Been deal with Neck and upper back chronic Myofascial pain syndrome since 2006. I do have cervical central canal and foramenal stenosis. My Neurologist is on it now, had EMG study. Dealing with the upper back/neck pain/cervicogenic headaches all daily. Diclofenac and muscle relaxers along with home Conservative message care. All helping some. Will be requesting PT treatment and better direction on home treatment. All this trigger point injections stuff makes me a little nervous but am now considering possibly maybe acupuncture. I’ve had carpal tunnel surgery based on 6 months of symptoms and a moderate EMG in both hands . My surgery in my right hand appears to have failed at 2 months post op. My hand surgeon says the nerves are too weak and the EMG appears to have been inaccurate. I’ve never heard of a faulty EMG except one that might read negative, when symptoms say positive. She is sending me to a neurologist without touching my left hand at all. I’ve got burning pain and asked for a steroid injection for pain and lack of dexterity in both hands to help me until I can get a neurologist appointment and a repeat EMG of my untouched left hand. She refused saying they’re only used to predict success or failure of surgery. My left hand has not had surgery at all. What would you advise your patient to do at this point?

My Dr ignored the hyperflexia, he even held my leg down so it didn't fly up like it does. When I first went for neck problems they said I needed surgery- I moved and in this state they totally ignored my MRI and symptoms. They blamed it on carpal tunnel. Had the carpal tunnel surgery- Failed. The surgeon had a fit when the neurologist said it was a failed carpal tunnel, then they said after surgery you will Always show during an emg you have carpal tunnel. I kept asking to fix my neck---they refused! Come now, surgery is urgent. Myelopathy....JUST as I told them, they failed to monitor it. So mad, but now they HAVE to do it, the delay makes me now have permanent damage. I was so gaslighted, i was near suicide.

Truly believe with all my heart and soul that neurosurgeons use us to make money, there is a thing called a spine market. I watched it on KZfaq. It’s a bunch of doctors some from Penn, which is where I had my surgery in Philadelphia. They sit around the big table and they say if you have a patient and they need a double fusion and try to be more conservative. You get paid more because they’ll come back because they’re gonna need another level fused it is a grimy thing to do to a patient. They are in it for the money, especially when you’re neurosurgeon only spends four minutes in the room with you.

I am so bad after I had the ACDF surgery that failed. I have home healthcare that takes care of me 59 hours a week and my home which is a family member. I can’t attend functions with family because the pain pain I cannot golf anymore. The more I move the more pain the doctors don’t listen sometimes I feel like I need to bring a KZfaq video for them to watch before they even try to talk to me.

I consider the spine part of the brain. One thing about fixing it later is the risk I face, osteoporosis. Had compression for 19 years. Pain. Now I have myelopathy, dropping things, at times excruciating pain, tandem gait, tingling. I'm scared.

They saw Myelomalacia at C6 C7 I had a CDF surgery for the compression and surgery failed. There is a nonunion my surgeon wanted to do surgery immediately. I said no I went to go see three different other neurosurgeon and they said absolutely no more surgery. I’m just so confused at this point, the pain is, it’s so unbearable. It makes me not wanna live and I mean that sometimes I sit here and wonder how I plan to kill myself another day. I have a pain management doctor. I paid $5000 for a year for him. He put me on MS Cott45 mg every 12 hours with Roxy cone 30 mg breakthrough pain. I’ve been on Roxicodone for 3 1/2 years. It doesn’t work.

I'm wondering about the affectiveness of EMG. My serious symptoms do not typically fully activate when the body is at rest. How can EMG show myelopathy when the patient is being tested at rest?

So awesome to see this channel. As a patient of yours for the past five years I’ve had so much improvement from the advanced procedures your practice has provided, the most was from the Medtronic implant, and the injections help more than the medication. Use Pregabalin, dosage from 75mg to 150mg. It works. Tolerance is always a factor, up the dosage as medically recommended.

I have mixed symptoms and signs of cervical radiculopathy/spondylosis and ? Myelopathy. I also have grade 3 lumbar spondylolisthesis and spondylosis with DDD. I was denied a cervical MRI after PT. My home program is fair. I noticed subtle problems with balance and dexterity. Any thoughts?

My Mother had all of the Above after "Routine" Surgery on her right Knee. I don't what the Hell went on in that Operating room. But she firstly lost the use of her left arm. Then it progressed to her right arm, then her legs, and eventually all her motor functions. And her bladder and back passage functions. Then eventually ended up in her losing even her ability to speak. She was just a "Living Corpse" towards the end of her life. She Passed Away 4 weeks ago. All, for a routine surgery of her right knee.

The earlier interventional spinal nerve block you mention, does that involve the trigeminal nerve, or for people with shingles on their upper regions such as neck or head? If not, is there a similar early interventional nerve block higher up the spine which replicates the success of the study you quote? Either way, I would be willing to take various nerve blocks asap because my preexisting, not awful trigeminal neuralgia was slammed into the pain and intractable itching stratosphere by getting a full blanketing of shingles, of course, in my neck, scalp and ear area, prime trigeminal nerve territory aggravated by some pinched nerves in the c2-c4 areas. So basically a tripling effect of 1. the pinched nerve, 2. preexisting trigeminal neuralgia amplified by 3. an incredibly severe case of ear, neck and head shingles with non stop zapping and scratch until you bleed pain. So far massive doses of pregabalin, carbomezapine, dilaudid, as well as lidocaine, cbd oil, even ketamine topicals have done nothing. A sub cutaneous nerve block was given in the upper c level spine, but nowhere near the nerve root. It helped a bit with the scratching the scalp to the bone bit, but didn't address the excruciating neck, ear and lower scalp pain that makes night time a sleepless nightmare. Still hoping for another block to address this area, but our major city has only one pain clinic that you have to wait 4 months to get into to get a pain block that takes 2 minutes to administer in an ER. I do keep 2 VERY graphic photos on my phone of my neck and ear and part of my face in full shingles lesions bloom, because when doctors are looking at you writhing in pain and your face, ear, and neck look pleasantly pink, post-lesion healing, and now appear barely touched by shingles, they need that jolt to see just HOW extensive and severe the shingles actually presented to appreciate the potential and likely severity of the current nerve damage. The irony is, that when I was in full grotesque lesions bloom a month ago for only a week, my pain was 1/100th of what it is now when my skin looks normal. I had no idea what lay ahead for me, and am now DESPERATELY trying to get the intervention I need before it's TOO late.

Only 4 comments? These guys are removing any comments that criticize their practice... ESI’s are not FDA approved! In fact they have all sorts of severe adverse event reports including permanent nerve damage, Arachnoiditis, and even death. Ask for the name of the particulate steroid they wish to inject, look up the package information or insert and read the FDA’s WARNINGS that were implemented beck in 2014. It says to stay away, they’re dangerous even with fluoroscopy!

I would like to ask what we do when the source of the pain is rather disregarded by orthopedists? They offer pills, try cortisone, have surgery, try PT. But they minimize the fact that the pain is real and the source should be discovered

I wish I could just talk to you guys. I suffer from permanent cervical myofascial pain syndrome. As long as you have myofascial pain syndrome in your title, I'm not offended. You both were spot on. I currently do lidocaine injections in the cervical area. I wish they had something for the neck but can't use those injections there. I also focus mostly on my posture and take it as it comes as far as treatment goes. (There is a change in lifestyle dealing with the syndrome. Many treatment options just have no long term effects. Short term great.)

I'm a 59 year old woman who's been suffering with migraine from age of 10 and myofascial spasms/flare-ups and fibromyalgia for over twenty years. It's a deep gnawing and intense pain; I used to take a knife and try to dig under the skin and tissues to get to the source (the acute pain was better than the chronic gnawing). Ive used a meat tendorizer tool, rubber balls, acupuncture, chiro, massage, but kinesthetic tape, applied along the affected areas provides almost immediate and lasting relief for several days and allows the circulation to move through the area, it was a game changer for me! for over 20 yrs I was never diagnosed, given high doses of steroid for cervical pain, told I was crazy that it was all in my head or I was a drug seeker. It wasn't until 2015, when I was diagnosed through labwork, x rays, EMG and exam with five types of arthritis, incl. OA, PA, RA DjD, fibromyalgia, herniated disks, and poly-neuropathy, that I was able to change a very progressive downward spiral and start to live again. While conventional pain medications help alleviate my herniated disks and sciatica, the only thing that helps me when a myofascial flare-up starts, includes taking a CNS depressant (benzodiazepams if your Dr. will prescribe a week or two;s worth) and possibly a week or two of Ambien to calm symptoms enough to work on myofascial release, tape and exercise. If men predominantly had these disorders, research and funding would have figured it out by now. I'm sad that there hasn't been more progress in the diagnosis and adequate treatment of the debilitating disease. Thanks for bringing more awareness and I'm glad to be part of research efforts since I'm an expert on experiencing the diagnosis and trial and error in treating it.

Hello from Puerto Rico. I’m heading in for an MRI soon. I had one a couple of years ago with no cord compression from some significant stenosis. Given my recent symptoms it sounds like I may have some Myelopathy. I am not asking for a diagnosis but about the time I felt some symptoms all of the joints in my hands where I have arthritis become inflamed and painful. It has been some weeks and the joints are still painful. Have you heard of patients experiencing the same experience? Thank you…

I'm 36 and currently getting over shingles. I was diagnosed the first week of October and the pain is just getting worse and worse even after the blisters have sabbed over and the scabs have fallen off, there's still pain. I was wondering if it normal for the pain to still be present at this stage of recovery. My doctor just tells me to use topical lidocaine and ibuprofen but it doesn't do much to take pain away. Any thoughts?

@SuperFacecloth I have since been diagnosed with PHN. The pain is still present and I take gabapentin and ibuprofen with topical lidocaine twice, every day. It's a bitch TBH.

B complex vitamin supplements Are good for the nervous system so I thought that would work to make sure we get enough the B complex vitamins to calm our nerves. This answers all my questions. Have a meeting with my neurologist coming week. I'm 60 years now. MRI shows: C4 C5 compression, no fluid and foramenstenose.Artrose. No pain but bad hand movement, numbness, loss of fine feeling in finger tips.etc.weak upper legs, muscle stiffnes, balance problem walking. Started slowly 4/5 month ago. Babinski positiv and Hoffman. So I'm like what to do? Surgery or not because I'm afraid it will progress. Thanks from the Netherlands.

At least your in the Netherlands. The doctors there are nationalised so they probably dont get paid huge amounts for each surgery they do like in the usa. SO if a doctor there tells you to get surgery its more likely you could trust him. In the USA if a doctor tells you you need spine surgery you got to wonder if he really thinks you should do it or if hes just looking for a huge payout.

Bla bla. Sorry but theory doesn't help. I take 1 piroxicam when I feel that I can't manage the pain, it helps a lot. Sometimes I have 4 to 7 knots on me and it's painful and when it hurts now one touches me piroxicam helps keep everything under control. 1 tablet 40 mg in 24h are times when I don't even have to take more 1 is enough
