The truth about Accutane for acne
.
Accutane is also known as Roaccutane or Oratane. The parent drug is known as isotretinoin, a form of vitamin A. This drug address all four causes of acne including-
Reduces skin inflammation
Normalises turnover of skin cells in the the oil gland
Reduces oil production, also known as sebum
Reduces the population of C. acnes, the bacterium implicated in pimples, pustules, blackhead, zits, whiteheads & cysts
Dermatologists have been treating cystic acne, hormonal acne, as well as recalcitrant acne with this medication for four decades. A treatment course varies from 4 months to beyond 18 months. Cessation of this medication is drawn when acne has resolved completely for a period of 2 to 3 months. This medication places acne in remission in about 60-70 % of cases. Patients who have severe acne, a family history of acne, or trunk acne-bacne may require additional courses of this medication. Typically the dose ranges from between 5-80 mg per day. Retinoid resistance is very rare.

Accutane is also an off label treatment for rosacea, seborrhoea or oily skin-scalp, as well as other skin conditions such as sebaceous hyperplasia, psoriasis, seb dermatitis, and even viral warts. Side effects are frequently seen, and this medication will require patient monitoring.

Side effects include, but are not limited to the following:
Skin side effects include: Dry skin, xerosis, dry lips, cheilitis, photosensitivity, cuts, bruises, ingrown toenails, eczema
Musculoskeletal: Bone pain, backaches, muscle pain, stunted growth, osteoporosis and decreased bone mineral density
CNS: Headaches, blurred vision, loss of night vision, intracranial pressure changes, mood swings, anxiety, depression
Reproduction: Foetal anomalies of the heart, limbs and CNS
Blood and blood test anomalies: include elevated liver function tests, elevated cholesterol, depressed blood counts

I was also on accutane 7 years back and my skin has changed for the better! My skin is very less oily. However, I wonder sometimes if it is a permanent cure.

My teen son took it, and it changed his life. The scarring has diminished over time, to the point where you can't even tell. It's impossible to communicate the improvement, not only in the acne, but the self-esteem of the child.

@@dj-oz2qv I think it is different for everyone, I went to the dermatologist for the 1st time march 2019, stopped going July 2019 after being on tazarotene, doxcycline and clindamycin for a few months. I stopped using those treatments and my acne got bad again and I just went to the dermatologist today again and he's gonna put me on accutane. Im 17, skin isn't severe so I think it depends

Pretty much anyone who’s ever had serious acne and took accutane considers it a life-changer and a miracle drug. P.S. Now that there are so many responses to this, I’d like to add to always go to a dermatologist for a prescription and don’t buy it online without one. It can cause some serious side effects and liver failure. The dermatologist will order blood tests to monitor your blood panel and keep you within safe limits.

Couldn’t agree more. Had dryness for about 2 weeks in my first month and felt pretty weird the very first two weeks and have had zero side effects since. Best thing I’ve ever done.

My only regret with this medication was not taking it sooner before I scared. Listen to your derm, moisturize, put on prescribed lip balm which they often give you a free test kit, and bye bye acne forever. The key point here is have a good relationship with a dermatologist you trust. Knowledge and understanding is everything

@@alexmajor3886 Just note it can take a short while for the the medication to begin working. Dry skin will happen quick, but can take a bout a month or more for pimples to slow down and stop. Cut Dairy & sugar out your diet, wash your face lots with the derm recommended gentle soap, make sure you have clean pillow covers. You acne will go quick

True there should be friendly relationship between you and your dermatologist which is rare. Most derm acts as they don't need to inform us anything because they know all and we know nothing.

I had SEVERE Cystic acne. Resistant to topicals & pretty much all else on the market. I had given up hope but then I was prescribed Accutane. It changed my life! I wish I would have found it sooner so as not to scar as bad as I did!

My husband was a trial for accutane back in the early 80’s I think. He had awful acne and it saved his life. The doctor was awe struck at how it cleared his tunneling acne up in a month! He has some scarring as shaving for years has reduced a lot of the scars. He’s 55 now and still it never came back.

I too was a trial user in 1983. I took the maximum dose for three months and my only side effects were dry skin and lips. My bloodwork was normal the entire time. I’ve never had any health issues related to Accutane. It literally saved my life because the cystic acne had me considering suicide. Unfortunately I already had some bad scaring by the time the medication hit the market, but the acne never returned.

Isotretinoin works by shrinking the oil, or sebaceous, glands in the skin. This reduces the amount of oil that is produced. When your skin isn't producing excess oil, it isn't hanging around clogging up pores and creating pimples. By controlling the oil, acne is also controlled.

Absolutely love this video. Very informative and above all nuanced. I live in Belgium and was prescribed 40mg per week. No follow up on blood or pregnancy tests whatsoever, no consent needed. I'm hesitating to try it, as I'm struggling with moderate hormonal acne for over 20 years. (I'm now 41.) Birth control helped amazingly, but I can't take this anymore due to a history of breast cancer. I think I will first continue using Differin. But I'm absolutely not opposed to trying Accutane as a last resort.

Had terrible cystic acne as a teenager, and had gone through 3 cycles of isotretinoin to clear my skin. The side effects are nothing to be scoffed at (had terrible dryness, chapped lips, among many other side effects), but ultimately the long-term effects of having cleared my acne on my mental well-being far outweighed the side effects I experienced. As Dr. Lim says, it’s very important to be well-read on the side effect profile - and make sure that you and your treating physician have a mutual understanding of the benefits & risks associated with isotretinoin.

I did it once in highschool and didn't finish the second round but now wish I did. Saw a dermatologist to start testing and for the second or hopefully last round of isotretinoin 🤞 Edit: Found out that I did antibiotics once and tried them for a second time. The side effects I told my derm about were not in causation of isotretinoin but is likely from the antibiotics plus I didn't do monthly tests to check blood levels on the pills.

@@realmigra375 I needed to change some things according to my derm and private doc according to some blood tests so I haven't taken any yet. Also going to edit my last comment because I found out it wasn't isotretinoin it was another of the 3 options. Antibiotics. For moderate to severe acne, you may need oral antibiotics to reduce bacteria. Usually the first choice for treating acne is a tetracycline (minocycline, doxycycline) or a macrolide (erythromycin, azithromycin). Then there is Spironolactone (Aldactone) is a diuretic (a “water pill”), which means it is a drug that removes fluid from your body. It can be used to treat acne in women by targeting the hormonal causes of acne. Then there is isotretinoin vitamin A derrivative which if your a guy you don't have to worry about pregnancy tests, if your a women you have to because fetuses have severe side effects with the drug, but they do monitor you every month with tests to make sure your levels are okay.

I took Accutane for 7 months. Changed my life SO MUCH for the better!!! I dealt with cystic acne, at it’s worst I stopped counting at 60 active breakouts. I now have clear skin & have never been this confident. I had most of the side affects - but did blood work each month so monitor my levels which were all normal. I dealt with acne since I was 13, but once I got 19, it all went south. I couldn’t be any happier now, I wish I started it sooner!

Ahhh this gives me hope! I'm 22 and have been dealing with acne since I was 12. I've had facial acne since I was 12 but once I hit 20 I started getting acne all on my jawline and sometimes my neck. I'm so nervous Accutane won't be a long-term solution for me. Did Accutane make your skin purge at the begging?

I think this was BOTH a very HONEST & INFORMATIVE vlog / talk on this medication . ( Roaccutane ) . There are so many scare tactics on YT , unbelievable !!! The way you explained this drug , it's a drug , and with every drug you have the possibillities of side effects ... Often , no matter what you get prescribed , after reading the information that comes with it , you'd be afraid to take it ! Scare tactics are very powerfull . Im on two months, and really all that’s bugging me is dry lips and pelvis pain. Also, my wounds do in fact seem to be taking longer to heal. Im starting to see my skin clear up already though and I’m pretty excited. Good luck to all of you trying accutane!

It sucked when I was doing it especially in the later months, but I did around 7-8 months of treatment to be safe. Glad I did. Really changed my life. After decades of trying every treatment except isotretinoin, I used it with miraculous results. No more excessive oily skin and maybe a pimple every few months. I had tried it when I was in my 20s and the SE were too great; i.e. cysts in my axillary areas, etc. When I became desperate in my 50s, it was a breeze with no SE! It’s worth it for your self~esteem. I’be been an RN for 23 years and read every advanced article that could find on isotretinoin before taking it.

I was very hesitant to go on accutane because of purging and depression. It made my severe acne worse for 2 months and cleared it all up in about 3 months after that. I was still on it for about a year and my skin has never looked that good since. Miracle drug indeed.

Thanks. This video is a lot more reassuring and informative than the slew of heavily biased and emotionally charged testimonials. This will be my second medication to try that’s linked with Suicide, as I am currently on antidepressants that really gave back my life for the positive.The internet can be terrifying world of coincidence=causation with everyone’s self diagnosis of permanent psychological damage.

I had been taking it for Rosacea prior to COVID, but had to stop due to physician dropping practice for 6 months. It really did help and I have gotten worse since stopping. I was taking 3 10 mg pills a week. What would hep me now. We have moved this that time and I am unable to get in with a dermatologist for 6 months. I know crazy times. I really enjoyed watching you and will continue to do so.

I am currently on a very low dose of isotretinoin and love it so far. I had acne from a very young age, (it started when I was 9 years old) and tried everything else (everything!!!!!) All I can say is that I had more side effects being on the pill (diane). I resisted isotretinoin for years, even when three different dermatologists suggested it to me, but got so fed up with acne when I hit 30 that I thought I give it a shot. So far it has been great! I havn´t had any issues with mood swings, actually I do feel better since being on the drug. Acne itself can be quite a downer as well as physically painful!

I've been prescribed by my dermatologist Accutane (Isotretinoin) a few days ago after consultation and a blood test to make sure its safe for Isotretinoin to be prescribed to me. A few days into treatment i have noticed dry lips and mouth as side affects but so far that is all. Eager to see the results as I've had moderate to severe acne for many years. No over the counter treatment has worked for me so fingers crossed!

I’m on 10mg daily for the past 8 days and the only side effect I’m experiencing is a bit of dryness on the edges of my lips(lip lines and nightmares, Is this normal? Especially because for the first 4 days it was so bad I had bad headache, face felt so dry and tight, even had a swollen face but I didn’t stop taking it as the swelling was subsiding by itself so I figured I shouldn’t stop...and then day 5,6,7 all stopped suddenly and the lips are the only thing that’s left( even them are not so bad). Can I be hopeful that the treatment will work?

Accutane changed my life as a 22 year old with severe cystic acne back in the late 80s (I believe the dosage was 30mg). I did have side effects like dryness and after the 4 month mark, joint pain, but all disappeared after completing the course. Cleared up the acne and not a single spot for 18-24 months. The acne did return but not as bad as the first time. Back to the Derm, for another course of Accutane. Between 22 and 45, I had to be on Accutane 4-5 times. Totally worth it. Been on it 4 times too. I seem to need it every 10 yrs. I come off 9 months ago (a very low dosage) and already have cystic acne returning. I am treating with azelic acid and trentoin as it is only on my face ATM. It helps. I am also supplements and a low carb high protein vegetarian diet. Will I need it again? Ask me in 10 yrs.

Ungvita Creame, it stinks like fish but it is vitamin A in a petrolatum base has prevented scarring from yrs of cystic acne for me. It about $8 dollars in the chemist.

I was prescribed accutane 3 years ago for cystic acne on my chin and I experienced some of the side effects (dry mouth and nose, my hair wouldn't get greasy at all, I bruised easily) and although I still get pimples sometimes it's not nearly as severe as before. Overall I'm happy with my experience with accutane but wouldn't go for a second round.

Cystic acne on skin happens due to hormonal problems. You should get treatment for that.

I start this next month and I’ve never been more excited, yet nervous to take. Already did blood work and pregnancy test. Just waiting for my second pregnancy test to come back negative! I really hope this works for me. I have very bad cystic acne. Hurts to the touch. Very deep, which makes it impossible for me to pop anything to relieve some of the tenderness and soreness. I’ve tried everything under the sun. Been going to the dermatologist for 10+ years and never knew about accutane. Found out about accutane through one of my friends actually so I went into my dermatologist the next week and was approved to start. Can’t wait!

I am 30 but still struggling with acne. I once had consumed accutane when I was 20 but because of my body can't bear the side effects, i stopped immediately. So the acne was coming back. But now, after a couple of years, i consumed it again, and I also practice the whole plant based diet.

I took Roaccutane 23 years ago, on the NHS, and it worked so well. I can't remember the dosage, but from what you were saying I guess it was a very high dosage. It was a miracle drug as far as I was concerned. Literally the only side effect I got was severe sweating, I had to wear an undershirt everywhere so it didn't show!

I was never really scared for accutane, and it’s worked so well for me! Just had some chapped lips, and there was a time for a bout a week that my skin was super peel-y, but other then that it’s been great! My highest dose was a 40mg pill once a day

All I know is that if I hadn't taken the medication 20 years ago, God knows what my face would look like now. Those six months I was on this medication were perhaps the worst six months of my life. The side effects were terrible (my lips looked like they had exploded and were incredibly painful and I lost a lot of weight because my derm had put me on a strict diet as well) but if I could turn back time, I'd do it all over again because it was the only thing that worked for me. My advice: find an experienced derm that you can trust, do exactly as they say, get blood tests done as instructed and stick to the treatment. My skin cleared in six months. Greetings from Greece!

@@nishithareddy8742 Sorry dear. I had been thinking whether to use it or not, but I decided not to use it at the last minute. I'm really scare of the side effect since I can't live without going to the beach, my house is beachfront. I think of a way that I never do before, so what I do is strict diet, no sugar, no diary products, only milk plant based plus regular exercise, I use benzoyl peroxide for facial foam, niacinamide serum, moisturizer n sunscreen. Eventhough I still have my hormonal acne sometimes but at least much better than before, I'm happy that way. I really hope that u can find what's best for u, dear ❤️

My only regret is not going on it sooner! It cleared my face and it’s absolutely great. Yes things got pretty dry but moisturizing was helpful and nothing got too dry or chapped. My depression got so much lower than ever as soon as I started to see my skin clearing up.

Ive been struggling with acne on my face/back/chest for years. It’s grown more cystic, although not extreme, for the past 2. The Pills/topicals I was given haven’t helped throughout the whole journey- if I mention getting on accutane- is it likely that they’d let me? Or do I have to have very severe acne? I feel this would make my life a bit more enjoyable instead of dealing with it for 5 more years :)

Much needed video..i have mild acne but my dermatologist recommended to start off with Accutane and antibiotics which I'm not comfortable with. Hopefully the topical treatments will work. Currently using adapalene 0.1% and benzoyl peroxide 2.5%.

Omg mine has been 5 yrs and 3 courses of antibiotics and 4 doctors (2 dermatologists)! I started accutane yesterday bcs i went to a dermatologist that ive been asked to go to (shes the best in my state) and she asked me to tell her my acne story from A-Z 🥺 no doctor ever asked me that and after a long explanation she finally decided to put me on accutane! I was too happy when she decided that. Im praying that this is our last time having these severe acnes :cc

@@dilahspardi Really? i asked my derm if i could start before the blood test due to wanting to get started asap and he told me no as its some sort of protocol because if they let you start and your test comes back and you have high levels of cholesterol or something its a massive risk, did you go private? mines through NHS but sounds like you got a good derm if they let you on before the test :D i have my test Monday then derm visit on the 4th which hopefully on that day i'll be given a prescription for accutane.

@@iTzCorey supposed to be like that and i initially thought I’d start accutane next month but when i was called to the counter after signing the paper they gave me my accutane and yes i went to private. But my next visit is on the 22nd june next month and my blood test is on 19th june but my derm also prescribed me with antibiotic named amoxicillin. Its okay you’ll surely get it by the 4th!

I changed my entire skincare routine after watching your videos. I also started Retin-A 0.5 strength and have been using it daily from 2nd week onwards. I use it alone on dry skin without any moisturizer and my skin becomes super oily within a few minutes of using Retin-A. Its the 3rd week now and I have not experienced any sort of peeling so I am wondering if it’s even working ! I have even increased the dosage to a large pea size amount. Am I doing something wrong ? Is it imperative for the skin to peel or is the product working without peeling my skin. I am so confused. Can someone share their experience? Dr. Davin, would appreciate comments from you on this. I have pcos and have been on the contraceptive pill for years but it just hasn't worked as well as I'd hope, even with a good diet and healthy lifestyle. Here is my story: I’ve developed cystic acne when I was 27yo. Basically I went through everything and nothing helped me until I changed my diet. When I started Keto my skin cleared out within a month or so. I was using Tretinoin for 2 years and it faded my scars, but I developed very bad side effects in my body. My doctor said Tretinoin is a toxin. I can’t tolerate it. For now I just use my Niacinamide powder with everything in my skin care routine. And I am 42 yo!!!! Just exam your body and follow to it.🙏🏻👋

I had to go on roaccutane twice unfortunately, the first time when I was 16 and the second time when I was 18 on a stronger dose. However, I am still strongly in the pro camp. I had tried all sorts of topical solutions and lifestyle changes but nothing would work. I had been dealing with acne for 4 years prior to starting roaccutane. I initially had so many tiny bumps along my t zone and then it shifted to deep and painful cystic acne on my cheeks with cysts that could last for 2-3 months. I'd dealt with hyperpigmentation and scarring. Wheb taking roaccutane my side effects included purging in the first few weeks and throughout the course, dry skin (of the face, scalp, lips, within the nose) that was in my control to manage with much moisturiser and lip balm and being gentle with fragile areas. I was pedantic about spf and avoided unnecessary sun, so I didn't experience sunburn. After I finished my 2nd round though, my hair loss daily was more than usual, but it stopped after a month. It's so true that when you feel helpless, have tried everything, understand the consequences of the drug and are willing to put in the effort, this can be a lifesaver drug. It's been 2 years since finishing roaccutane and I usually just get a few pimples before a menstrual cycle starts. I hope this video and my comment can put at least one person at ease if they need to resort to roaccutane and are worried about the side effects.

I'm currently on a very low dose (20mg) going on two months specifically for body acne. Every inch of my body is dry and irritated but I can already say it is the best decision I have ever made. I’ve had quite a bad acne and for about 4 or 5 years I was refusing going on accutane because I was afraid of all those side effects, I was afraid it would affect my fertility in the future, etc. Then, I went on accutane anyway and I regret that I didn’t do it sooner. I had no side effects whatsoever (only dry lips), but now I have to deal with bad scarring from not treating acne properly for years :(

Accutane changed my life. Tried everything from expensive to very cheap skin care. Went to my dermatologist for my 13th birthday to clear up my cystic acne on my back. I still have scars but my face has cleared. Managed the dryness by just moisturizers and staying out of the sun. And my skin type turned from very very oily to actually normal! Huge self esteem benefits. Though my mum is convinced that it has stumped my height (not sure of the evidence there)

I am 21 and my two sisters 26, 29 all have acne. I had acne from age 10 and I still have it.... I am considering taking accutane. I have an insane amount of sebum like aloTT. Bugs used to die on my face.... like fruit flies. BUt yeah I really hope it helps me :( I am convincing my sisters to do it too.

I had acne for many years, and put off going on Roaccutane for a long time because of the side effects. My face got to a point that it was too painful to touch and I had to cake makeup on everyday to cover it all. I finally accepted that I needed help. I had a great dermatologist that went through everything with me. I had to sign an agreement not to get pregnant and use double contraception, had blood tests every few months, and after 9 months got to come off it with beautiful skin. I did get some side effects, dry skin and lips and felt depressed by the end of the course. Unfortunately, I've been off it for about 3 years now and have recently started getting more and more pimples. It hasn't been a forever solution for me.

Thank you for the vid. I just began taking 10mg of isotretinoin 5 times a week, and the dose will progressively go down for the next 9 months, adding up to 1.2 grams in the end. This is meant to treat oily skin and back acne. I notice this differs from the most common cases in which the dose begins at 10 or 20 mg and goes up progressively, sometimes to 60 mg/day. Is it likely that my treatment won't be effective due to the low daily dosage? Thanks

I was 33 when I started. Took about 4 months to clear. Had another breakout 2 years later but the 2nd cycle cleared it for good. Wished I had taken it much earlier, could have prevented so much misery.

Hey Davin, I was wondering is azealic acid was safe for men to use daily or alternate days? It can be a DHT inhibitor but when applied topically(15 percent) on the skin but can the effects go systemic, or is the DHT inhibition localised in the skin? How much does azealic acid reduce systemic DHT in men compared to something like finasteride and can this have an effect on masculine traits eg resulting in a weaker jawline? Cheers
