I knew it would go wrong when I saw the big changes in height of the deck compared to the ladder. Also they didn’t have any safety equipment, he should be attached with rope to the ship he was boarding and they could be more careful that way.

Le conducteur de la vedette n'a pas de l'expérience pour l'accostage par mauvais temps et le pilote a hésité longtemps pour embarquer ,il aurait dû attraper l'échelle de pilote lorsque la pilotine était au sommet et non pas sur la bas de l'échelle,tous les 02 deux manquent d'expérience et surtout le pilote doit être agile,de cette façon l'accident n'aurait pas eu lieu ,c'est un pilote maritime de plus de 30 années qui le dit.

By the looks of the ding next to the ladder the injured pilot this ain’t working for you guys! Why not run a hoist with a proper harness so that you leave a 5 meter gap between vessels?

So Stupid , with our technology nowadays. Have you ever heard of a safety harness, or a crane? What does a helicopter do when it saves someone? this is medieval . I've never seen a pilot boat with a platform like that. Almost all of the ones I've seen they get on the ladder from the deck for just this reason. Why have another structure that can trap or pin you? In Bremerhaven, Germany, the pilot boat actually has a retractable boom. They telescope out, pilot climbs on, they telescope it back. Of course, that pilot boat is huge.

The only one I've seen that scared me was in San Francisco. The guy was disembarking our ship in rough seas. He jumps off the ladder onto the roof of the pilot boat's wheelhouse. I came close to throwing the lifering that day.

Sehingga selain memperkuat armada timur juga sewaktu - waktu mungkin bisa ditambahkan untuk memperkuat armada wilayah TNI - AL yang lain.
