When I do a 3 finger method, i actually end up obliterating the pulse resulting in wrong readings. What to do?

I had to look this up AFTER passing my EMT-R because they talked so little about it and they only had us measure it once, and we didn't have to do it for our practicals lmao. Completely forgot how you do it, but I wanna make sure I know how to use them because I intend to actually make use of them when I can.

This is the full NREMT test. Must be done in less than 10 minutes. Not entirely in order. You would not take vitals on scene. Make sure to initiate proper trauma protocols for your local area or department. Critical failures are automatic failures. Takes or verbalizes appropriate PPE precautions SCENE SIZE-UP Determines the scene/situation is safe Determines the mechanism of injury/nature of illness Determines the number of patients Requests additional EMS assistance if necessary Considers stabilization of the spine PRIMARY SURVEY/RESUSCITATION Verbalizes general impression of the patient Determines responsiveness/level of consciousness Determines chief complaint/apparent life-threats AIRWAY -Opens and assesses airway (1 point) -Inserts adjunct as indicated (1 point) BREATHING - Assess breathing (1 point) - Initiates appropriate oxygen therapy (1 point) - Assures adequate ventilation (1 point) - Manages any injury which may compromise breathing/ventilation (1 point) CIRCULATION - Checks pulse (1point) - Assess skin [either skin color, temperature or condition] (1 point) - Assesses for and controls major bleeding if present (1 point) - Initiates shock management [positions patient properly, conserves body heat] (1 point) - Identifies patient priority and makes treatment/transport decision (based upon calculated GCS) HISTORY TAKING Obtains baseline vital signs (must include BP, P and R] (1 point) Attempts to obtain SAMPLE history SECONDARY ASSESSMENT Head -Inspects and palpates scalp and ears -Assesses eyes -Inspects mouth**, nose** and assesses facial area Neck -Checks position of trachea -Checks jugular veins -Palpates cervical spine Chest -Inspects chest -Palpates chest -Auscultates chest Abdomen/pelvis -Inspects and palpates abdomen -Assesses pelvis -Verbalizes assessment of genitalia/perineum as needed Lower extremities -Inspects, palpates and assesses motor, sensory and distal circulatory functions Upper extremities -Inspects, palpates and assesses motor, sensory and distal circulatory functions Posterior thorax, lumbar and buttocks -Inspects and palpates posterior thorax -Inspects and palpates lumbar and buttocks areas Manages secondary injuries and wounds appropriately REASSESSMENT Demonstrates how and when to reassess the patient CRITICAL CRITERIA Failure to initiate or call for transport of the patient within 10 minute time limit Failure to take or verbalize appropriate PPE precautions Failure to determine scene safety Failure to assess for and provide spinal protection when indicated Failure to voice and ultimately provide high concentration oxygen Failure to assess/provide adequate ventilation Failure to find or appropriately manage problems associated with airway, breathing, hemorrhage or shock Failure to differentiate patient's need for immediate transportation versus continued assessment/treatment at the scene Performs other assessment before assessing/treating threats to airway, breathing and circulation Failure to manage the patient as a competent EMT Exhibits unacceptable affect with patient or other personnel Uses or orders a dangerous or inappropriate intervention

Isn't he supposed to grab MIAS first in case of an AVP trauma scene before starting with the PA? @EMTprep correct me if I'm wrong. In Addition to that, I did not see him checking his material correctly ( testing the neck collar for example ), also in case of PA procedure he is not check Airways in a good way

For everyone prepping the day before their trauma finals, STATE WHAT YOU SEE and then let the proctor correct you. Working under platinum 10 means time is everything and waiting for every question to be answered takes more time then stating what you see. Good luck!

All that was missed was maintaining manual C-spine stabilization. From what I was taught, you should immediately stabilize the neck with your hands, and once you have checked the neck during your head-to-toe assessment, apply a C-collar and continue the assessment. Never hurts to do that regardless of the MOI, but ejection from a vehicle is an MOI that definitely warrants spinal precautions.

This was a refreshing episode. I was a fire medic who loved ems and was able to take a role in my FD called an EMS LT and now I work as a nontransport single role paramedic supervisor. I love my job.

So PASTE wouldn’t be used here even with a breathing problem? Not shortness of breath but it’s difficult breathing which I’d assume you’d still use PASTE at that point ?
