I think doing weighted exercise for the neck or doing any types of push and hold press that puts tension on the neck could help (not 100% sure, ask a physio about how to treat a cervicogenic headache)

I’ve had this problem for over 20 years, debilitating over 10. Many physicians do not understand the difference between migraine, cluster, cervicogenic “Arnold’s neuralgia” and a fever type headache. They can therefore misdiagnose. I do now understand it and how to manage it. This video is excellent and the 5m model very well summarizes how it can be treated. The most important red flag for a cervicogenic headache is that there will often be neck pain and tightness (spasms, tension, knots) in the trapezius and mid scapular areas. Pain can radiate over to top of head, behind eye and below ear lobes. It can occur after a whiplash type injury and be aggravated even after many years. There can be upper spine (e.g., atlanto-axial) instability and with this often comes dizzyness, brain fog , sleep disturbances, memory and focus problems, and many other symptoms that are not pain.

I am 45 and have suffered with Cervicogenic migraines for around ten years now. They only happen on the left side, come around the rope of my head and stop behind my left eye. I have been through several neurologists and currently have a neurologist and a neuro surgeon. I had a C5-C6 fusion last April that promised to rid me of the migraines once and for all. They are not gone. Since April I have had several nerve blocks, IV steroid infusions, Botox and PT. Mine are very positional and are triggered by sitting and laying down. I currently get 5-7 a week. My life is nothing what it used to be. I feel like the doctors aren’t listening and don’t care. I don’t want pain meds, I want to know the trigger to work on it and eventually fix it. I balled my eyes out in the nueros office this morning… just asking her “why can’t you help me”? If you have any suggestions they would be soooo much appreciated. 🙏🏽🙋🏼‍♀️🤍

Doctor you nailed it! My neck was back forward about 30 years ago and a construction accident. At this point I started getting a lot of neck pain accompanied by headaches in the back of my head. I will talk to my doctor about dish and the C2 or C3 blockers. Thank you!

I had this tipped headache few months ago.I done some exercises for neck and use some tips from your video about Mattress & pillows .Now pain gone.so thank u for your use full video. 🙏🌹

Dear Dr. Furlan, I was diagnosed with occipital neuralgia 20 years ago and suffered ~20 migraines per month. Nerve blocks did not help. Then I discovered simple neck exercises which reduced my migraines to 10 or less per month. However, my beautiful, healthy, slim 50 year old daughter died of a freak heart attack in October, and I have almost daily migraines since then. Nothing seems to work, the triplan, the nsaids, the exercises. I guess time will be the only cure. Thank you for helping us with your detailed lectures on headaches.

I hope your headaches have improved. Have you tried a migraine cap that you put in the freezer? I use the Halos one, good relief while it’s on.

Is this likely to be occipital neuralgia or another type of migraine? A tight aching neck and kind of prickly/fizzy/sunburn feeling at the base of the skull, like you’ve brushed your hair too hard or have brain freeze. This can creep up the back of the head, over the top & into the temples but mainly stays at the base of the skull. It gets steadily worse and starts to throb at the occipital bone & turn into a squeezing pressure headache. Sumatriptan makes it worse. After a few days of this, I get the most severe nausea that can last up to 2 weeks. I’m also fatigued, weak, loose bowels and can’t stomach food. I don’t have the electric shooting pains or eye pain often described with occipital neuralgia. The extreme nausea is the worst part of it for me because no anti-emetics touch it.

Can we perform the neck exercises and massages in the acute phase of pain right from first day of symptoms?will this not exacerbate the pain?

Hello doctor Andrea, awesome video as always. Can you tell me if there is something that is natural I could take to help heal a severe scaitic nerve pain! I have this pain and it is so painful. My doctors prescribed something but it is not helping! I don't want any drugs that is stronger! I would love to know if there is something natural for pain and inflammation! Thanks in advance!

Thanks a lot for ur information, few days back in the month of July I was having severe headache,even my jaws were paining I was trying to figure out frm where the pain is originating,I massaged and pressed the two sides of my neck and I felt relieved ,I think Iam also suffering frm cervicogenic headache. @@DrAndreaFurlan Thanks for the reply. If the patient has a chronic cervicogenic pain , do they not develop a tolerance to baclofen or robax ? Also what is the best nsaid in your opinion for this kind of pain. Again, not seeking medical advice just for my knowledge (23 Canada/Ontario)

Great video. Thank you doctor. I wake up with headache almost every day. I start front back of my head, above my neck, then travels to the front and my eyes. Sometimes I get sick from the pain. One of the drs I saw recommend botox in my scalp. What are you thoughts on that? Does it work? Thank you

Suffered from cervical facet syndrome and cervicogenic headaches for over 10 years. Have not heard about amitriptyline till watching this video. How does it help? What is the mechanism of action? Thanks.

It can help block some or all the pain receptors before they reach the brain. It helps me but isn’t a cure all, which I’m desperate for

I suffer from horrible cervicogenic headaches. My doctor feels it is from a pinched nerve. No idea what started it. Epidurals and nerve blocks did not help. Chiropractor help has not worked. Meds just take a bit off the pain and mood swings.
