They are tricky to keep. They require deep sand beds, moderate flow, and they are very shy. You should make it a spotted garden eel tank first before you make it a reef tank. You could probably tell if they’re sick the same way you would any other fish, by looking for physical and behavioral anomalies (spots on the fins, not eating, etc).

Actually this is something that does happen in the wild but it's with crabs, crabs will often be seen with different species of anemone attached to their back it's a basic symbotic relationship actually! The anemone gets protection and access to more food (crabs garbage) and the crab gets extra protection and food (if they end up catching a fish or something that's dinner for the crab plus anemones)
