Thank you for your case First of all you should assess the symptoms of the patient. Is there any symptoms of gluten intolerance? loose stool or diarrhea? bloating? weight loss? Second what about the physical examination? Do we have signs of cachexia? Dermatitis herpetiformis? smooth toungue? angular cheilitis? Abscence of intraepithelial lymphocytosis or villous blunting make the celiac less likely according to modified March Score. The most common cause of duodenitis is H pylori. Other diagnoses are like NSAIDs drug induced, Crohn disease, eosinophilic enteritis, CVID, infectious enteritis, .…

Thank you for your case It’s not in favor of celiac disease. There are two characters that should come together in celiac biopsy which are increased intraepthelial lymphocytosis and villous atrophy and measured according to modified March Score. I think your case could be eosinophilic gastroenteritis or other allergic conditions

@@Ali_AL_Eddey يعطيك العافيه حكيم عهالحالة... صراحة وقت شفت الفوهة خطرلي يجرب الواحد بمنظار الاطفال يشوف اذا بيقدر يعبر ويشوف لوين بتوصل ولو انو ممكن مايكون في فائدة سريرية... كمان السؤال ما ممكن بعد العلاج بعد كورس علاجي بال ppi بحال التئام المنطقة يتجرب بكليبسات عادية 🤔

شكراً جزيلاً الك دكتور. بالنسبة للعبور بمنظار الأطفال فكرة واردة اذا متوافر بقياس يسمح للعبور. الموقع التشريحي للناسور بين فتحتي المفاغرتين يدل على أن الناسور يعبر من المعدة باتجاه العروة الصائمية المسحوبة لإجراء المفاغرتين. بالنسبة للإغلاق بملاقط عادية، أتوقع أنها صعبة التحقق. عندما أخذت الخزعات من المنطقة المقترحة، كانت منطقة قاسية و زلقة يصعب أخذ الخزعة منها بدقة فما بالك بملاقط معدنية متعددة. لا أستطيع أن أضمن نجاح هكذا تجربة مع وجود نسيج متقرح و متنخر محيط بالفوهة إلا إذا تم الاستعانة بطريقة تحزيم أو زم الملاقط (ENDOLOOP PURSING) و كلفة عدة ملاقط قد تكون أكثر من كلفة ملقط OVESCO وحيد.
