This is pretty common in rural Japan. Unlike hyper urban areas like Tokyo/Osaka public transportation is fairly limited in rural areas due to a lack of population density. People in these areas have to drive around as using public transportation is pretty inconvenient and may not even be a thing. For example its entirely possible that the only public transportation is a single bus that goes to a train station in a different bigger village/town.

​@@VectoRaith She must've either said that in error or used the term in the narrow traditional sense, i.e. in reference to major organs. 内臓 is a term conventionally usually used for the larger organs inside the abdominal and thoracic cavities, i.e. the heart, spleen, lung, liver, kidneys, digestive organs and the bladder, due to the influence of Chinese traditional medicine (these are the major zangfu (臟腑) or 五臟六腑), so I imagine her grandmother could have used the word in that sense when explaining the situation to her. Either way logically the only thing that exists as a set of four within the body that can have one part removed with minimal issues is the parathyroid gland. The corresponding surgery would be a partial parathyroidectomy. There's really nothing else that would fit the bill. Especially since she said it's not easy to explain, which indicates that it's not a procedure most non-medical people would be familiar with.
