Here's how my local authority does it: do a carer's assessment and give no advice, type up the notes two months later and get them wrong. Then, because the cared for is self funding, give them an out of date list of care agencies, some of whom didn't cover the area and most of whom had no staff to come to the house unless mother wanted to get up at 11 am (she didn't) and go to bed at 6pm. They system in practice is an unfunny joke.

Haha love the ending, classy! I came to this video because I have three children with special needs and was told you can tell me about a one page profile. I am so pleased that you explained it beautifully and simply. I now have the confidence to help them create their own. Thank you.

I am a quadriplegic survivor. A few years ago I could only move my left hand thumb up & down a fraction. I believe strongly in good communication especially between care givers. I also Very strongly believe in closely monitoring who gets to access and use any kind of, "Profiling”... Not one of us, Rich, neither poor, critically aware nor the very needy can part from our past. The only full stop put to genocide of those dependent on help from others and state is in our hands. At all times it is paramount to keep a vigilantly watch over whom and how our Disabled, Elderly & handicapped are managed. It is also most important to fully disclose all plans which might and do direct and indirectly affect the handicapped an absolute necessary healthy well taken care of, “Care Givers”. So when it happens as sure as the next disguised Regime go’s to save public moneys by excision behind closed doors. We all in at least will have the chance to physically if not merely voice out for Compassion to help the needy and handicapped.

Does the individual have online access to their own care plan/report, and are do they have online access to amend/update the data? Are they also permitted to give access to family/friends if they want to?

Thanks for all the questions during the webinar - I think all of them were answered apart from this one from Donna. If there are any more, then please get in contact! Q from Donna Kemp: Is there any research base to the 4+1 0r the Stay Well Plan? The 4+1 is a purely analytical tool, so doesn’t have a research base as such. The reason for including it in the presentation was the suitability of the tool for situations in which I have found in services where I’ve worked. Basically anywhere where you want to review progress and move forward, whether this is with a group of people, an individual, a partnership or within a team. The Stay Well Plan is an adaptation of a more traditional care plan, taking into account the evidence for a focus on Well Being. The key evidence base for a well being approach is the New Economics Foundation work on the 5 Ways to Well Being, which I’m sure you’re familiar with. Even though it’s not recent work, my view is that the 5 ways are an amazingly powerful way to reframe a huge amount of discussion around Mental Health, and we’re only just scratching the surface of what we could do with it.
