Gleason 3+4=7, TOTAL PSA was 2.9; however, PERCENT FREE PSA was 9.3 ( the initial red flag and why I did a 3Tmp MRI which found several spots). Opted for Focal Laser Ablation (FLA). FLA process was without incident, recovery has been excellent thus far. Timeline was March for MRI, April was the MRI guided biopsy, early June for MRI guided Focal laser ablation outpatient procedure (all done this year 2024). Positive vibes and prayers to all that have this cancer.

Yes the score is confusing. My score was 4+4 =8 But then they said 4+5=9 . I was stage 4. Had a radical prosectamy. A recent pet scan showed no cancer . I decided no radiation. Psa is climbing. .going to enjoy myself

I am aged 66 . My PSA is 4.1 The MRI prostate w wo contrast RAD showed 4 and the biopsy report from 12 samples showed one RA , biopsy 3+3 and one RMPZ biopsy 4+3 . The bone scan showed no spread . Am i considered as Intermediate Risk Prostate Cancer ? Should i do Radiation therapy with or without hormone therapy ?

An excellent video - but the elephant in the room was somehow missed. You talk about side effects for surgery and radiation, but not for hormone therapy. This is a pity since some of them are both physically and psychologically important and may have a considerable effect on the dynamics of the couple. From the point of view of the oncologist its understandable that they focus more on curing the cancer than on preparing the man for the ways his life may change after the treatment. Understandable, but I feel it's selling the guy a pig in a poke. Or to switch metaphors, when offering advice to a complete novice in chess, telling them that taking the queen is a really good thing to do, without warning them to foresee how that capture might degrade their future prospects, does not help the novice to understand how to play the game.

When I was pregnant with my daughter I started leaking milk every day at around 6 months. I'd go through 5 or 6 breast leaky pads a day, especially after a shower. I asked my ob/gyn how to save it up in case I was unable to nurse quickly after birth (it had been a terrible pregnancy), and they told me that it was useless and I couldn't use it. Turns out, I really should have done so because the birth was even more of a mess than the pregnancy. Why do so many doctors/hospitals have such different views and policies? So many of them seem downright adversarial to what mothers want and feel is best for their baby. Even within the same staff at a single hospital, they will come up with 4 or 5 completely different answers to one question, it's very confusing.

I'm on active surveillance based on Gleason 3 + 3 = 6 since 2020 First year was PSA test every 3 month, since second year reduced to every 6 months. Prostrate size 60cm first PSA was 15. has been 19 and lowest 12 last PSA was 15 now into my 4 year. Two cores were found cancerous out of 16. Had two biopsies 2020 & 2021 were in 2021 they only found one of the previous two cores. What they dont tell you is that cycling raises the PSA

This is asuperb video for anyone facing this decision. I was 49 at diagnosis and treatment. PSA7.1 Gleason 7 (3+4) T3A I took the radical prostatectomy route . I saw it as the fastest way to a cure and was also advised due to my young age this is a more favourable option. I lost half my nerves as a result and sadly still suffer with ED 18 months on. I was incontinent but after 3-4 months I regained most of it and as of typing this I am now a good 95% with just some minor mishaps here and there. Make the decision that’s right for you and consider all options. Life will go on afterwards so keep that at the front of all your thoughts. I thought this consultant in this video was fantastic and wish he had been mine. Hope this comment helps the reader.

Interesting discussion. I'm on active surveillance based on Gleason 3 + 3 = 6. This surprised my consultant, who hinted that my PSA, 8.4, PI-Rad 5, scores pointed to something more serious. I feel I've dodged a bullet but that it's more of a reprieve than anything else because my PSA last year was 5.3 last year and rose to 8.4 this year. Prostate size, 38cm.

What they missed on the discussion was erectile problems after beam radiation. It is the reverse of prostatectomy. After 6 months mine went to nothing and viagra isn't really helping so far.

Why does the abscess pathway only relieve pressures for the registrars? Why not the foundation Doctors and SHOs too ? Do PAs see themselves as equivalent to a foundation Dr or a SHO?
