Thank you so much for this explanation. But what if during assessment, you enquired of pain from patient, and patient said there was no pain. How do you score the pain post operatively in your planning ( bear in mind in the planning scenario, no pain score given)

In this case, you wont be able to quantify the pain. If you decide to use pain as an element, then state that Daisy is at risk of pain following surgical operation so that you need not to state the extent of pain

Thanks vincent, are you saying we should start with the observations first e.g bp, pulse , temp etc before the neurological assessment? The GCS chart statr with the spontaneous eye opening, verbal response before the vital observation. What do you suggest? Follow the chart systematically or start with vital signs observation

I kindly want to ask this: supposed I have a scenario during assessment where NEWS2 score is 0 and monitoring frequency per policy is supposed to be 12HR, can I now say to the examiner that I will monitor patient 4HR because he’s in the hospital instead of 12HR especially for a fall and fracture scenario which patient is for a surgical procedure?

Yes, that will do because the 12hr is the maximum time. Lets say the NEWS is 0 and patient is in pain, you may monitor them every 30minutes because of the pain
