U got ur facts on the coffee one wrong. You said few grams instead of 0.6 grams, which would only be 6x the recommended dose. Thats not what happened. He mistakingly took a very large dose of caffeine powder thinking it was whey protein or something else thats consumed in large doses. Thats why he got few hundred times the recommended dose.

One of the tasks of the pilot on the aircraft was to double-check that everyone who was going to jump, had on their parachute and clear them to go before they left, as an additional safety measure. He didn't notice the missing parachute, either, because the camera pack looked like a parachute.
