Thank you Very informative. Would like to discuss with my Doc what you noted early about having a more-supportive or larger cage...ALIF and LLIF. Any links to additional info on those and additional remarks you made after those points. Thanks in advance. Kevin

What category of sports types would you place a Master/Senior (Male 66yo) who has competed at high-regional and respectable-natuonal level with respectable results and medaling that competed in ALL R&F events including Decathlon, Heptathlon, Pentathlon, Distance, Hurdling, Steeplechase, all Jumps and Pole Vault, and All Throws. Also Golf, Triathlon, Biathlon, and Basketball on a more-recreational level? Nov 2016 rear-ended collision halted nearly all activities but lifting and easy runs occasionally. Developed moderate to severe multi-level SEL after putting on over 60 lbs (~177 to 239+). Surgery options in discussion in Phila PA in mid-June. Mindset is COMPETING IS BEST MEDICINE REGARDLESS OF LEVEL.

What is prolotherapy? NOTE: @17:40 - Prolotherapy is whacking glucose everywhere in the top and botton part of the joint and on the conjoined tendons and pubic surfaces in the front Patient Satisfaction: @18:00 - 3x Injections, 50% glucose, per site - 40% people had zeo pain and nothing else had to be done. NOTE: 2 of these people required a top up at 9 years. - 30% are much better to the point with a bit of soft massage and physio to keep things under control - 20% pelvis was stable and things didn't change - 10% where Prolotherapy did nothing - They did have 1 Infection which was a disaster.

FYI - Treatment - Prolotherapy - discussed @14:00 Prolotherapy Treatment - 3 Injections - 6 Weeks Apart - Bruce uses 50% Glucose and puts 12ml in every area. Other Radiologists around Melbourne only use 15% and put 1ml in. This is NOT Prolotherapy! So a good question to ask your pain management practioner if you're considering prolotherapy 1. What is the Glucose percentage in your prolotherapy treatment and how many ml do you put in each area. 2. How many injections do I need in order to confirm prolotherapy is working and how many weeks apart?

Amazing discussion. Planning patient specific alignment targets are the key to ending the cycle of failure/revision. Reliably achieving that planned alignment is the next bear to tackle. It's clear the surgeons who continuously work to understand and improve alignment data. Great question at the end: how do we engage and/or capture those degen surgeons who see PROM improvement immediately following a single level spondy fusion, but maybe don't see all the revisions coming back 3, 5, 10 years later?

The future of spinal surgery. No more butchering the spine with indirect decompression and fusion. Now real spine surgery endoscopic. This is an orthopaedic procedure.

I was diagnosed with Tethered Cord Syndrone (Occult). I was born with Spina Bifida Occulta. The doctor that delivered me saw the signs of something he was not knowledgeable about. He told my mom it would resolve itself. This was 1950. I had symptoms as a child but my parents had no clue. I was limited in the sports I was able to play. Ended up taking swimming. Fast forward to 2016, had successful surgery. Was just diagnosed two weeks ago with re-tethering! So at age 73, I am looking at another surgery because I have symptoms that are worsening.

Hi I too am diagnosed with thethered cord but detethering surgery was performed around 10 yrs ago ,later after 10 yrs I found out that the spinal cord has retethered with worsening symptoms like difficulty in walking, what kind of worsening symptoms did you notice which made you to look at another surgery.

Pure non sense surgery i feel. Whats causing the instability?? 2 things 1) atlas and or axas out of alignment 2) ligament laxity causing hyperbobility. Treatment Repair, grow thicker and stronger ligaments so it makes them also stronger to stabilise the top two joints using prolotheropy, prozone therapy, PRP or stem cell therapy. 2) adjust the atlas/axas or which one is out of alignment. Then your cured and the symptoms will go as your body will heal.
