AMCI is one of the most informative coding website I found. I need more training in cardio, digestive, radiology, and medicine. I failed my test by two points and radiology and medicine was my lowest scores. A review in these areas will be very helpful.

you have the same example of the 57- year old in another video. Except they said to use +92921 because the instructions on the previous page say “PCI performed during the same session in additional recognized branches of the target vessel should be reported with add-on codes” and the angioplasty was done on a diagonal which is a branch of the Left anterior descending. Which answer is right?

I think the Angioplasty performed in the Diagonal coronary artery should have been reported with an add on code 92921 bcoz a primary code is reported once per vascular family. Since Diagonal is a branch of LAD family and Base code 92941 is already report, moreover 92921 is allowed to be used with 92941 as per Parenthetical guidelines. What do u think?

A Primary code is reported once per treatment zone. Angioplasty is bundled into the higher treatments (e.g., stents). Per guidelines page 725 of the 2021 CPT manual "PCI codes are built on progressive hierarchies with more intensive services inclusive of lesser intensive services." AND when you read the code language carefully, it says that angioplasty is bundled. I hope this helps.

How could we code 92920, when angioplasty is bundled with other codes, it's written also in 92928 with coronary angioplasty ,artery or branch both included in that...

Correct, if Angioplasty is carried out in the same vessel, however if a different vessel it is not. In our case, only Angioplasty was carried out in the Left Diagonal Branch, and it was therapeutic NOT diagnostic. I hope it helps.
