Because it's a horror game disguised as an edutainment game (just look at the original, for instance). While Plus lacks a lot of horror stuff right now, it'll absolutely bring it when the full thing is ready.

Yep. If you manage to trap Dr. Reflex in a spot where he can't have immediate access to the hospital (where he normally gets his hammer from), he'll manifest it immediately.
