). Luckily, I think it was only 2 shifts were I felt it was badly placed because I felt like I was sitting in pee.

They get mad at me sometimes because I absolutely refuse to let them put an IV in my hand. It hurts so much more than any other site and continues to hurt for a long period of time.

As a patient take your time im sick of assholes blowing my veins out and they hurt for weeks. I bet they never fully recover ever. Im probably gonna die of a blood clot later in life cause of shitty interns

as a 30yr veteran of the OR CST, and now a instructor at a local college, i can tell the most important reason you remove jewelry(no fake-nails, nail polish, etc)..is because it has been shown to harbor bacteria. Whether you are open or closed gloving. we also refer the top folds as cuffs, not lips. A very old term ..my OR RN mother(yup..i basically grew up in scrubs) used to use was "miter" your hand..which meant to cuff it. Otherwsie a very good video, with alot of key points.

New research has shown for peripheral iv, dressings do not need to be changed if not infected, soiled, or damaged. In fact research is showing a higher risk of infection or short use span when being changed often 😊 Central lines continue to be changed every 7 days 😊

Excellent information. the Policy at Trinity health livonia as of 6/18/24 is still to require a change after 7 days but hopefully we see policy changes that reflect the new findings soon.

Hospitalized 3.5 days for PE, also have hiatal hernia, tachycardia. They gave me the 2500ml unit. Instructions mostly similar but didn't mention holding breath until it drops. I'll change my method. I've gone from 1200 to 2500. Any issue with getting a higher volume unit? I do have trouble maintaining the right zone for more than a couple seconds maxed at 2500.

I wish videos could be done on patients that have hard veins, extremely crepey skin, no muscle tone, rolling and sclerosed veins from drug use. The reality of drawing blood is so different from these training videos.

yeah, the legality of that is touchy, so I stick to the manikins in most cases unless I use myself or a friend. The thing about difficult venipuncture is that the best way to get better is to keep trying. I look at the videos as a way to get the groundwork in and give you what you need to start trying. once you get some confidence under your belt the trial and error phase is where the real learning comes in.

@@nursingeducation3952 Yes, I get that. However, I have had nurses tell me they will do it when I miss the vein and I refuse to dig around. Funny thing is those nurses didn't get the vein either. One nurses was an absolute disaster and I had to tell her to leave. My biggest concern for my draws is making sure I have good samples and of course the correct patient!

What about a 24 gauge? Especially for use in the elderly...this is what I would go for.

24 gauge will work for most things. They aren’t a common stock at my facility which is why I don’t really mention them. There are some infusions that may not be able to flow well through a 24 gauge because it’s smaller. Blood is the only thing I know of off the top of my head. But anything smaller than a 22g will likely clog/clot with blood and potentially can cause hemolysis.

I'm curious as to how many people actually work in this ideal environment. I'm usually having to singularly hold legs apart, either situate a light on the bed or in my mouth. The extra drapes, while may be helpful with an unconscious, paralyzed patient, are more of hinderance than anything.

Curious, I had a blood draw where they put the vac tube connected to the butterfly needle prior to insertion. I feel like he already broke the vacuum seal prior to inserting as I did not hear the normal pop is dangerous for the patient?

I guess what I’m asking is a possible for air to exit the vacuum tube and enter bloodstream if the vacuum tube is connected to butterfly needle prior to insertion

If they broke the seal it wouldn’t have pulled any blood. So if it was a successful draw they likely set it in the adapter and once they got access pushed it onto the needle and got the blood. That’s pretty common practice from what I’ve seen others do.
