What happened is since the cap is not directly a part of the bottle therefore it passes the pressure equally to the liquid as well as the glass instead of the glass which first takes the impact on itself and breaks Therefore the sudden pressure change inside the bottle causes the gas in the liquid to burst up due to the reaction thus forcing the cap out and redirecting the bottle towards the thrower

It is not necessary, you may take same. If we keep them at different distance ( moment arm) they will produce different torque even weights are same
