Here’s something that is just pure truth and it is unforgivable. If you are white and American and you apply for Medicaid because you are elderly and need to be in a nursing home, the government will do the equivalent of giving you two middle fingers, insulting you, and punishing you financially. Then they will give you impossible or very unrealistic stipulations to complete before denying you three times, then maybe-JUST MAYBE, if the planets align, it’s a leap year and you’re still alive at 90, THEN you can get approved. BUT….. If you’re an illegal, you get an instant check and pre-approval for it all. Your biggest problem is deciding on the amount to make out the blank check they give you. You have one, MAYBE TWO pages of paperwork and that’s only to confirm where your money goes so you don’t get inconvenienced. This makes me vomit.

Dave Ramsey says the average nursing home stay is 2 1/2 years, not 1 1/2. Which is it? Is it more for women? That’s important to know when coming up with a plan. I’m a male and single. I don’t want to stay more than 1 1/2 years in a nursing home. They can overdose me after that and I’ll be fine with it. Quality of life is an Issue.

My vote is for MH Zarr Patrick Kennedy! I am advocating for solutions to this system from the perspective of what went wrong. We have to do more prevention. Yes. Which means also some serious educating to the public of warning signs of psychosis, especially cannabis induced, to avoid full blown SZ or SZA from THC. We need the public to become aware of anosognosia as an obstacle to care. I believe 2 solutions could stop the churn of repeat hospitalizations: an effective AOT law that connects patients to care in the community. You mentioned clubhouses for SZ. Those will not work without an AOT. After MA+CT pass the AOT, we streamline it with best practices as a guide so federal funding criteria will ensure the most effective and strict criteria for an AOT. During an AOT, the Stepped Care Solutions 2.0 could be implemented on a daily basis to see what level of care is needed, ranked from mild to moderate, 1-10. Software on your phone that connects you with local therapists, online therapists, group therapy, a peer, and during the acute stages connects you with local fire and rescue. Methadone saved my life but I got a job as a software engineer and moved to California and they made me drive 45 minutes every morning to pick it up at the clinic for 2 months making me late to a new job for 2 months even though in NY I got 20 take-home doses. I also have panic disorder and I've taken Valium for 20 years as needed (it's never been a problem) but finding a psychiatrist in a new state who would prescribe it because I was honest about being a former heroin addict was a nightmare. And there's no quicker way to relapse than to self-medicate because a doctor takes you off of an anxiety medication you've relied on for 20 years because "there's a stigma". Not to mention that I often have the choice with doctors of being honest, or being taken seriously. It's tough.

People have a right to not have homeless people endangering themselves and families. Most homeless people are not dangerous but some are. Because nuance has been denied on this issue by progressives, we now have a reaction and swinging to the other direction.

The very notion of division is the issue. "Those people" are US in either a separate point in time or life tangent. Life is precarious at best and what history writes about a people is quite possibly dependant on how it handles it most vulnerable. We have to accept that "they" could be "me" with the right set of circumstances. Think "Freaky Friday" and put yourself in someone else's shoes for just one day.

Thank you for sharing these important insights! I have a KCNJ2 variant in which there is not yet enough information gathered to classify it as pathogenic (it’s considered probable pathogenic by Varsome and benign by Invitae). Based on family and personal medical history, as well as physical traits, Andersen-Tawil Syndrome is suspect, which is estimated to occur in 1 in one million and can result in fatal cardiac arrhythmias. My doctors and I are currently managing my symptoms, though many are have to deep dive into research to learn ad hoc because doctors just aren’t taught the nuances of many rare diseases in medical school. Facebook support groups and the Periodic Paralysis Association have been incredible resources. In my own journey I’ve heard so many go through similar situations. I’m so grateful for how far we’ve come and, having medical background and continued interest, I’m constantly curious about ways we can continue to move forward. For those of us with rare disease who have interest in research, what do you believe are some ways that we can be most effective in advancing research? What kind of resources are available for those that want to go into academia, to improve the healthcare system, to improve data, etc. in rare disease? Where do you see the greatest needs in terms of a career in rare disease?
