There have been many comments and questions about why this situation was classified as cheating. 1. The issue is that the questions that were studied (memorized) by the students who cheated came directly from Step exams. It’s clear policy from USMLE that all test takers agree not to take or distribute test questions. This is because some test questions are going to be the same from one exam to the next. 2. This situation is different than studying practice questions from UWORLD, for example, since the questions from UWORLD are sample questions designed to help you learn the material, not direct copies of questions from the real exam. UWORLD designs these questions rather than taking them word for word from the exam. 3. There’s a big difference between memorizing something and actually understanding it. The entire purpose of USMLE Step exams is to ensure a certain level of competency in US doctors. Memorizing stolen questions and answers might result in scoring well, but it doesn't result in medical understanding. This is why USMLE is so strict on their rules about no one sharing the questions they encountered on the actual exam. Bottom line: Utilizing practice questions from reputable sources that aim to teach you the material is completely okay. No matter where your practice questions come from, aim to learn the actual material, not just the exact wording of the question. And don't buy practice questions from questionable sources.

The USMLE wasn't surprised. It's why their system for catching folks is so sophisticated. The cheaters took a risk and and it didn't pan out. End of story.

Yeah, I think the way they do the SAT and PSAT where tests are available for only a day or two before redoing to prevent this type of cheating is very important.

To my personal experience and other I have viewed recently, we are misdiagnosed by physicians. It usually comes to central reasons, that's in the form of doctors disbelieving our complaints, and stating things like we are exaggerating or delusional. Thus, we need mental health cheek instead. Even if we suffer pain and continues symptoms. One patient said towards this, that doctors may dismiss our words cause they don't match the textbook. They don't feel to "adventure" outside of the norm. Rightnow, I don't have a clear request or question regarde this, but I wish that all patients receive a fair diagnosis and treatment 🖤🖤🖤 Also, I wish to open a discussion about this subject. Thanks 🙏🏾

PYQ or Previous Year Questions is a standard learning tool in most exams in India snd neighborinf countries. It will be a difficult sell to convince people that these are illegal. Quite silly for the exam body to use a database and not prepare fresh questions. They have a year for that. Poor capabilities.

It's a standard learning tool in every country. But the reason for looking at previous exam questions is just to get an understanding of what material will be on the test so you know what subjects to study. USMLE wants you to understand the concept not the answer. They want you to know WHY question 5 is A not just that question 5 is A.

We dont even have these lazy repeats. In our exam its just the concept which is repeating ; and not the entire question with exact options. Considering amount of resources this organisation has it should have atleast bothered to shuffle and parphrase the question at bare minimum

They are illegal. They don't need to be convinced. They need to not cheat and play on a level playing ground with the others. Those guys who have cheated need to have their right to apply to residency in the US revoked. This is a Jordanian speaking. one of the countries that are apparently involved in this shameful practice.

it is so obviously cheating. It's not "practicing" using potential questions. It's quite literally just memorizing the exact questions that are going to be on the test. 90% overlap between the document they were using and the actual exam is ridiculous.

I will say that, as an IMG myself, have heard about these WhatsApp and Facebook recall groups from many other nationalities, not just Nepal. Perhaps their operation system has not been as effective as the Nepalese one and that's why they have not been caught yet. To be clear, I am sure this is only being done by a tiny percentage of IMGs, but I am confident that these recall groups are more widespread than it seems and that this scandal is just the tip of the iceberg

Not a medical school prof but do teach biochemistry and related subjects at a couple Colleges and can say that there are TONS of methods that can be used for cheating for tests, assignments and lab reports and they are most commonly used by international students in my experience. Places like Studoc, and CourseHero are littered with assignments that were submitted by previous students and I’ve even caught current students submitting their lab reports to said websites and the two mentioned are just a small number of the overall amount of them. Couple that with some administrations that either 1.) Don’t care about stopping it or 2.) Don’t want to clamp on it along with the ridiculous amount of time that needs to be done by profs to properly document the cheating is nuts given everything else that needs to be done.

In the Australian physician exam (racp), knowing the recalls is expected. In fact, the examining body even states that 50 out of 170 questions are recalls. Without recalls, it would be almost impossible to cover the material required to pass the Australian exam

@@yabbamita yes, and there will be a further 10-20 questions which resemble previous questions but may test a different aspect of the concept. In the past, those 50 questions would have no marks awarded and be used to set the pass mark, but now they are counted in the final talley. Even then, the pass mark is 55-60%

same with the Saudi exam lmao. the way you study is pretty much via recalls and it's not shamed upon. everyone does it and I don't even think its "wrong" in the sense that there are no rules saying you're not supposed to do that. this is probably the case for most countries except the US and Canada or something

When I gave my USMLEs, the world of IMGs was very tribalistic... Arab IMGs would form their own group, Pakistanis, Nepalis, etc. I used to wonder, how come alot of Pakistanis, Nepalis, Arab IMGs study for like 3 months and get scores like 250-260-270?? I mean, the Law of averages and standard deviations should dictate against it. Then when I gave my step 2 CS, I realized how. All IMGs would book a specific hotel, I ended up inadvertently doing so too. And those who gave the exam, would come back and give a recall of all the cases they saw. That's when I realized that Arab, Pakistani and Nepali IMGs (maybe even Indians too, although I wasn't aware of it.) Had their own recall groups for Step 1 and 2CK. They had just learnt how to game the system really well. I think that this is just the start. Alot of IMGs are perhaps going to have their scores invalidated in the future.

What I am curious to see... is if it would have mattered. If cheating on a standardized test would have created less competent physicians. I've always been curious about this. Many people at this level of knowledge cheat out of insecurity and fear, not from lack of knowledge or aptitude. I've always have an interesting perspective on cheating at these levels of academia and I'm genuinely curious of the non-ethical impacts it may have... or not have.

how well you do on a test beyond a certain threshold will not demonstrate competency or lack thereof. these standardized tests have more than one goal ie they don't just test your knowledge. they help in stratifying people for residency among other things. if you really want to test someone's clinical knowledge and competence in dealing with situations in a clinical environment then an oral exam in the form of realistic cases is the way to go. you remove the luck component of guessing present in multiple choice questions and remove the bias of having good test-taking skills. cause it seems that the medical world cares more about test-taking skills rather than actual competence, in my humble opinion.

Canada requires being resident visa holder first and then a residency match. Usually possible only for spouses of other residents. IMGs avoid the country

No shit it's cheating. Buying a 100 page pdf of stolen exam questions to memorize is certainly cheating. Disclosing exam questions to other people who have not taken the test yet is also cheating. how is that hard to understand.

With this many people being alleged to have cheated, it does kinda make me think that this isn't really an issue of cheaters, but of a system that incentivizes cheating. So I'm a little worried that people will take away the wrong lesson here, and just spend more effort detecting cheaters/implementing anti-cheat measures, without really examining the root cause behind why so many people decided to cheat.

These tests play an enormous part in being selected for a specialty then later for a residency program. Foreigners come here because the pay is a lot higher than in their own country. If they don't match in both of those areas, they may be forced to return home. Each year, we graduate more med students than there are residency positions. I don't know why anyone wants to do their medical training in the US. We used to produce great doctors before switching over to producing predominately bad ones. In med school, they're blasted with a firehose of info that's impossible to retain without perfect recall. They're trained to take tests. At graduation, they haven't acquired the skillset needed to be a generalist or any other kind of doctor. In residency, they're grossly overworked, chronically sleep deprived, and greatly underpaid. It's a 4+ year indoctrination camp where they lose their critical thinking skills and independent thought to become an enslaved bot that follows established algorithms.

I think they should just accept the fact that there are always going to be people that use past exam questions, and make sure that proper systems are put into place to level the playing field. Just make those questions public like the IGCSEs. Make it known that some questions are going to be reused.

If recalling the past questions and memorizing the answers is cheating, USMLE should invest some resources onto making completely new set of questions for each and every individual. Just blaming my country Nepal isn't suited you very well. We people in South Asia has tendency of studying past year questions from the beginning and that is what we used to do in every exams either USMLE or PLAB or NEET or MEC.

The way USMLE handled this was the worst. 1. They knew about other countries cheating yet singled out Nepal in a post in their website. 2. They gave out the scores THEN took it away a day before match week even though they claim their system detects anomalous patterns. People usually apply to 50-100 programs each costing 29 bucks. So on average already spent 1500 per person. So USMLE scammed the students by not letting them know of the invalidation sooner. They probably earned more than 200k+ on it 3. The whole exam is pretty shitty anyway. Multiple choice question to test a doctor's ability (mind you these are MBBS doctors who have already seen patients and taken care of them). I see a few comments here talking about how they don't want cheaters as their doctors. Have you seen the US healthcare system? it's all politics. If you've ever been to an emergency room in a hospital, you have to wait hours before the doctor shows up. Dare I say this is so much easier in europe and even asian countries. if you're really comparing a doctor's skill, look at it as a whole and not just their ability to take some shitty MULTIPLE CHOICE exam some FOR PROFIT PRIVATE organization comes up with. The study materials like UWORLD, or that Conrad Fischer is selling, is question bank. As of today the cost for 90 days is $439. This is what they have in their site: The Trusted Leader for USMLE® Step 1 Preparation 3,600+ questions at or above exam-level difficulty So they change a few variables and the patient's name, age, scenarios than the exam. and that is somehow legal?

A question being worded in a slightly different way shouldn't throw you off when writing an exam. This is the problem with memorizing exact questions and answers instead of learning the material. But the larger issue here is that exact questions were stolen from USMLE exams, which is against the rules med students agree to before taking the exam. USMLE needs to address this for sure, and having more unique questions is a start, but it's still on students to maintain integrity throughout medical school and as a practicing doctor.

how is it cheating though? they did not slip in phone or pass out answers to each other while actively giving the exam. If USMLE was really expecting med students to understand the concept, they would make use of situation based questions and modify them every year. Every student prepares for competitive exams using previous year questions to recognise the trend and pattern of paper. I dont see how exactly the students were at fault when the federation chose to take the easy way out.

It's cheating because they got an un-fair advantage. Nepali scores were so high in comparison to other countries, even the US because they are basically memorising questions, rather than understanding the material. Which is important for FUTURE PATIENTS. If you can't get this in your head, then maybe you should leave medicine.

@@dnnnforfordnnn7838 hey, i am not a med student yet but in the process of becoming one. I read up online about it a little more to educate myself about the scandal. I realised that use of recalls can be a very big slap in the face of honest exam takers as it goes word to word without any explanation. However, if this is something which has be reccuring for years, i hope we can agree on the fact that USMLE did set itself up for this day. I highly discourage cheating the system, especially when you are dealing directly with people's livelihood and would like to correct my previous comment.

@manya1535 USMLE changes the pool of questions every year for their exams and they do that officially. Out of the 10,000 questions pool, Step 1 has 280 questions which are randomly generated. Out of those 280 questions, not all questions are repeated but some questions do repeat for different test takers in the same region. So giving out the questions and answers to others before the question pool changes is definitely cheating. These are not "Previous Year Questions" which we here in India practice. Hope you get the point.

dr jubbal hope u read this im planning to give my usmle step 1 this year on September .. does this mean that the Q s this year are gonna be different ? so will taking an NBME and free 120 tests be as accurate as how they used to be in being likely of the same difficulty as the step 1 exam? and will the NBME also be updated ?

if the USMLE just answered the letter İ have sent them 6 years ago and changed the system ın the way İ have suggested all of this wouldnt be needed and actually even the extreme studying behaviours will not be needed anymore

Even if the Usmle used previous year questions, why can’t they put some effort into jumbling up the 5 answer choices and change the question a bit? I don’t get it. But the cheaters do have incredible memory to be memorizing hundreds or thousands of questions and answers. Rather if they put that same effort into learning the concepts then it would’ve benefitted both them and their future patients.

İ guess you dont actually know that 60% of medical books and the exam questions are not related to the actual medical practice İ am not saying cheating is ok but to imagine reading books on books and thousands of pages and memorising them entirely .....it is sadistic and actually outdated medical doctors dont need to memorise information anymore they should be able to have basic medical knowledge and know how to use an evidence based approach and reach the best uptodate treatment with organised and time management of multiple factors and cases ... but that a different discussion İ have actually submitted a request to change the entire examination process for all medical exams to represent accurately these features and suggested how ....but my letters obviously fell on deaf ears

If I take step exams in Jordan but I'm a Syrian medical student (Cause there are no exam centers in Syria), will this be a bad point for me in the future? Or is the country mentioned in the video based on where the student studies?

as a nepali pre-med student who had no idea abt this scandal i am very disappointed, but i think it does root from pre-med entrance because PYQ are considered more important to study and practice as they give u insights on how the question are made and ofc we dont have official PYQ ppl gather and try to recall what questions had come after the test , which is not considered unethically (to my knowledge dont quote me on this) so, i am quite surprised to know its illegal ( maybe im the only ignorant one here) idk

There is a big difference between discussing high yield topics, and giving/having the exact questions from past years. One requires you actually study the material, while the other is memorization of questions/answers. You will see their analysis shows people do NOT know the material which was not leaked, indicating knowledge deficits.

For people that don't understand why this is a big deal, this test is designed to show that you understand a concept not that you can memorize questions. You need to understand WHY you are picking a certain answer, it's not enough to just know what the answer is

You are a American? If so you need to graduate as a PreMed, others countries are different (Here in Brazil you can go to Med School after High School, but is very difficult to get in a good one, so is usual to study 1-3 years before getting approved in the ENEM)

That is not cheating. Since when is memorizing past papers cheating. They are smart about taking these exams. Either create more questions or have them undergo oral exams. And even with that you can remember an examiners Favorite questions😂 The big scandal is invalidating people’s test results here

So... Have those that did cheat caused harm when they were practicing? Because if not; then what's the serious problem? If they are conscientious doctors who make sure to stay up to date with current medicine and do everything they can for their patients, why would it matter what they got on a test and how they got it? I can see it being a problem with securing residency (typo in the video I believe; "resideny") with it being so competitive but to remove licenses because of a test result?! Seems wild to me. Maybe this med school exam is different but in my experience all exams are useless (this is in my opinion and I say that being a well educated person who has sat many, many exams)

If they’re willing to cheat on such an important exam, in what other areas are they willing to cut corners? Maybe they haven’t provably hurt anyone yet, but they have shown a willingness to compromise their ethics in a significant way.

The exams are there for a reason. It's literally the lowest bar to measure the lowest amount of competency required. If a doctor is unwilling to spend time to actually learn the actual medical concepts under external pressure and resort to cheating, then what makes you think they will catch up to the current medical advancements and study on their own with no external pressure? That's like saying the student who is unwilling to run a mile in gym class is the one who is training for a marathon in their own time... If you think about it logically, It just does not happen.
