What do you do when your client is ambivalent in a way that is négative. Like this: on one hand I could take ask them to give me some tools so that next time I can succeed in doing the job, but on an other hand, if I ask for tools, and they give them to me, I am not sure I would use them, so , I will not ask for tools.

I love this video and your words of wisdom. My problem is I sometimes deal with clients (adolescents) who are kind of on a timeline--either change or go to jail! As a recovering addict myself, it took a sufficient amount of pain and a sufficient amount of hope to get me to change. Pain = the unacceptable consequences of continued drug use (prison). Hope = life will be better when I am clean and sober, and I know I can do it. But, I meet these kids who are forced into treatment and who don't want to change. Many are in group homes, and they face pain and misery on a daily basis, so substance abuse is their escape. And, the consequences of their substance use/abuse hasn't been as severe...yet. I find myself going in circles with them using MI and even a little CBT, and I wind up "preaching" to them which does not work because they already get a ton of that. What keeps me grounded is my recognition that I am a helper, not a fixer. If I wanted to be a fixer, I should have become a mechanic!

Hello. I am a substance abuse counselor that works with many clients who waiver between contemplation and pre-contemplation, or ambivalence vs indifference. As a counselor, it is frustrating for me when it seems like progress is being made (change talk and movement into preparatory behavior), then they slide back into precontemplation (I don't really think I have a problem). Maybe after I watch your MI with a precontemplation client video, I'll get some tips. Always willing to learn more.

I find that when I teach OARS to health care workers, they come back after one lesson to tell me how they used it with their kid/spouse/etc... instead of in the work place. I'm always excited they were willing to try it anywhere, but it's clear they see the usefulness right away in personal relationships. As an aside, I find parallels between the OARS and the parenting book "How to Talk So Your Kids Will Listen, and Listen So Your Kids Will Talk" by Faber and Mazlisch.

I love your point of removing “I” from feedback. People with trauma can be very sensitive to this messages about their worth, no matter how subtle- which makes a lot of sense when we think about acceptance and safety. Indeed, MI calls on us to accept our clients as they are right here, right now, and so removing the conditional “approval” is key. Thanks for the reminder!

Addiction is very saddening and maddening. I left my husband of 35+ years last July. Caught him smoking drugs in our bathroom at 3:30 AM. It was truly the final straw. Plus, it scared me near death. He is almost 63 years old. Was diagnosed with congestive heart failure last February. Assumes it's just genetic, not drug use. Lives in denial. Unable to speak the truth. So, enough about "him". I have nothing more to give. I am in receiving mode only. Recovering from the addict.

I don't know if you will see this but I am a program manager for Nurse-Family Partnership. I support a team of nurses that goes into the community to support high-risk 1st-time mamas. We are learning and implementing MI and this channel has been a "game-changer" I cannot thank you enough for your work and for sharing it in this forum. We have trained Nurse-Family Partnership staff before. YOU DO SUCH INCREDIBLY IMPORTANT WORK! I'm so glad to be helpful. Holler if you need any "MI booster" sessions, our trainer Chris Miles would say "working with nurses is my JAM!" and she integrates a lot of burnout prevention in her work.
