I don't know why anyone's not seeing the elephant in the room here - Anna is suing for medical emancipation and still she cannot name the one reason she might perhaps have had for it, and she cannot even tell this reason to herself because she pretends she is doing this for her sister. She's questioning the basis on which she is her parents' daughter; she thinks she only exists because she can donate organs and tissues to her sister and she wants the relationship to be put on another basis besides her just being a source for replacement parts for Kate. She also wants to know whether she is also being loved as a daughter, for herself. But her mother clearly shows here that this is not the case, she has no right of existence outside of being a donor. But she cannot even tell her mother that because on one level she has indeed internalized this. And it is rotten of Kate to demand of Anna to go through this fight because on one level she expects Anna to fight her battles for her, just as she accepted her organ and tissue donations. That's why I also dislike the ending of the book so much because in the book Anna ends up braindead after an accident and despite having won her lawsuit, she is harvested for the kidney that Kate needs. It's as if she's being punished for questioning the reason for her existence. I don't know whether it's ever said in the movie because I watched that movie such a long time ago that I do not remember all the details, but in the book, it is mentioned that Anna's name is short for her actual name, which is Andromeda, the maiden from Greek mythology who is sacrificed to punish her mother for her vanity. They should have named Sara Cassie, short for Cassiopeia, the mother of Andromeda.
